It is necessary to extend the contract for approximately three (3) months in order for the Douglas County Community Mental Health Center to work with the State of Nebraska on certification and licensing issues. Douglas County Community Mental Health Center has agreed to perform the additional work for the same amount of $7,500.00 per month. All other provisions of the original contract shall remain in full force and effect. Finance Department is authorized to pay the Funds in the monthly amount of $7,500.00, with the total amount not to exceed $22,500.00, to Douglas County Community Mental Health Center. Funds will be allocated in the amount of $16,250.00 from 2018 Keno Fund 12118, Org 126017 and $6,250.00 from 2018 General Fund 11111, Org 111111, Chief Administrative Functions, Account 42239, Professional Fees—Other. By Coun nember Adopted APR 2 4 20181~- City Clerk Approved 4.-th Mayor Second Addendum to Agreement between Douglas County, Nebraska and City of Omaha This addendum hereby revokes and replaces Sections 7 of the contract executed on or about January 31, 2017, and amended on or about January 1, 2018, by and between Douglas County, Nebraska, on behalf of the Douglas County Community Mental Health Center and the City of Omaha for Civil Protective Custody Services as follows: 7. TERM and TERMINATION This Contract shall be in full force and effect from the 1st day of February 2017, • through and including the 31st day of December 2017, unless terminated by a Party as provided herein. This Agreement may be extended for up to six (6) periods of one (1) month each upon written agreement of the Parties. Either Party may terminate this Agreement at any time for any reason upon thirty (30) days written notice to the other Party of its intention to terminate this Agreement. This addendum only affects the provisions specifically stated and any and all other contractual provisions found in the Agreement executed on or about January 31, 2017, and amended on or about January 1, 2018, shall remain in full force and effect.
