The Omaha Fire Department respectfully requests that the Assistance to Firefighter Grant program consider the reinstatement of the initial funding requested to fully fund the equipment under the Wellness and Fitness portion of the 2016 AFG application FO-06996. We initially requested $5363/unit for 25 All in one functional trainers(body building/weight/lifting type equipment) and $5340/unit for 25 Gait simulators (cardio)to have one placed in each fire station. The funds were cut in half. That request was a well-developed assessment of models that would best assist the members of the Omaha Fire Department improve their physical fitness capabilities, help reduce injuries, and improve health and fitness post rehabilitation from injury. The products that we researched fell within the bottom of the price range but are still of good quality. The requested are not excessive. The estimates we received are listed. They are broken down so they could be compared similarly although each vendor bid them with freight, shipping and install slightly different. They are broken down so you could plainly see the costs per item as if they were installed in a station. There are 25 stations. Functional Trainer(weights)/Arc Trainer(cardio) $5820/$7220 $6485/$6041 $5843.04/$5490.88 $7156/$6365.92 We respectfully request the$127,575 be reinstated for a total of$267,575 under wellness and fitness equipment and $11876 in contractual remain so we can properly implement this program as intended. IX. Drug-Free Workplace Regulations All recipients must comply with the Drug-Free Workplace Act of 1988(41 U.S.C. 701 et seq.), which requires that all organizations receiving grants from any Federal agency agree to maintain a drug-free workplace. DNS has adopted the Act's implementing regulations at 2 C.F.R Part 3001. XII. Duplication of Benefits Any cost allocable to a particular Federal award provided for in 2 C.F.R. The term does not include your procurement of property and services needed to carry out the project or program (for further explanation, see Sec._.210 of the attachment to OMB Circular A-133, "Audits of States, Local Governments, and Non-Profit Organizations"). iii. A subaward may be provided through any legal agreement, including an agreement that you or a subrecipient considers a contract. 4. Subrecipient means an entity that: i. Receives a subaward from you (the recipient) under this award; and ii. Is accountable to you for the use of the Federal funds provided by the subaward. 5. Total compensation means the cash and noncash dollar value earned by the executive during the recipient's or subrecipient's preceding fiscal year and includes the following (for more information see 17 CFR 229.402(c)(2)): i. Salary and bonus. H. Awards of stock, stock options, and stock appreciation rights. Use the dollar amount recognized for financial statement reporting purposes with respect to the fiscal year in accordance with the Statement of Financial Accounting Standards No. 123 (Revised 2004) (FAS 123R), Shared Based Payments. iii. Earnings for services under non-equity incentive plans. Other compensation, if the aggregate value of all such other compensation (e.g. severance, termination payments, value of life insurance paid on behalf of the employee, perquisites or property) for the executive exceeds $10,000. XV. False Claims Act and Program Fraud Civil Remedies All recipients must comply with the requirements of 31 U.S.C. §3729 which set forth that no recipient of federal payments shall submit a false claim for payment. See also 38 U.S.C. § 3801-3812 which details the administrative remedies for false claims and statements made. • XVI. Federal Debt Status All recipients are required to be non-delinquent in their repayment of any Federal debt. Examples of relevant debt include delinquent payroll and other taxes, audit disallowances, and benefit overpayments. See OMB Circular A-129 and form SF-424B, item number 17 for additional information and guidance. XVII. Fly America Act of 1974 All recipients must comply with Preference for U.S. Flag Air Carriers: (air carriers holding certificates under 49 U.S.C. §41102) for international air transportation of people and property to the extent that such service is available, in accordance with the International Air Transportation Fair Competitive Practices Act of 1974 (49 U.S.C. 40118) and the interpretative guidelines issued by the Comptroller General of the United States in the March 31, 1981, amendment to Comptroller General Decision B-138942. Hotel and Motel Fire Safety Act of 1990 In accordance with Section 6 of the Hotel and Motel Fire Safety Act of 1990, 15 U.S.C. §2225a, all recipients must ensure that all conference, meeting, convention, or training space funded in whole or in part with Federal funds complies with the fire prevention and control guidelines of the Federal Fire Prevention and Control Act of 1974, as amended, 15 U.S.C. §2225. XIX. Limited English Proficiency (Civil Rights Act of 1964,Title VI) All recipients must comply with the Title VI of the Civil Rights Act of 1964 (Title VI) prohibition against discrimination on the basis of national origin, which requires that recipients of federal financial assistance take reasonable steps to provide meaningful access to persons with limited English proficiency (LEP) to their programs and services. DHS published the required recipient guidance in April 2011, DHS Guidance to Federal Financial Assistance Recipients Regarding Title VI Prohibition Against National Origin Discrimination Affecting Limited English Proficient Persons, 76 Fed. Reg. 21755-21768, (April 18, 2011). The Guidance provides helpful information such as how a recipient can determine the extent of its obligation to provide language services; selecting language services; and elements of an effective plan on language assistance