Transmitted herewith is a Resolution approving Amendment No. 2 to a Professional Services
Agreement with CDM Smith, Inc. to perform professional engineering services for the Burt Izard
Lift Station Improvements, known as OPW 52472 (CSO) was approved by City Council
Resolution No. 994 on August 12, 2014.
The attached Amendment 2 to the Agreement authorizes CDM Smith, Inc. to conduct final
design and bidding assistance services. The exact scope of services is included in the attached
Amendment. CDM Smith, Inc. has agreed to perform the services detailed in Amendment 2 of
the attached agreement for a fee not to exceed $505,190.00 to be paid from Sewer Revenue
Improvements Fund 21124, CSO Control Implementation Organization 116918; Sewer Revenue
Bonds will be issued to finance this project.
CDM Smith, Inc. has filed the required Contract Compliance Report Form (CC-1), as determined
by the Human Rights and Relations Department. As is City policy, the Human Rights and
Relations Department will review the contractor to ensure compliance with the Contract
Compliance Ordinance.
The Public Works Department requests your consideration and approval of the attached
Resolution and Change Order.

TO THE AGREEMENT FOR ENGINEERING SERVICES
FOR OPW 52472—BURT IZARD LIFT STATION IMPROVEMENTS (CSO)
The Professional Services Agreement between the City of Omaha, Nebraska (OWNER) and
CDM Smith Inc. (ENGINEER) dated August 12, 2014 for professional services to provide
study and preliminary engineering phase services, is hereby amended as follows:
ENGINEER is obligated to comply with the Utilization of Small and Emerging Small
Businesses as provided in Exhibit A.
A detailed description of the scope of work and services to be performed is listed in Exhibit B.
In summary, this Amendment No. 2 only incorporates changes to the scope of work outlined
in Amendment No. 1 for OPW 52472 — Burt Izard Lift Station Improvements (CSO); all of
the original scope of work outlined in Amendment No. 1 remains in effect.
The estimated man-hours for this additional work are shown in Exhibit C. The maximum
amount for the services covered by this amendment shall not exceed five hundred five
thousand one hundred ninety dollars ($505,190) without prior written authorization by the
City of Omaha. This amendment does not change any of the provisions of the original
contract.

SCOPE OF SERVICES
AMENDMENT NO. 2
OPW 52472 (CSO)
This AMENDMENT provides for engineering services for final design and bidding services of
improvements to the Burt Izard Lift Station Improvements Project.The engineering Final
Design will expand upon the preliminary design completed in 2015. This Amendment No. 2
incorporates changes to the scope of work outlined in Amendment No. 1 for OPW 52472—
Burt Izard Lift Station Improvements (CSO) and all of the original scope of work outlined in
Amendment No. 1 remains in effect.The major improvements include:
• Upgrades to the electrical components in the Grit Basin Room including lighting,
receptacles, motors, and panels to meet Class 1 Division 1 per NFPA 820. Physical
separation of the Grit Basin Room and Storage Room to allow the Storage Room to be
meet Class 1 Division 2 per NFPA 820
• New hydraulic lids for Grit Basin No. 1.
• Replacement of the crane and clamshell in the Grit Building to meet Class 1 Division 1
and provide for improved operations.
• Addition of a second mechanically cleaned bar screen in the Bar Screen Room.
• Replacement of the screenings handling in the Bar Screen Room to include new
screening conveyance to drop screenings from two bar screens to one lugger.
• Replacement of the pumps, motors, drives, piping, and valves in the lift station.
Final design shall mean the development of the project for bidding including plans, specifications,
permits, contract documents and a Final Design Report.
Bid Phase Services shall mean assistance during the bidding phase including attendance at the pre-bid
meeting, plan and specification clarifications, preparation of addenda, and bid evaluation assistance.
ARTICLE 1.SCOPE OF SERVICES
This AGREEMENT provides for engineering services for final design and bidding services to update
the Burt Izard Lift Station including replacing equipment, updating the facility per current code
requirements, and providing improvements for grit removal.
The PROJECT work will be performed as part of the overall Omaha CSO/RNC Control Program.
ENGINEER will be required to comply with the written Plans, Protocols, Procedures, Standards, and
Guidance documents developed for the CSO Program.
This scope of services is organized in categories of project assumptions, project management, and
major project design tasks. Major elements of work will include:
• Task 1 -Project Management
• Task 2 -60% Design
• Task 3—90% Design
• Task 4—95% Design
• Task 5—Final (100%) Design

Notes:
1. Any documents listed above may be updated during the duration of the
project. In the event that a change is made to a document that affects the
scope of work included herein, a contract change may be required.
