Transmitted herewith is a Resolution approving Change Order No. 2 for OPW 52928, being the
2016 Major/Residential Street Resurfacing — Various Locations project. The contract for this
project was awarded to Western Engineering Company, Incorporated by Council Resolution No.
702 on May 24, 2016. This change order increases the contract for the resurfacing of Road
Maintenance District 745-26, located on South 113th Street from Pacific Street north to its
terminus south of Leavenworth Street.
Western Engineering Company, Incorporated has agreed to perform this work for an additional
$92,692.60. The Change Order contains the details of the increase in the contract and all other
provisions of the original contract shall remain in full force and effect. The costs incurred will
be paid from the Capital Special Assessments Fund 13573, Special Assessments Organization
116168, year 2016 expenditure.
Western Engineering Company, Incorporated has filed the required Contract Compliance Report,
Form CC-1, in the Human Rights and Relations Department.
The Public Works Department requests your consideration and approval of the attached
Resolution and Change Order.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, Western Engineering Company, Incorporated was awarded a contract by
City Council Resolution No. 702 on May 24, 2016, to construct OPW 52928, being the 2016
Major/Residential Street Resurfacing-Various Locations project; and,
WHEREAS, it is necessary to increase the contract to incorporate the resurfacing of Road
Maintenance District 745-26 located on South 113th Street from Pacific Street north to its
terminus south of Leavenworth Street. The specific changes are listed in the attached Change
Order No. 2, which by this reference is made a part hereof; and,
WHEREAS, Western Engineering Company, Incorporated has agreed to perform this
resurfacing for a total additional cost of$92,692.60, which is to be paid from the Capital Special
Assessment Fund 13573, Special Assessments Organization 116168, year 2016 expenditure.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF OMAHA:
THAT, as recommended by the Mayor, Change Order No. 2 with Western Engineering
Company, Incorporated for the resurfacing of Road Maintenance District 745-26 on OPW
52928, being the 2016 Major/Residential Street Resurfacing - Various Locations project, is
hereby approved.
BE IT FURTHER RESOLVED:
THAT, the Finance Department is authorized to pay an additional sum of$92,692.60, for
this change order, from the Capital Special Assessment Fund 13573, Special Assessments
Organization 116168, year 2016 expenditure.
