The contractor has on file a current Annual Contract Compliance Report Form
(CC-1). As is City policy, the Human Relations Director will review the
contractor to ensure compliance with the Contract Compliance Ordinance.
The Finance Department is authorized to pay the cost of Dam Site 18 wastewater
operation, inspection,maintenance and repair from the Flood Control and Water
Quality Organization 1451, Fund 001.
The Public Works Department recommends acceptance of the bid from
Commonwealth Electric Company, in the amount of $23,773.00, being the
lowest and best bid received within the bid specifications, and requests your
consideration and approval of this Resolution.

ASSOCIATED ELECTRICAL SERVICES, STANDBY
GENERATORS. ULTRAVIOLET DISINFECTING
FACILITIES A 'D STORAGE BASINS
L. SCOPE
The contractor shall provide operation and maintenance services. The operational services shall
include sampling, testing and perform inspection, The maintenance services shall include.
inspection, and repair on all the electrical and mechanical equipment for the City of Omaha. The
location of the installation is 216"' Street and West Center Road, and shall be maintained and
operated in accordance with operation and maintenance manual and plans that are a part of this
contract. The facilities of this contract are.
1. Combined lift station and forcemain for flows from S &ID's 303 and 57 plants. •
•
2. Stand-By generation facilities for S &ID 303 and lift station. .
- 3. Storage basin and Aerators for S &ID's 57 and 303 flows.
•
4. Storage basin pump station and forcemain to the Elkhorn River.
5. Ultraviolet disinfecting facilities.
It shall be the responsibility of the prospective contractor to familiarize himself with the
equipment, services to be performed, 0 &M Manual and the environment prior submitting. a bid.
Lack of knowledge concerning any or all parts of the work shall not excuse the prospective bidder
after the bid opening or contract perfbrrnarnce after the bid opening or contract performance after
award. An 0 &M Manual of the facility will be available for inspection at 5600 So. 10th St.,
Omaha, Nebr., Attn: Mike Merriex, 444-3903.

II. SERVICES TO BE PERFORMED

B. OPERATIONS PROCEDURES OF DAM SITE 18 - PERFORMED WEEKLY . •
Pump Station No. 1 •
The maintenance of pump station No 1 shall conform to all instructions set forth
in the 0 & M manual. Any irregularities that are noted during a maintenance visit
shall be brought to the attention of the City of Omaha personnel. Log in run times
of pumps. At the end of month forward a copy of this information to the City of
Omaha.
Back-up Generator and Electrical Equipment
The maintenance of the back up generator and electrical equipment shall conform
to all instructions set forth in the 0 & M manual. In addition, at the generator, the
level of the diesel fuel shall be checked. If the level is 1/4 of a tank or less refill
tank. The electrical cabinets shall be inspected and opened bi-weekly to ensure
the integrity of the cabinets and the equipment within.
Road up to the Holding Basin
Any hazardous conditions along the road to the holding basin shall be brought to
the attention of the City of Omaha as soon as possible.

Ultraviolet Disinfecting System
The maintenance of the UV disaffection system shall conform to all
instructions set forth in the 0 &M manual and the VHS video tape
supplied by the City of Omaha. During weekly maintenance visits the
inside of the UV system shall be opened and inspected. Any deficiencies
shall be noted and corrected. Any corrective maintenance of the system
during April 1 to September 30 shall be completed within the same day. A
form shall be available for the operation/maintenancepersonnel to fill in
meter run time for the system and the date checked. Log in run time of
UV. At the end of month forward copy to the City of Omaha. •
If during the course of the inspection it is required to energize the
Ultraviolet lamps the operations/maintenance person shall, repeat shall,
have on proper UV eye protection. Anyone not wearing the proper eye
protection shall not work on or near the Ultraviolet system while the
covers are off and the lamps are energized.
Miltronics Multiranger
Inspection of the multiranger shall also occur weekly. If a LOE signal is
flashing on the screen of the multiranger, the operation/maintenance
person shall follow the procedures set forth in the 0 & M manual to
attempt to reinitialize the multiranger. If the system fails to respond notify
the City of Omaha as soon as possible.
Pump Station No. 2
The maintenance of Pump Station No. 2 shall conform to instructions set
forth in the 0 & M manual. Any irregularities that are noted during a
maintenance visit shall be brought to the attention of the City of Omaha.
Also, at this station the pumps shall be turned on td ensure proper
function, and no unusual sounds are emitting from the pumps during their
operation, A form shall be available for the operation/maintenance person
to fill in the date inspected, pump run time from meter, and length of time
pumps are run for test. One (1.0) minute minimum run time per pump.
From April 1 to September 30 the UV system must be turned on prior to
• pumps being tested. At end of month forward copy of log to City of
Omaha.