2. City of Omaha Standard Specifications and Plates shall be used, where
applicable, unless special conditions require modification. Modifications are
to be approved by the CITY.
TASK 1—PROJECT MANAGEMENT
Objective: Provide for the overall project management and administration, project
coordination,throughout the duration of the PROJECT.
Deliverables: Updated Quality Control Plan; Updated Project Health and Safety Plan; Updated
Project Execution Plan; monthly invoices; monthly progress reports; monthly
schedule updates; progress meeting and other meeting minutes.
Scope:
1.1. Project Management and Administration. ENGINEER to provide project management and
administrative activities in addition to the scope of work in Amendment No. 1 including:
1.1.1. Project Management and Administration.ENGINEER will provide project
management and administrative services for an additional 2 months.
1.1.2. Updated Quality Control Plan (QCP). No changes to scope from Amendment No. 1.
1.1.3. Updated Project HSP. No changes to scope from Amendment No. 1.
1.1.4. Updated Project Execution Plan (PEP). ENGINEER will update the PEP that was
created for Amendment No. 1 to include the additional scope.
1.1.5. Initial Project Kickoff Meeting. No changes to scope from Amendment No. 1.
1.1.6. Monthly PMT Meeting. ENGINEER will conduct two additional monthly progress
meetings with the PMT.
1.2. Agency and Project Coordination. ENGINEER will provide agency and project coordination
activities including:
1.2.1. Utility Coordination. No changes to scope from Amendment No. 1.
1.2.2. Agency Coordination and Permitting. No changes to scope from Amendment No. 1.
1.2.3. CITY Agency Coordination. ENGINEER will meet with CITY agencies to discuss code
requirements for the Grit Building.
TASK 2—60% Design
Objective: Develop 60%deliverables for review and utility coordination.The purpose of the
60%design submittal is to provide the CITY and PMT the opportunity to review the
plans, specifications, and appurtenant material so that the ENGINEER can proceed in
completing the plans, adding all necessary details, notes, etc.
Deliverables: 60% plans, 60%specifications, updated Basis of Design Report, Updated Cost
Estimate, Construction Schedule, Updated Environmental and Construction Permit
List and Draft Permit Applications, Updated Risk Register, and Updated Sustainability
Goals TM.
Scope:
2.1. Prepare 60% Plans. Expand on the decisions and concepts developed in the Preliminary
Design and finalize sizing calculations. 60% plan documents shall include major
modifications, equipment locations, and plans and sections to allow final detailing after the
60%submittal. Specific activities in addition to the activities listed in Amendment No. 1 are
shown below:
a) General and Civil Plans (no additional sheets, revise 3 sheets)
• Update the lift station process flow schematic to identify all improvements to be
included in this project (there will no longer be a Phase 1 and 2 implementation
strategy).
• Finalize pump hydraulic calculations.
b) Demolition Sheets (no additional sheets, revise 12 sheets)
• Modify the demolition sheets to include replacement of the Grit Basin No. 1
covers and grit room clamshell, modifications to allow for installation of a second
bar screen and screening handling in the Bar Screen Room, and replacement of
the pumps, motors, piping, valves, and existing bridge crane in the lift station.
c) Structural Plans (1 additional sheet, revise 8 sheets)
• Update the grit basin plan for new covers for Grit Basin No. 1 and separation of
the Grit Room and Storage Room for electrical classification.
• Update the bar screen room channel modifications plans to accommodate two
bar screens and new gates. Update the bar screen room for new screening
handling.
• Update plans for replacement of the existing bridge crane.
• Develop additional details.
d) Architectural Plans ( 2 additional sheets, revise 12 sheets)
• Add the Grit Building life safety plan.
• Develop details for replacement of the overhead rolling door in the Grit Building.
• Develop additional architectural details as needed for separation of the rooms in
the Grit Building.
e) Process Mechanical (3 additional sheets, revise 10 sheets)
• Add plan sheet for the grit building modifications.
• Update the Bar Screen room plans and sections for addition of a second screen,
new gates, and installation of new screenings handling.
• Modify the plans for the lift station to include replacement of the pumps, motors,
piping, valves, and existing bridge crane in the lift station.
• Develop additional process mechanical detail sheets.
• Finalize calculations for the pump hydraulics.
• Coordinate with pump manufacturers to develop bar screen specification.
• Finalize details for the pump cooling.
f) HVAC and Plumbing Plans (no additional sheets, revise 6 sheets)
• Add additional details and coordination of HVAC for the bar screen room.
g) Electrical Plans(6 additional sheets, revise 8 sheets)
• Update the Grit Building area classification for replacement of electrical
components to rate the Grit Room as Class 1/Division 1.
