Transmitted herewith is a Resolution approving an extension of a contract with Heritage
Environmental Services, LLC for OPW 51340, being Household Hazardous Waste
Transportation and Disposal. The original contract was approved by the City Council on May
20, 2014 by Ordinance 39997.
The original contract provided for an initial term of two years with one annual extension at the
option of the City with Council's approval. This extension will be in effect from September 1,
2016 to August 31, 2017.
The cost for the continuation of this contract with Heritage Environmental Service, LLC will be
at the original bid prices for OPW 51340 and will be paid from the Chemical Disposal Fund
21129, Household Hazardous Waste Facility Organization 116925.
The Public Works Department requests your consideration and approval of the attached
Resolution.

On behalf of Heritage Environmental Services, LLC, I am pleased to acknowledge
Heritage's intent to formally accept the offer the extension as described in the Contract
(Special Provisions, page 4) to commence on September 1, 2016 for a one-year period
that would expire on Ausgust 31, 2017.
Heritage has been managing Household Collection managing hazardous, non-hazardous
materials, as well as providing other services for over 20 years. On behalf of the entire
Heritage team, thank you for the chance to continue to provide these services to the City
of Omaha UnderTheSink Collection facility. As we have demonstrated in the past, going
forward we are committed to continuing a safe, positive and productive working
relationship.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, Heritage Environmental Services, LLC was awarded contract OPW 51340,
being Household Hazardous Waste Transportation and Disposal by Ordinance No. 39997,passed
May 20, 2014; and,
WHEREAS, OPW 51340 provides for an initial term of two-years and includes
provisions for one one-year extension at the option of the City; and,
WHEREAS, the Public Works Department recommends the extension of OPW 51340,
through August 31, 2017.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF OMAHA:
THAT, as recommended by the Mayor, the contract with Heritage Environmental
Services, LLC for OPW 51340, being Household Hazardous Waste Transportation and Disposal
be extended through August 31, 2017; and, that the Finance Department is authorized to make
payments to Heritage Environmental Services, LLC at the original bid prices from the Chemical
Disposal Fund 21129, Household Hazardous Waste Facility Organization 116925.