for LEP persons. Lobbying Prohibitions All recipients must comply with 31 U.S.C. 1352, which provides that none of the funds provided under an award may be expended by the recipient to pay any person to influence, or attempt to influence an officer or employee of any agency, a Member of Congress, an officer or employee of Congress, or an employee of a Member of Congress in connection with any Federal action concerning the award or renewal. XXI. Non-supplanting Requirement All recipients who receive awards made under programs that prohibit supplanting by law must ensure that Federal funds do not replace (supplant) funds that have been budgeted for the same purpose through non-Federal sources. Where federal statues for a particular program prohibits supplanting, applicants or recipients may be required to demonstrate and document that a reduction in non-Federal resources occurred for reasons other than the receipt of expected receipt of Federal funds. XXII. Patents and Intellectual Property Rights Unless otherwise provided by law, recipients are subject to the Bavh-Dole Act, Pub. L. No. 96-517, as amended, and codified in 35 U.S.C. §200 et seq. All recipients are subject to the specific requirements governing the development, reporting, and disposition of rights to inventions and patents resulting from financial assistance awards are in 37 C.F.R. Part 401 and the standard patent rights clause in 37 C.F.R. § 401.14. XXIII. Procurement of Recovered Materials All recipients must comply with section 6002 of the Solid Waste Disposal Act, as amended by the Resource Conservation and Recovery Act. XXIV. Contract Provisions for Non-federal Entity Contracts under Federal Awards a.Contracts for more than the simplified acquisition threshold set at $150,000. All recipients who have contracts exceeding the acquisition threshold currently set at $150,000, which is the inflation adjusted amount determined by Civilian Agency Acquisition Council and the Defense Acquisition Regulation Council as authorized by 41 U.S.C. .$1908, must address administrative, contractual, or legal remedies in instance where contractors violate or breach contract terms and provide for such sanctions and penalties as appropriate. b.Contracts in excess of$10,000. SAFECOM All recipients who receive awards made under programs that provide emergency communication equipment and its related activities must comply with the SAFECOM Guidance for Emergency Communication Grants, including provisions on technical standards that ensure and enhance interoperable communications. Terrorist Financing E.O. 13224 All recipients must comply with U.S. Executive Order 13224 and U.S. law that prohibit transactions with, and the provisions of resources, and support to, individuals and organizations associated with terrorism. The Omaha Fire Department(OFD)is an urban career Fire Department with 641 sworn members and 5 civilian support staff.We provide fire service, paramedic ALS transport service, heavy rescue,search and rescue,confined space, high angle rope rescue,water and ice river rescue,hazardous material,industrial extrication,arson investigation,community risk reduction,code enforcement,fire prevention and training.We recognize that we cannot provide full service to our community without partnerships.We work and participate extensively with tri-county mutual aid fire group,the Omaha Metropolitan Medical Response System (OMMRS),Community Emergency Response Team(CERT)and Medical Reserve Corp(MRC), local and state health departments,the Emergency Management Agency,law enforcement,hospitals,and schools.We have partnerships with local business and are currently working to form Nebraska Preparedness Partnerships with business and government. Our community is bordered on the east by the Missouri River which presents the need for water rescue capabilities.We are transected by the Union Pacific Railroad,the Burlington Northern Railroad and Interstates 80 and 29, posing major transportation concerns and other risks.We have a large increase in ethanol production in the area that has put more transport vehicles on our roadways.We have two nuclear power facility to the north and south border and Offutt Air Force Base Strategic Command to the southern border.All of these assets increase the needs for our responders to have safe,reliable equipment, have proper training and be fit both physically and mentally to respond to a wide variety of hazards. Omaha has its social challenges.We have immigrant influx of about 60percent which is double the national average presenting competition of community resources from the schools and social based programs to meet the needs of incoming populations of largely people who do not speak English.Omaha also continues to see extremely high drug trafficking issues as it relates to Interstate 80.Omaha's homicide rate in 2015 was at a high of 50, largely due to gang activity.Swift response to correct the problem has resulted of a significant reduction in homicide currently which were cut nearly in half at 28.These are just a few examples however of community challenges that require allocation of local dollars to address.They draw away from resources to address fire challenges which do not go away on their own. OThe City of Omaha hosts the Tri-county Urban Area Security Initiative Working Group and although our UASI status has been removed we continue to plan as a cohesive team.The OFD strives to maintain our ability to serve the public to the highest possible standards to meet the demands put forth post 911 even as threats increase and funding decreases.We are very conscientious in our planning and training to address the core capabilities as a cohesive group ensuring OFD and the surrounding partners are meeting the National Preparedness Goals for our community.We work with our partners to strive towards