Aerators
The maintenance of the aerators shall confirm to all instructions set forth in the 0
& M manual. In addition, a visual inspection of the equipment in operation shall
be performed during the weekly maintenance inspection. If during operation the
aerators are making loud enough noises to be heard on the bank of the holding
•
basin, the aerator shall be turned off at the breaker, then removed for an in - shop
inspection as soon as possible. City of Omaha shall be notified of the aerators
conditions as soon as possible.
The Holding Basin
The Holding Basin shall be inspected weekly during the maintenance visit
of the facility. This inspection shall be visual in nature, with emphasis on
looking for sink holes, degradation of the holding pond walls, inner and
outer, road surface and surrounding fenced in area. If there are any of the
aforementioned problems, or other conditions that may be of an injurious
nature to the facility, bring it to the attention of the City of Omaha
immediately.
Operational Maintenance Service
Operation maintenance shall conform to prescribed procedures as stated in
the O&M Manual, and any additional instructions in this specification.
The contractor shall perform operational maintenance of the
aforementioned facilities three (3) times per week.
Operational maintenance service shall be bid on a price per week for fifty-
two (52) weeks.
Procedures for the Testing of Effluent
The following procedures are guidelines of how and when the City of
Omaha requires the testing of the Holding Basin's water to be executed.
As per our permit we have three locations that are required to be tested
monthly. Location A, is the holding basin itself, Location B, is the outlet
of the Ultraviolet Disinfecting System, Location C, is the outlet pipe to
Skyline Golf Course. These locations should be tested early each month,
preferably within the first two weeks, to ensure the receiving of test results
in a timely manner for quarterly permit DMR's to be submitted to the
NDEQ. The City of Omaha is required to disinfect all the water that is
pumped from the Holding Basin from April 1 to September 30. During
these months if flow is required for the grab sample at the outlet of the UV
System and the Elkhorn River the contractor gathering the samples shall
ensure that the UV system is operational prior to sampling and turning on
the pumps.

If a situation arises where the UV system is out of service, the contractor
responsible for sampling will be notified by the primary contractor prior to the
date of sampling and alternate sampling times will be established.
Emergency Procedures for Dam Site 18
Currently at Pump Station No. 1 we have security measures that will
inform the proper personnel in the event of a power loss, and a back up
power generation system to keep the facility in operation until corrective
measures are taken.
At Pump Station No. 1 there is the Chatter Box Automatic Dialer and
Voice synthesizer. Should a high water, or power failure alarm be
registered, the Chatter Box will call in order:
•
1. Maintenance Contractors Responsible Individual
2. Maintenance Contractors Home Office
3. Missouri River Wastewater Treatment Plant
4. Manager of Dam Site 18 Facility.
The Chatter box will call each number in order until it receives an
acknowledge response from the number it calls. To acknowledge an alarm,
first a touch tone phone must be used. The number 9 pushed, the chatter •
box will then announce that the alarm has been acknowledged.
•
The following personnel must be aware of what must done in the event a
call comes into their location.
1. The maintenance contractors responsible individual, is the person who
is performing the ongoing maintenance inspections and the work that is
required at facilities. The number logged into the Chatter Box will be this
individuals home phone number, in the event that an alarm is issued at
night. (If the number is not to the individuals' home, then the maintenance
contractor shall be required to show the City of Omaha the method by
which his maintenance person will be notified by the Chatter Box in the
event of an alarm). If during the day an alarm is issued, and the
responsible individual is not at home to acknowledge the alarm, it will dial
the next number in the stack, the maintenance contractors main office.
2. If the maintenance contractors main office is called by the Chatter
Box, whomever is receiving the calls for the office must be made aware
that this event can occur, and all that is required of them is to acknowledge
the alarm and inform the responsible individual of the alarm and the time
it was received.
3. The Missouri River Wastewater Treatment Plant. If the Chatter Box calls
this plant, the contractor above has failed to acknowledge an alarm, and may not 41