• Add plans and details for modifications in the Grit Building.
• Update the one-line diagram for the Grit Building, Screen Room, and Lift Station.
• Update the detailed plans for the Screen Room and Lift Station to reflect the
additional equipment included in the project.
• Develop detailed plans for the Grit Building including the layout, power plan and
lighting plans.
• Develop additional electrical details.
• Develop electrical specifications for the Grit Room modifications.
h) Instrumentation and Control Sheets (No additional sheets, revise 3 sheets)
• Update the P&ID's for the bar screen room and Lift Station to show all as new
(not future) work.
• Add instrumentation details for the screening handling and pumps
• Update controls of new pumps with new VFDs and PLC and level of control
desired with the WWTP. Update control system architecture and panel drawings.
2.2. Prepare 60%Specifications. ENGINEER will provide additional specifications to those
included in Amendment No. 1. The technical specifications for the new work to include the
following:
• Hydraulic Lids for Grit Basin No. 1
• New Grit Building Clamshell
• New Screenings Handling System
■ New Pumps and Motors
■ Piping
■ Valves
■ Electrical Component Replacement in the Grit Building
■ Overhead Rolling Door
■ Additional Bridge Crane
2.3. Lead-Based Paint Sampling. ENGINEER will add additional paint survey and asbestos
survey to the original sampling included in Amendment No. 1 to expand to additional areas
of demolition.
2.4. Design Workshops. ENGINEER will conduct two additional design workshops to those listed
in Amendment No. 1 to discuss the scope of replacement for electrical in the Grit Building
and the pump selection.
2.5. Construction Sequencing/Phasing. ENGINEER will consider construction sequencing and
phasing of the electrical work in the grit building, screening handling equipment
replacement, and installation of new pumps with the phasing already considered in
Amendment No. 1.
2.6. Constructability Review. ENGINEER will consider constructability of the electrical work in
the grit building, screening handling equipment replacement, and installation of new pumps
with the phasing already considered in Amendment No. 1.
2.7. Permits. No changes to scope from Amendment No. 1.
2.8. Risk Management. ENGINEER will update the mitigation strategy listed in Amendment No.
1 for risk factors to include the added work.
2.9. 60%Opinion of Probable Cost. ENGINEER will provide 60% opinion of cost based on the
60%deliverables document for the electrical work in the grit building, screening handling
equipment replacement, and installation of new pumps with the phasing already
considered in Amendment No. 1.
2.10. Prepare Addendum to the Basis of Design Report.An Addendum to the Basis of Design
Report will be prepared in accordance with the Deliverable Guidance Document as
described in Amendment No. 1 and will include the additional work included herein.
2.11. Prepare Preliminary Erosion Control Plan. No changes to scope from Amendment No. 1.
2.12. Submit 60% Deliverables for Review.The 60%deliverable described in Amendment No. 1
will include the 60%documents developed for the additional work described herein.
2.13. 60% Design Review Workshop.The electrical design lead will participate in the 60%design
workshop described in Amendment No.1.
2.14. Plan-in-Hand Review.The electrical design lead will participate in the 60% plan-in-hand
review described in Amendment No.1 to review the modifications to the Grit Building.
2.15. Address Plan and Specification Review Comments. CITY and PMT review comments will be
compiled in one Quality Review Form (ORE) document per deliverable and will be submitted to
the ENGINEER. ENGINEER will incorporate comments from the QRF into the 90%design including
the additional design described herein.
2.16. Small and Emerging Small Businesses Technical Memorandum. No changes to scope from
Amendment No. 1.
2.17. Contractor Outreach. ENGINEER will meet with up to four construction contractors to
discuss constructability of the project and any potential issues. Comments will be documented
and documents updated as needed.
TASK 3—90% Design
Objective: Develop 90%deliverables for review.This submittal is to provide the CITY and PMT
the opportunity to review the plans, specifications, and appurtenant material prior
to submitting for regulatory review and bidding the project for construction.
Deliverables: 90% Design Drawings, Specifications and Final Design Report; 90%deliverables
include: Draft Construction Permit Application, Final Design Report, SEB Inclusion
Report, Updated List of Permits, and updated OPC.
Scope:
3.1 Prepare 90% Design Drawings.The 90% Design Drawings are meant to be final, bid-ready
plans except for addressing comments from the CITY, PMT, and regulatory review and
including the additional scope items listed in this amendment.
3.2 Finalize Contract Documents.ENGINEER will finalize the additional technical
specifications for inclusion in the project contract documents.