resiliency. Omaha has experienced serious economic issues in the local budget over the last 10 years and has had its bond rating reduced in 2014.Omaha have a sewer project that is over 2 billion dollar to mitigate largely because it has continued to go unaddressed for so many years. Unfortunately for the fire service the rest of the bond rating blame has been placed on the unfunded liability of the police and fire pension system.The unfunded liability has a significant impact to the local economy, public perception of first responders and the fire budget.We have been working diligently to improve our image through community risk reduction efforts. Because the budget constraints are related to the fire pension, it is difficult to convince taxpayers to support additional revenues for necessary fire budget items. Omaha overall has tightened its belt by reduced staffing.City civilians have faced reduction in retirement from defined benefit to defined contribution and a decrease in insurance benefits with an increase in co pays.The Fire Union negotiated its own Health Care Trust in order to maintain its membersL benefits. The State of Nebraska taxes are flat so we expect no assistance from the State.Local property taxes for 2016 were decreased.We have lost Con Agra food headquarters which is not only iconic in the Omaha downtown but it has historically been a large contributor to beautification and revitalization.It is also a large contributor to the tax base.Strict spending limits have been put on all departments. OFD has reduced our force by 1 engine, 1 medic units and 1 heavy rescue truck even though we continue to grow in coverage area.OFD is down 35 members from 2009.We hired the first class this year in 4 years taking a back seat to 4 police recruit classes hired over that same time frame. Increased crime concerns continue to compete with our ability to fund projects. The cuts that occurred during the financial crisis were never replaced,therefore the necessary purchases needed to provide and maintain safe,effective,efficient and reliable equipment are always made when items are far beyond repair.We often find ourselves scrounging to to pull together the pieces and parts to make something work.We had to borrow a stretcher from another department in 2013 because ours was not in proper working order and we had a 69 percent reduction in equipment purchases In 2014 to give you an idea of where we have been. It will be used to replace cameras older than 11 years,failing or failed cameras,that are non NFPA compliant and inconsistent across our department.Funding from this grant will take us to 100%of where we believe we want to be to be reasonably covered by thermal cameras across the department though it will not place one on every apparatus.They will be placed on 6 of the 9 trucks because we currently have 3 newer models.They will also be placed on both the frontline and backup hazardous material vehicle(2)because they both are in service frequently and on 8 engines in areas that are more geographically isolated to assist with RIT per SOP.Many of these engines do not currently have TIC4s because as the number of functioning cameras has decreased.As the cameras go bad and beyond repair they have been shunted to mainly truck companies and OFD has been unable to replace them on the engines as originally intended. The total cost of each thermal imaging cameras is roughly 13,500 X 16 cameras for a total of 216,000.The breakdown of the purchase is: $12,000 1801 NFPA compliant Thermal Imaging Camera which includes options such as compass,laser pointer range finder,picture/video capture with transmit options $750 Vehicle kit including mounting-for regular daily use,vehicle mounted charger 2 rechargeable batteries $750 Station charger-for use when regular apparatus is out of service for repair or maintenance-includes carrying case, 2 rechargeable batteries,multiuse charger,retractable lanyard to allow for seamless maintenance at all hours with minimum out of service times We will use our regional training center to provide training for the thermal imaging cameras to our suppression crews. Thermal imaging camera per unit-13500 x 16 units including installation of chargers and training for training staff for a total of 216,000.The camera is roughly 12,000 and the different charging systems are roughly 750 for an in station and 750 for a vehicle mounted based on our assessment and several estimates. Having both chargers is necessary for Omaha Fire Department so the TIC can remain in service as we have regular maintenance and repair work on apparatus round the clock.We do not want firefighters without critical equipment. First and foremost the benefit is improving safety for our firefighters and our citizens.We average well over 700 structure fires per year and likely every one of those fire we use or would use a thermal imaging camera in some capacity.They would be used to check safe distances to a building in some cases,during extinguishment and overhaul in nearly all cases and less common but most critical in life safety instances for increased chance of more rapid victim rescue and recovery. In 2013 and 2014 we experienced several fires in apartment complexes that would not meet fire code had they been built today.The voids and spaces created for mechanical and electrical that allow fire to travel and grow undetected caused injury to several firefighters and ended the career of a firefighter with young children. It also changed his life forever.This grant will allow OFD to reduce the chance of that reoccurring.It fills a void in a problem that we have already attempted to remediate through training that occurred immediately following these incidents and by code enforcement through the updating of our fire codes to the 2012 International Fire and Life Safety Code in 2014.We are