have anyone responding to the situation at the pump station. The personnel at the
plant both day and night will be informed and instructed as to what should be
done in the event they receive an alarm. Plant personnel will call the maintenance
contractor to have a maintenance person sent to the pump station.
4. The manager of Dam Site 18. If the automatic dialer calls this individual all
else has failed. He will in turn acknowledge the alarm and notify the contractor
that there is an emergency situation at the pump station.
The general operation and maintenance services listed below shall apply to the
pump systems and other equipment as previously described.
+1. Examine each piece of equipment and device to see that it is functioning properly
and that it is in good operational condition.
+2. Clean all components of dust, old grease, etc., to allow the designed equipment to
function as when new.
+3. Inspect all linkages, motors, drives, etc. If a deviation from original design
settings or positions, inform maintenance person of required adjustments.
+4. Inspect all sensors, monitoring, output, safety and readout devices for proper
ranges, settings, and optimum efficiency. •
*5. Adjust all linkages, motors, drives, etc. that have deviated from the initial design
settings and positions. •
*6. Calibrate all sensing, monitoring, output, safety and readout devices for proper
ranges, settings, and optimum efficiency.
Repair the device by the addition or replacement of manufacturer approved parts,
should the above maintenance not be adequate.
Replace the device if needed, in view of its condition, age, and cost of previous
and subsequent repair.
*7. Test and cycle all equipment as a system after ; it has been cleaned, lubricated,
adjusted, and calibrated, to see that it is in proper operational condition and at
optimum efficiency.
*8. Assist the City by recommending stock items to be maintained in their
inventory initially, with the prospective contractor replacing any of the inventory
used during the period of the contract.

The preceding operation performance is estimated to take approximately two (2) hours
per week total, spread over three (3) visits per week.
The prospective contractor shall bid this service as a weekly lump sum amount.
Contractor is responsible for his own estimate of hours and cost
- Denotes duties performed by a qualified operations person.

C. PREVENTATIVE MAINTENANCE SERVICES
Quarterly perform:
1. Voltage -Amperage check of motor and compare with startup test runs - maintain
written record of these readings and file quarterly a copy of these readings with the
Plant Maintenance Engineer, Environmental Services.
2. Temperature check of pump bearings. •
3. Lubricate pump bearings. .
4. Check, clean and tighten all contacts and connections in the motor control center.
5. Check and verify operational sequence of controls.
6. After each service call, a confirming visit report shall be furnished.
Each of these quarterly inspections and tests estimated to take approximately three hours. The
prospective contractor shall bid this service as a quarterly lump sum amount. Contractor is
responsible for his own estimate of hours and cost.
Annually perform-,
1. Lubricate motor bearings.
2. Inspect impellers of the pumps.
This annual inspection and test is estimated to take approximately eight hours. The
prospective contractor shall bid this service as a annual lump sum amount. Contractor is
responsible for his own estimate of hours and cost.

D. PREVENTATIVE MVIALtiTENANCE
The contractor shall perform at least four (4), three (3) hour maintenance
inspections each year. During each inspection, the contractor shall perform
preventative maintenance in accordance with the written preventative maintenance
schedule (as outlined in P-aragraph DC).
The quarterly preventive maintenance shall be bid on a price per quarter, every
three months, date to start with the notice to proceed for four (4) quarters.
•
There shall also be an Annual Preventive Maintenance inspection once a year,
service time not to exceed eight (8) hours. This item shall be bid as a price per year
for one (1) year.
E. SAMPLING AND TESTING
In connection with the Nebraska Department of Environmental Quality NPDES
permit (copy attached) requirements, monthly and annual composite sampling must
be undertaken at two of three (3) locations. Sampling and testing will be conducted
in an approved manner as required by the NPDES permit for B.O.D., suspended
• solids, Ph, and Fecal Coliform indicator tests, with the contractor submitting the •
sampling analysis report not less than 15 days prior to the end of the quarter . (See
PAGE 2 for detailed information).
•
The prospective contractor shall bid this service as a lump sum amount. (Example,
Point A, bid price $120.00/yr, Point B, bid price $120.00/yr., this translates to a
bid price for Point A at $10.00/mo./site and $10.00/moJsite at Point B ) This
amount shall be used if additional sampling points are required within the
immediate area. Two (2) sampling points are included in this initial solicitation.
Two samples per month for twelve (12) months.
In addition to regular monthly sampling is a grab sample for fecal coliforms. This
occurs from 1 April through 30 September of each year. This is an additional
sampling cost, and also will be bid lump sum for a period of six months. One
•
sample per month for six months, conducted during the time of monthly sampling.
The lump sum bid for sampling and testing should follow this format; lump sum bid
for monthly sampling per site; lump sum bid for grab sample for fecal colifonn
testing per month from 4/1 through 9/30.
F. MOWING SERVICE
The Contractor shall also, submit as an additional service mowing for the facility.
The mowing service shall include areas shaded in attached drawing. This will
occur on an as needed basis, and will begin when requested by a representative of ,
the City of Omaha This should be bid as lump sum per request.

