Transmitted herewith is a Resolution approving an Interlocal Agreement between the City of
Omaha and the City of Bellevue, Nebraska for a connection to Omaha's Papillion Creek
Interceptor.
The City of Bellevue has proposed a splash pad and water playground in Banner Park, which is
in an area of Sarpy County served for wastewater conveyance and treatment by the Papillion
Creek Interceptor Sewer. The Omaha Public Works Department Sewer and Environmental
Quality Control staff have reviewed the City of Bellevue's application, designs for the project,
and agreement submissions and have determined that this connection is acceptable to the
operation of the Interceptor Sewer.
The Public Works Department requests your consideration and approval of the attached
Resolution and Interlocal Agreement.

THAT, the following is a legal description of all property to be included in the Sewer District.

(30) thirty days after completion of construction on reproducible Mylar
THAT, all streets in which sewers are to be constructed, have been dedicated to public travel.
THAT. Certified Copies of recorded Easements have been submitted to the Public Works
Department where sewers are to be constructed on private property
THAT, the established grade, as shown on the Profile, has been approved by the Public Works
Department and the Local Approving Authority
THAT, the sewer shall be built to conform with the Sewer Specifications of the City of Omaha,
and any addition or correction thereto.
THAT, I will secure both the Local Department of Health and the Nebraska Department of
Environmental Control approval of the sewer project before the resolution is submitted for
Omaha City Council consideration.
THAT, I have read the standard Resolution and the current Procedure, Rules and Regulations of
the City, pertaining to the construction of sewers, and I shall conform to them.
THAT, a continuous video tape will be submitted to the Public Works Department within (30) thirty
days after completion of all sanitary sewer construction, for which this application is
made, showing the Internal Condition of the Conduits.

and provided the following stipulations, rules and regulations are complied with
This Agreement applies only to direct connections to the Papillion Creek Interceptor Sewer
THAT, the sewer and its connection to the interceptor shall be constructed at the applicant's
entire cost and expense as per approved plan furnished and on file in the Public Works
Department.
Construction inspection of the connection shall be performed by a City Inspector from the Public
Works Department. A certified check for $500.00 will be posted to cover the costs of inspection,
engineering review and administration with the City of Omaha Public Works Department at the
time application for the connection is made
A Performance, Payment and Maintenance Bond, acceptable to the City. shall be filed with the
City of Omaha. Public Works Department. (Bond Forms are available on the 6th Floor
Omaha/Douglas Civic Center).
All work and material shall conform to the Standard Specifications for Street and Sewer
Construction, 2013 Series of the City of Omaha, and the rules and regulations for the construction
of sewers and any additions thereto or modifications thereof
The Applicant shall furnish the Public Works Department with the original or a reproducible print
of the 'As Built" plans of said sewer job, showing all wyes tied to existing property lines, and all
elevations of manholes and pipe, using the USGS elevation datum. The"As Built" plans shall be
in the Public Works Department within (30)thirty days after completion of construction.
The Applicant shall conform with the rules and regulations specified in the Application and the
procedures for private sewer connection to the Papillion Creek Interceptor.
The Applicant shall submit a copy of the contract for construction of said sewer to the Public
Works Department, and said Contract shall, in part, show the unit cost and the total cost.
The Applicant shall show on the as built plans the flow limits of the area served by the
connection, the estimated flow being induced into the existing sewer, the calculated flow capacity
of the existing sewer, and the calculated capacity of the sewer being proposed.
The Applicant agrees not to permit any extensions or connections to this sewer system, even
though provision may be made for the additional flow, without the express consent of the City of
Omaha and the local governmental authorities having regulatory jurisdiction.
This Agreement is made with the City of Omaha as the owner of the interceptor sewer.
Permission of the local governmental authorities have regulatory jurisdiction is also required as
provided by law.

