The City Council of the City of Omaha approved a three-year contract with Corvel Enterprise
Comp, Inc. by Ordinance No. 39703 on June 18, 2013, which contract included two one-year
options to be exercised at the City's sole discretion. Corvel Enterprise Comp, Inc. provides
Workers' Compensation Administration services at an annual cost of no more than two hundred
eighty thousand ($280,000) per year for the original term of the contract. By exercising the first of
the two-year option, the option contemplates that all flat rate fees are subject to a three and one-
half annual increase from contract inception.
The Contractor has on file a current contract compliance report form CC 1. Funds for this purchase
will be from Fund No. 11111, Organization 118017.
Approval of this Ordinance exercising the first of the two one-year options in the contract with
Corvel Enterprise Comp., Inc. is respectfully requested by the City of Omaha Human Resources
Department.

AN ORDINANCE to approve exercising the first of one year options in a contract between the
City of Omaha, a municipal corporation, and Corvel Enterprise Comp, Inc.; the initial
contract was for a three-year term and was approved on June 18,2013,at a cost not to exceed
two hundred eighty thousand dollars ($280,000) per year, and the option contemplates an
increase in the flat rate fee of three and one half percent per year dating from contract
inception to provide workers' compensation third party administrative services;such fund to
be paid from Fund No. 11111,Organization 118017;and to provide an effective date hereof.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
Section 1. That authority is hereby granted and approval hereby given for the exercise of the
first of two one-year options contained in a contract between the City of Omaha and Corvel
Enterprise Comp,Inc. for Workers' Compensation third party administrative services which three-
year contract was originally approved on June 18, 2013, by Ordinance No. 39703.
Section 2. That the Mayor is authorized to execute and City Clerk to attest on behalf of the
City of Omaha;any necessary paperwork to extend to exercise that option which provides Workers'
Compensation third party administrative services for an additional year at a cost not to exceed two
hundred eighty thousand dollars ($280,000)plus an increase in flat rate fees subject to a three and
one half percent per year dating from contract inception. Such funds are to be paid from Fund No.
11111, Organization 118017.
Section 3. As this Ordinance not being legislative in character shall be in full force and take
effective immediately after its passage.