III. QUALIFICATIONS
A. Two (2) year minimum experience in commercial/industrial electrical and
mechanical maintenance services shall be required for all personnel on the job.
B. Services shall be provided by qualified and trained service personnel. Subcontracting
portions of the services to be provided shall not exceed fifty percent (50%) of the
total work for the year. The subcontractor selected by the prime contractor shall not
commence any work until written approval by the City of Omaha is issued to the
prime contractor. Prior to City of Omaha approval the subcontractor shall submit a
qualification record demonstrating the ability to perform the required tasks that are
being subcontracted. The contractor shall assume all responsibility and liability for
any and all subcontractors which he uses.
C. The prospective contractor shall ensure, the individual to perform the operational
functions has a valid State of Nebraska Department of Environmental Quality Class

IV. PARTS REPLACEMENT
A. All parts, components, or devices for the equipment being serviced that are worn or
are not in proper operational condition, shall be repaired, or replaced with new parts,
components, or devices of the same manufacturer or City approved equal.
B. When equipment or parts are replaced in their entirety, and a newer design of this
device/part is available, the device/part of the newer design shall be used as the
replacement if approved by the City.
C. All repair/replacement parts, components and devices for the equipment shall be
supplied by the contractor at cost plus percentage markup as bid.
D. All miscellaneous parts and supplies necessary to maintain the various systems and
equipment shall be supplied by the contractor and shall be included in the cost of the
service program that is, contractor is to provide air pressure gauges, lubricants, tools,
test instruments, meters, etc. required in the performance of his maintenance, repair,
replacement responsibilities.
V. EMERGENCY SERVICE CALLS
A. The contractor shall provide emergency services as needed on a 24 hour basis.
(See operational procedures for detailed information).
l. All labor costs expended on such a call shall be provided by the co tractor
and shall be at the hourly rate as indicated on the bid sheet.

VI. SERVICE PERFORMANCE GUARANTEES
A. WARRANTY
•
The contractor shall provide an ongoing warranty covering the workmanship and
services of himself, and subcontractor, if appropriate. All parts and materials shall
carry the manufacturers warranty and a facsimile of the manufacturers warranty shall
be submitted prior to installation of replacement parts.
B. PERFORMANCE REVIEW .
1. The City may review, at any time, the services provided and reports submitted,
to verify that the service is, in fact, being properly and adequately performed.-
Complaints, lack of service, or deficiencies in the performance of the contract
shall be submitted to the contractor in writing for correction. •
2. Chronic problems or deficiencies that reduce the efficiency or continuous
operation of the facility, or failure of the contractor to correct deficiencies
within the time period agreed upon, shall constitute cause for termination of
services upon thirty (30) days written notice.
VII. BONDS. PAYMENT. AND CONTRACT PERIOD
1. The contractor shall submit a bid bond or certified check in the amount required on the
• bid sheet.
2. To secure the full and faithful performance of each and all terms and conditions of this
contract, the contractor shall fiirnish a.performance bond,with a well-recognized surety
company, to be approved by the City of Omaha, in the amount of 50% of projected
scheduled total annual maintenance. The performance bond shall be for a period of one
(1) year and shall in part indemnify the City against any loss resulting from any failure
of performance by the contractor. In the event of renewal as provided in this
specification a sixty days notice of such contract extension be given to the contractor by
the City of Omaha at which time the contractor will furnish a new performance bond in
the same amount or an extension of the original bond.
•
3. The contract period shall be for a one-year. The contract may be extended monthly /
mutual agreement of both parties at no increase in contract cost. This extension s1 rtl;::'
not exceed an additional twelve (12) months.