PROCEDURES FOR SEWER CONNECTION
TO
PAPILLION CREEK INTERCEPTOR
IN SARPY COUNTY
(See Attached Flow Chart)

1 The applicant shall submit (2) two prints of the proposed sewer to the City of Omaha
Public Works Department Design Division, 6t floor Omaha/Douglas Civic Center,
1819 Farnam Street, and the governing authority in whose jurisdiction the connection
is to be made — (all prior to making application to construct the sewer and make the
connection to the City of Omaha Papillion Creek Interceptor) for the purpose of
obtaining necessary application form, procedures, agreement terms for connection
and bulk sewer service.
2. The applicant shall present (2) two copies of the completed application (see attached
form), to the Public Works Department and shall also make an application to the
governing authority in whose jurisdiction the connection for interceptor to be made.
3. The applicant shall submit (9) nine copies of the final plan to the Public Works
Department for approval one of which is retained by the Quality Control Division.
After approximately (2) two weeks the applicant receives (7) seven copies back
stamped (Preliminary Approved) to be distributed as follows — (4) four copies to
Nebraska Department Environmental Control; (2) two copies to Local Health
Department; (1) one copy Local Jurisdiction.
4 The applicant shall submit to the Public Works Department, General Service Division
(5) five copies of the agreement for bulk sewage service or bulk sewage treatment
service; the application to be executed by the City of Omaha prior to making any
connection to the interceptor—(1) one copy will be given to owner of sewer.
5 The applicant shall submit the required number of copies of the final plan with City of
Omaha for preliminary approval: to the Nebraska Department of Environmental
Quality in Lincoln (4) four copies, to the Local County Health Department, (3) three
copies, and the appropriate Agency having jurisdiction over the area serviced by the
proposed connection, (2) two copies
6. All of the above approvals required, shall be mailed or given to the City of Omaha
Public Works Department prior to making any connection to the interceptor.
Note: The Local County Health Department retains (1) one set of plans for their
use and return (2) two sets to the Engineer or owner of the proposed sewer— (1)
one of which is given to the City of Omaha Public Works Department and (1) one
set is given to the Local Jurisdiction.
The Nebraska Department of Environmental Quality retains (1) one set of plans,
and returns (3) three sets to the Engineer or owner of the proposed sewer, one of
which is given to the City of Omaha and (1) one set is given to the Local
Jurisdiction.
Copies of approvals by each agency should be sent to the City of Omaha Public
Works Department, Omaha/Douglas Civic Center, 1819 Farnam Street, Omaha.

7. The Public Works Department will submit resolutions to the City Council granting the
applicant permission to construct the private connection to the City of Omaha
Papillion Creek Interceptor Sewer and enter into the Sewage Service Agreement.
8. The Applicant pays required connection fee of $500.00 to the City of Omaha Public
Works Department.
9. Applicant notifies Public Works Department Sewer Maintenance Division 48 hours
before construction begins (444-5332).
10. Applicant notifies City of Omaha Quality Control Division when construction is
completed to flow monitor and set bulk service charges (444-3908).
11. Applicant sends as-built plans to City of Omaha Public Works Department 30 days
after completion of construction.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, the City of Omaha owns and operates a sanitary interceptor sewer serving a
portion of the jurisdiction of the City of Bellevue, Nebraska, known as the Papillion Creek
Interceptor; and,
WHEREAS, the City of Bellevue has made application to the City of Omaha to make
connection to this sanitary sewer in order to drain water from the proposed splash pad and water
playground in the area known as Banner Park ; and,
WHEREAS, the City of Omaha has reviewed the application and accepts the connection
according to the terms defined in the attached Interlocal Agreement, which by this reference
becomes a part hereof.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF OMAHA:
THAT, as recommended by the Mayor, the Interlocal Agreement between the City of
Omaha, and the City of Bellevue, Nebraska allowing the connection to the Papillion Creek
Interceptor for the purpose of draining water from the proposed splash pad and water playground
to be located in the area known as Banner Park, is hereby approved.