3.3 Prepare Final Design Report(FDR). The FDR is a final compilation of the Basis of Design
Report(BODR),the addendum submitted at the 60 percent design stage, and any
additional modifications and/or calculations that support the final design of the project and
will include the additional scope items listed in this amendment.
3.4 Risk Management. No changes to scope from Amendment No. 1.
3.5 Prepare a Final Erosion Control Plan. No changes to scope from Amendment No. 1.
3.6 Small and Emerging Small Businesses Utilization Form. No changes to scope from
Amendment No. 1.
3.7 Prepare Final OPC. No changes to scope from Amendment No. 1.
3.8 Construction Schedule. No changes to scope from Amendment No. 1.
3.9 Permit Applications. No changes to scope from Amendment No. 1.
3.10 Submit 90% Documents for Review. No changes to scope from Amendment No. 1.

TASK 4—95% Regulatory Review Submittal
Objective: Prepare 95% Regulatory Review Submittal.The 95% regulatory review submittal is
meant to be a complete, bid-ready set of construction documents incorporating
CITY/PMT comments to be submitted to NDEQ for review and approval.This set will
be considered ready for construction and will not include "Not for Construction".
Deliverables: Updated Final Design Report; 95% Design Drawings and Specifications; Final
Construction Permit Application.
Scope:
4.1. Prepare 95%Submittal Package. ENGINEER will include drawings and specifications for the
additional scope items in the 95% submittal package.
TASK 5—Final (100%) Design
Objective: Prepare final design development documents including plans and specifications
suitable for bidding.
Deliverables: Updated Final Design Report, 100% Design Drawings and Specifications, Final Cost
Estimate, and permit applications with supplemental data as applicable.
Scope:
5.1 Prepare 100%Submittal Package. ENGINEER will include drawings and specifications for
the additional scope items in the 100% submittal package as described in Amendment No. 1.
5.2 Sealed Contract Documents.The additional plans and specifications included in this
amendment shall be signed and sealed by the project manager and the appropriate
disciplines and included with the deliverable described in Amendment No. 1.
TASK 6—BIDDING ASSISTANCE AND AWARD OF CONTRACT
Objective: Provide assistance to the CITY, on an as requested basis, for assistance during the
bidding process and selection of a CONTRACTOR for construction of the Project.
Deliverables: Pre-bid conference agenda and minutes, addenda and other items identified by the
CITY during the bidding phase, and letter on bid evaluation and recommendation.
Scope:
6.1 Transition Meeting No. 1: No changes to scope from Amendment No. 1.
6.2 Administration of Advertisement. No changes to scope from Amendment No. 1.
6.3 Pre-Bid Meeting: No changes to scope from Amendment No. 1.
6.4 Bid Period Questions. ENGINEER will answer questions pertaining to the additional scope
items and provide technical interpretation of the contract bid documents and will prepare
proposed responses for addenda.
6.5 Prepare Addenda: The ENGINEER will assist the CITY and PMT in preparing and issuing
addenda as required to address questions raised at the pre-bid meeting,written questions
from bidders, or other Contract Document plan and specification clarifications for the
additional scope items. ENGINEER will distribute the addenda to bidders as described in
Amendment No. 1.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, a professional services agreement with CDM Smith, Inc. to perform study
and preliminary engineering phase services for the Burt Izard Lift Station Improvemments Project,
known as OPW 52472 (CSO) was approved by City Council Resolution No. 99401 August 12,
2014; and, •
WHEREAS, additional final design and bidding assistance services not included in the
scope of the original Professional Services Agreement and are required to accomplish
construction goals of this project; and,
WHEREAS, CDM Smith, Inc. has agreed to perform the additional services needed as
detailed in the attached Amendment No. 2 to the Agreement, which by this reference is made a
part hereof, for a total additional cost of $505,190.00, which will be paid from the Sewer
Revenue Improvements Fund 21124, CSO Control Implementation Organization 116918. Sewer
Revenue Bonds will be issued to finance this project.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF OMAHA:
THAT, as recommended by the Mayor, Amendment No. 2 to the Professional Services
Agreement with CDM Smith, Inc. to provide additional final design and bidding assistance
services for OPW 52472 (CSO), being the Burt Izard Lift Station Improvements Project, is
hereby approved.
BE IT FURTHER RESOLVED:
THAT, the Finance Department is authorized to pay an additional $505,190.00 for these
items, to be paid from the Sewer Revenue Improvements Fund 21124, CSO Control
Implementation Organization 116918. Sewer Revenue Bonds will be issued to finance this
project.