seeking to reduce the likelihood of firefighter injury at fire scenes.These cameras will complete our assessment of what needs to be done.We will however continue to evaluate to ensure we have properly addressed the problem and adjust strategies to improve accordingly. The cost of this project is 216,000.Over ten years we would use these cameras a conservative minimum of 7600 times, making their use about 28 dollars per fire. This seems a very reasonable expense to increase the likelihood of finding the fire,victims or downed firefighters and minimize property damage for the owners. For each of our citizens this is less than.43 cents in one year.04/year spread over 10 years-which is a reasonable life for a camera under high use.The cost is very reasonable.The cost of the medical issues for one injured fire fighter with time off work and in hospital medical bills alone exceed the amount of this grant request in a matter of a week.This would also be similar for civilian injury and children lost in inclement weather(as we experience here nearly every year).Their suffering may be prevented or their exposure minimized and life improved with the use of a TIC. *Section#3 Statement of Effect: How would this award impact the daily operations of your department?How would this award impact your department's ability to protect lives and property in your community?*4000 characters The benefit for the Omaha Fire Department will be tremendous. Having consistent reliable equipment that is in good condition will improve our ability to serve the community and reduce property damage,save lives and rescue our own.We had 760 structure fire last year that used the TIC.That number will continue to grow as Omaha expands its fire district.Most of our TICs in service are older than 11 years, unable to be repaired after 2018,not NFPA compliant and are of various models and brands. Being able to pick up a camera at one station and having the same camera at the next station will bring a higher level of safety,efficiency, and confidence that we do not currently enjoy.Our firefighters move stations frequently and being able to have consistent equipment will drastically improve not only safety and efficiency but job pride. It will improve OFD's ability to train our firefighters more efficiently. It will allow our training staff to concentrate on the evolutions at hand rather than worrying about ensuring that the camera stays working or being familiar with the nuances of the different varieties of cameras. It will help us streamline our SOP's so they look more like an operating procedure and not a large collection of old manuals. New cameras over the last 5 years cost about 30,000 to repair. Many did not get repaired and have remained out of service.These cameras will no longer be able to be repaired as they are at a manufacturer end of life in 2018.This is pertinent information to understand but not a cost savings as we do not have the funds nor could we fix our current cameras if we wanted to do so. Without this grant we will continue to serve the citizens of our community to the best of our ability.We cannot say that someone will surely die or destruction is definitely eminent.We can say that we are running with failing equipment and that much of it will not make it another year. If and when it fails, it is not repairable. As we sit now funds are not available in next years budget to replace the cameras that have been taken out of service currently nor do we have funding to address any future failures so your assistance is critical. Further, please describe what you are requesting funding for,including budget descriptions of the major budget items, i.e., personnel,equipment,contracts,etc. *4000 characters After an extensive needs assessment the Omaha Fire Department has determined that it desperately needs to replace its physical fitness equipment for its members.The current equipment is a mix of city purchased equipment from 2002(making it 15 years old),personal equipment brought from home and extra equipment purchased from second hand sales. It is old,dilapidated and a hazard at best.Treadmills are broken down in the center,do not work properly and are at risk for causing more hams than good.There is no all in one functional trainer and stations have various weights that put ff at risk for injury.For that reason we are respectfully requesting your assistance to purchase equipment that is effective for both strengthening cardio and core that keeps the body properly in line We are looking to purchase 2 pieces of equipment for each station that helps reduce chance of injury to our firefighters and improves balance, muscle and strength-one piece is a functional trainer,one piece is an arc style gait simulator trainer.The functional trainer will help simulate advancing a charged hose line, breaking through a roof,carrying equipment,carry high-rise packs,carrying and climbing a ladder,direct a charged hose line,hoisting equipment,using forcible entry tools and moving victims. Equipment that will simulate these also keeps the body inline training and strengthening the muscles for optimum use.The arc trainers or similar device provide increased cardio as well as activating the muscle groups and do so with no impact to joints reducing injury.There are virtually no breakable parts minimizing maintenance on these devices giving them long life in a high use environment. Seventy eight percent of our injuries are related to sprains and strains.That accounted for 78 injuries last year. Both of these pieces of equipment give our firefighters the opportunity to reduce these injuries by increasing their stamina and strengthening their overall body including a piece of equipment that provides 184%higher utilization of their core while keeping them in perfect alignment.We know they use the equipment.Firefighters are going to local schools to use the track and most do workout outside of the department setting and are purchasing things to bring in. It is not that we have a job that is not interested in equipment that we must force to be fit. Our department has had such high needs that the station equipment has not been a priority and in previous years we have not met the Priority 1 activities in order to apply in Priority 2 activities.We now meet that expectation in immunization,behavioral health,initial and periodic physicals so it is important to get our firefighters the equipment they need to maintain their physical health. Provide justification for the budget items relating to the cost of the requested items.