Transmitted herewith is a Resolution approving Change. Order No. 1 for QPW 52893, being the
168th Street - West Dodge Road to West Maple Road Project. The contract for this project was
awarded to Hawkins Construction Company by Council Resolution No. 1323 on December 05,
2017. This Change Order increases the contract to provide for the inclusion of construction costs
related to a phasing change allowing Blondo Street to be opened early.
Hawkins Construction Company has agreed to perform this work for an additional $185,000.00.
Early completion incentive provision of $2500.00 per day is also applicable, not to exceed
$75,000.00. The Change Order contains the details of the increase in the contract and all other
provisions of the original contract shall remain in full force and effect. The costs incurred will be
paid from the 2014 Transportation Bond Fund 13184, Transportation Bonds Organization 117117,
year 2018 expenditure.
Hawking Construction Company has filed the required Contract Compliance Report; Form CC-1,
in the Human Rights and Relations Department.
The Public Works Department requests your consideration and approval of the attached
Resolution and Change Order.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, Hawkins Construction Company was awarded a contract by City Council
Resolution No. 1323 on December 05, 2017, to construct OPW 52893, being the 168th Street -
West Dodge Road to West Maple Road Project; and,
WHEREAS, it is now necessary to include construction costs related to a phasing change
allowing Blondo Street to be opened early. The specific changes are listed in the attached Change
Order No. 1, which by this reference is made a part hereof; and,
WHEREAS, Hawkins Construction Company has agreed to perform this work for a total
additional cost of $260,000.00, which is to be paid from the 2014 Transportation Bond Fund
13184, Transportation Bonds Organization 117117, year 2018 expenditure.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
OMAHA:
THAT, as recommended by the Mayor, Change Order No. 1 with Hawkins Construction
Company for additional work on OPW 52893, being the 168th Street-West Dodge Road to West
Maple Road Project, is hereby approved.

BE IT FURTHER RESOLVED:
• THAT, the Finance Department is authorized to pay an additional sum of $260,000.00 for
this Change Order, from the 2014 Transportation Bond Fund 13184, Transportation Bonds
Organization 117117, year 2018 expenditure.

1. Hawkins will commit to having Blondo St. back open to east-west
traffic in 120 days with the following conditions:
www.hawkinsl.com a. Hawkins will need an additional $150,000 to cover the cost for lost
productivity. The lost productivity comes from Blondo St. being
open which will delay construction traffic at the intersection.
Hawkins will also need to commit an additional superintendent to
the project that was not included in the original bid. The additional
dollars will cover the extra overhead costs.
b. There will also need to be a temporary signal installed at the
intersection of 168th and Blondo St., after Blondo St. is re-opened
.to traffic. This signal will be equipped with an active control used
to stop east-west traffic on Blondo St. and allow construction
traffic to go north-south on 168th St. The approximate cost of this
temporary signal is $35,000.
c. The 120-day count will not start till MUD has completed their gas
main work at the intersection of 168th St. and Blondo.
2. Hawkins will receive an incentive of$2500 per day for every calendar
prior to the 120 calendar days that Blondo St. is open to east-west
traffic. The maximum incentive amount that can be earned is$75,000
(30 days).
3. All other terms and conditions of the original contract remain
unchanged.
