The attached Resolution approves a Contract Change Order with 14C, LLC dba 4C
Construction increasing the contract amount for the construction of one (1) additional house in
the Locust Glen Neighborhood, 19th Avenue and Locust Streets.
On September 26, 2017, Res. No. 1115, the City Council approved a contract with 14C, LLC
dba 4C Construction in the amount of $375,000.00 for the construction of two (2) houses. A
subsequent change order increased that amount to $391,020.00. The attached Change Order
in the amount of $187,500.00 provides for the construction of one (1) additional house in the
Locust'Glen Neighborhood, 19th Avenue and Locust Streets. The contract amount with change
orders is now $578,520.00.
The Contractor has on file a current Annual Contract Compliance Report Form (CC-1). As is
City policy, the Human Relations Director will continue to review the Contractor to ensure
compliance with the Contract Compliance Ordinance.
The Finance Department is authorized to pay the cost of Locust Glen Phase II. project in the
amount of $578,520.00. Funds shall be payable from FY 2016 NAHTF No. 12141,
Organization No. 128101, FY 2016 HOME Investment Partnership Program, Fund No. 12179,
Organization No. 128071 for the construction of two (2) houses in the amount of $391,020.00
and FY 2017 HOME Single-Family In-fill Housing Program, Fund No. 12179, Organization No.
128071 for the construction of one (1) additional house in the amount of$187,500.00.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, bids were received on August 9, 2017 for the Locust Glen — Phase II Project for
the construction of two (2) new houses in the Locust Glen Neighborhood, 19th Avenue and Locust Streets;
and,
WHEREAS, a contract with 14C, LLC dba 4C Construction in the amount of $375,000.00 for
Locust Glen — Phase II Project was approved by City Council of the City of Omaha on September 26, 2017,
Res. No. 1115; and,
WHEREAS, a subsequent change order increased the contract to $391,020; and,
WHEREAS, it was necessary to increase the contract by $187,500.00 to provide one (1)
additional house to the Locust Glen — Phase II Project thus increasing the contract amount to $578,520.00;
and,
WHEREAS, 14C, LLC dba 4C Construction has complied with the City's Contract Compliance
Ordinance No. 28885 in relation to Civil Rights - Human Relations.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
THAT, the attached contract change order from 14C, LLC dba 4C Construction for the Locust
Glen — Phase II Project increases the contract amount to $578,520.00 is hereby approved and the Finance
Department is authorized to pay the cost of construction of Locust Glen — Phase II for the construction of two
(2) houses from the FY 2016 NAHTF No. 12141, Organization No. 128101, FY 2016 HOME Investment
Partnership Program, Fund No. 12179, Organization No. 128071 and for the construction of one (1)

The foregoing changes to the above mentioned project shall have the same force and effect as if such change or changes had been
incorporated in the original Work Write-Up and this Change Order Number( 4 )to the Contract Documents will be incorporated in
and become a part of the Contract Documents of the Work.
This Change Order Number( 4 )shall in no way be construed as altering the aforementioned Work Write-Up;meaning and intent,
except as expressly set forth in the above mentioned items, and all other provisions of the Work Write-Up shall remained unchanged
in full force and effect.
Present status of the account for the above contract is as follows:
Original Contract Sum: $ 375,000.00
Net change by previously authorized Change Orders: $ 16,020.00
The Contract Sum prior to this Change Order: $ 391,020.00