*4000 characters The benefit to the Omaha Fire Department is safety.The safety goes directly to its members by improving their overall health and fitness.Health and fitness of firefighters communicates to better response to the citizens improving response times,minimizing injuries during high stress work and reducing liability of the taxpayers.Firefighters in good physical shape can go on to retire and suffer less debilitating injuries and live longer healthy lives. The cost of the project is approximately 290,000 to outfit all 24 emergency stations and the 1 community risk reduction station housing public education and code prevention sworn staff.That is about.57 cents per citizen in Omaha in one year or less than.06 cents per year over 10 years per citizen in Omaha.This is a very reasonable expense. One back related injury in Omaha averages over this amount when you combine time lost at work,call back,and medical expense.We are losing an average of over one fire fighter per year over the last 5 years to back related injuries and muscle strains that may have been prevented by core strengthening fitness.The pay out on the fire department pension for one or these injuries exceeds the amount of this purchase in two years for one of these injuries alone.Strains accounted for an additional 67 injuries in 2014 and 2015 and over 423,393 dollars in loss to the fire department.The benefit of having equipment to help reduce this number would be incredible.We have been working diligently in conjunction with the police department to ensure the priority one items in place,assistance with the equipment would complete the needs we need to fill for our hardworking and dedicated firefighters. There would be a great benefit related to repair costs as well.This is difficult to determine because there are many items that are not able to be repaired.In addition some items that FF are using their own equipment. It is not quality equipment and is probably a contributor to injuries.There will be a benefit realized from removing those machines and replacing them with reliable equipment though the numbers are not readily available. • *Section#3 Statement of Effect: How would this award impact the daily operations of your department?How would this award impact your departments ability to protect lives and property in your community?*4000 characters The affect of this award would be incredible for the Omaha Fire Department. Better fitness and a more stable core will yield improvements in strength,endurance and overall ability to perform better at the fire scene for the individual firefighters.This has a direct benefit to firefighters as well as their families by improving quality of life and long term health and fitness.AFG assistance with Priority two items(a high priority request)would help our department improve emergency response and better serve the community. It will also increase morale for firefighter across the department.When firefighters have more of the equipment that they need they feel more valued as an employee. It is not at all that they have not been valued to this point;it has been so many other items of basic firefighter needs needed to be met that if it was not truly broke we were not able to fix it.We are at that point with health and safety exercise equipment that is 15 years old and we are desperate to address it with this grant.We have worked very hard to improve all our items in Priority 1 fitness to be eligible to apply for this grant. It is the second time we have applied.Many staff members,peer fitness trainers,health and fitness committee members have worked to get us new equipment and have been unsuccessful.This has been cut from budget and unfunded by grants in the past so the realization of hard work coming to fruition will be very positive and has a positive impact job wide. This award will also reduce sick and injury leave.The ability to keep apparatus running at optimal staffing on a daily basis is huge and it will be possible when our firefighters are able to workout at work with proper and safe equipment much like they do on their day off.This will decrease injury rates and OFD will be better prepared to provide service to the community.The 67 injuries over 104 weeks accounts for someone off for a strain across the entire 2 year time frame because most of the injuries exceed one set of days.To be able to reduce that number by even half would be significant. Injury of firefighters on the fire scene detracts from the original mission and decreases our ability to help the community. Being able to reduce the number of injuries will improve our efficiency on the fire ground-decreases strain on other firefighters that have to carry additional burden when other firefighters are not fit for duty from either less than optimal physical fitness to those injured.This grant helps the job as a whole. This award would eventually benefit the community by reducing the burden on taxpayers.It does so by reducing the number of debilitating injuries that plague our pension system and further drive a wedge between the fire service and the community in the city financial picture.
