Omaha/Douglas Civic Center 1819 Farnam Street,Suite 601 Omaha,Nebraska 68183-0601 The City of Omaha has already entered into a contract with Deffenbaugh (402)444-5220 Industries to provide solid waste and recyclable material collection service which Telefax(402)444-5248 will begin on January 1, 1996. The Contract provided for an early startup option. Michael J.DeSetm,P.E. The current contract with Watts Trucking extends until December 31, 1995. The ActingDirector Public Works Department desires to exercise the early startup provisions with Deffenbaugh Industries. This Resolution adjusts the provisions of the contracts with both Deffenbaugh Industries and Watts Trucking. Exhibit A addresses the contract with Watts Trucking by deleting the collection of solid waste after September 17, 1995, reimbursing certain expenses totalling $92,055.00, and requiring the fulfillment of a transition plan. Exhibits B and C detail the changes to the Deffenbaugh contract in order to achieve the early startup. These exhibits adjust both the number and size of vehicles which Deffenbaugh will employ. The Resolution also authorizes payment to Watts Trucking of$92,055.00 from the General Fund. The Public Works Department requests your consideration and approval of the , attached Resolution. Date August 9. 1995 Item CONTRACT PRICE No. ITEM AND DESCRIPTION OF CHANGES DECREASE INCREASE This Change Order No. 1 initiates the early startup option at a reduced rate on September 18, 1995,and deletes the operation plan submitted with bid proposal and 1 replaces it with the operation plan submitted July 30, 1995. In the event that some existing provisions are in conflict with these amendments,these provisions shall prevail in such conflicts. The major changes are detailed below: 1. Initiate early startup of solid waste collection as provided on Page SWC19a for 1995 $2,042.33 annexations on September 1, 1995,(1,305 stops)at one-half the$3.13 per stop per month rate. 2. I Initiate City-wide solid waste collection on September 18, 1995. Total number of stops $1,164,571.28 is determined to be 106,305 per week which includes annexations in 1 above. 3. Bid price of$3.13 is reduced to$2.81 per household per month for City-wide service $119,061.60 (106,305 stops)on September 18, 1995. September City-wide service partial month will be paid at 50%of full monthly rate. 4. Increase the capacity of 20 solid waste collection vehicles from 27 to 31 cy each. N/C N/C 5. Increase the capacity of 2 solid waste collection vehicles from 16 to 20 cy each. N/C N/C 6. Delete two(2) 16 cy solid waste trucks;replace with five(5)3 cy solid waste trucks. N/C N/C 7. Delete four(4)27 cy and two(2) 16 cy new backup solid waste trucks;replace with N/C N/C fourteen(14)27 to 31 cy used backup trucks for solid waste. Allow collection of recyclable materials in select alley locations,approved by the N/C N/C director,on separate compartments on solid waste collection trucks. 9. A. Each vehicle shall be inspected annually during the month of January. Each vehicle shall also be inspected before being added to the "Authorized Collection Vehicle Log" (ACVL). If a vehicle is removed from the ACVL for any reason it must be inspected before being added to the ACVL again. Vehicles not passing inspection will not be added to nor remain on the ACVL. B. All vehicles must be equipped with a reverse alarm with an automatic braking system and meet all prevailing government regulations relating to air and noise pollution as well as OSHA requirements • C. • The vehicle inspection shall be performed by an independent third party. This party shall be selected-by the Contractor and approved of by the City of Omaha. Vehicles inspected by personnel-not approved of by the City of Omaha will be considered not inspected for the purposes of this Section. The Contractor is responsible for all costs associated with the inspections and for making any repairs required to pass inspection. D. The vehicles shall have the following items checked as a minimum during the required inspections. A form will be provided. 1. Lights (head, tail, park, turn indicator, back-up, and clearance) 2. Brakes (front, rear, emergency and parking) 3. Reverse alarm with automatic braking system 4. Steering and Suspension 5. Horn 6. Windows and windshield for cracks in the viewing area 7. Windshield wipers 8. Tires 9. Bodies shall be inspected for water tightness and ability to keep the load from being blown out by wind. 10. All other mechanical and electrical systems shall be inspected to determine their ability to perform the job for which intended. An inspection report, signed by the inspection agent, shall he sent to the Solid Waste Management Office for determination of suitability of the vehicle to remain on or be added to the ACVL.
