(402)444-5300 oR 4ry FAX(402)444-5314 apED FEER FAX(402)444-5317 City of Omaha Mikki Frost Jean Stothert,Mayor Director Honorable President and Members of the City Council, The City Council of the City of Omaha approved a three-year contract with Corvel Enterprise Comp, Inc. by Ordinance No. 39703 on June 18, 2013, which contract included two one-year options to be exercised at the City's sole discretion. Corvel Enterprise Comp, Inc. provides Workers' Compensation Administration services at an annual cost of no more than two hundred eighty thousand ($280,000) per year for the original term of the contract. The City exercised the first of the two one-year options last year by Ordinance No. 40720 on May 10, 2016. By exercising the second of the two one-year options, the option contemplates that all flat rate fees are subject to a three and one-half annual increase from contract inception. The Contractor has on file a current contract compliance report form CC1. Funds for this purchase will be from Fund No. 11111, Organization 118017. Approval of this Ordinance exercising the second of the two one-year options in the contract with Corvel Enterprise Comp., Inc. is respectfully requested by the City of Omaha Human Resources Department. BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF OMAHA: Section 1. That authority is hereby granted and approval hereby given for the exercise of the second of two one-year options contained in a contract between the City of Omaha and Corvel Enterprise Comp, Inc. for Workers' Compensation third party administrative services which three- year contract was originally approved on June 18, 2013, by Ordinance No. 39703 with the first of the on-year potions being approved on May 10, 2016 by Ordinance No. 40720. Section 2. That the Mayor is authorized to execute and City Clerk to attest on behalf of the City of Omaha; any necessary paperwork to extend to exercise that option which provides Workers' Compensation third party administrative services for an additional year at a cost not to exceed two hundred eighty thousand dollars($280,000) plus an increase in flat rate fees subject to a three and one half percent per year dating from contract inception. Such funds are to be paid from Fund No. 11111, Organization 118017. Section 3.