4. Payment shall be made monthly upon approval by City of Omaha, upon presentation of
invoices for services performed.
Payment due the contractor shall be based on each job being itemized and invoiced as
an individual item, with sufficient detail to define the scope of work. Evidence of cost
of material and rented equipment necessary for peribrimance of work must be provided
and the cost to the contractor shall be shown including all items for which the quantity
is readily ascertainable. Rental equipment required shall be equipment which is not
usual to the regular maintenance program.

All work must be authorized by the City of Omaha's Environmental Services Division.
Work will be authorized by the Plant Maintenance Supervisor or his designated agent.
Once work is authorized, it is to be completed in the most diligent, professional and
expeditious manner.

VllI. INSURANCE
Contractor shall carry Workmen's Compensation Insurance in accordance with the laws of
the State ofNebraska to protect himself or itself and the City of Omaha against liability
• under the Worker's Conipensation and Occupational Disease Statutes of the State of
Nebraska
Contractor shall carry in his or its name'a policy or policies under a comprehensive firm
to insure contractual and automobile liability as well as operations other than automobile
with coverage of not less than $300,000 each person, $500,000 each accident bodily injury
liability, and $300,000 each accident for property damage. This policy shall also name the
City of Omaha, its officers, agents and employees, AS ADDITIONAL INSURED.
IX. GENERAL
A. Contractor shall conform with all ordinances, laws, rules and regulations
applicable to this contract, and his operations shall be in accordance therewith.

B. The contractor shall indemnify and save harmless the City of Omaha, its officers,
employees, and agents from all claims, suits or actions of every kind and character
made upon or brought against the said City of Omaha, its officers, employees and
agents, for or on account of any injuries or damages received or sustained by any
party or parties by or from the acts of the said contractor or its servants, agents, and
sul.-contractors, in doing the work and rendering the services herein contracted for,
or by or in consequence of any negligence in operations or any improper material
or equipment used, or by or on account of any act or omission of said contractor or
his or its servants, agents, and sub-contractors; and also from all claims or damage
for infringement of any patent in fulfilling this contract. This indemnity shall include
attorney's fees and costs and all other expenses incurred in the defense of any suit.

necessary to establish the ability of any bidder to perform all conditions of the
contract in order to avoid awarding a contract to a firm unable to produce the
quality of service required.and intended by this specification.

A. OTHER REQUIREMENTS
1 . The arithmetic mean of the values for effluent sampling measuring
biochemical oxygen demand ( 5-day) and suspended solids collected in
a period of 30 consecutive days shall not exceed 15 percent of the
arithmetic mean of the values for influent sampled collected at
approximately the same times during the same period (85 percent
removal- minimum) .

2. There shall be no discharge of floating solids or visible foam in
other than trace amounts .
3. Sludge shall be disposed of or utilized in a manner approved by the
Department of Environmental Control .
4 . If an applicable standard for sludge use or disposal or numerical
limitations for pollutants in sewage sludge , promulgated under
Section 405(d) (2) of the Clean Water Act as amended by the Water
Quality Act of . 1987 , is more stringent than the sludge pollutant
limit or manage1oent practice in this permit , or controls a
pollutant not limited in this permit , this permit shall be modified
or revoked and reissued to conform with the requirements
promulgated under Section 405(d) ( 2)..
5. The permittee shall give prior notice to the Director of any change
planned in the permittee's sludge disposal practice .
6. This facility requires a Class I Certified Operator in accordance
with NDEC Title 197 , Rules and Regulations for the Certification of
Wastewater Treatment Facility Operators in Nebraska .
7 . The permittee shall request a modification of this permit from the
Director prior to the land application of the treated wastewater
( irrigation) from the holding basin .

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS,bids were received on November 15, 1995 for Dam Site 18 wastewater
operation, inspection, maintenance and repair including pump,pump station and testing of holding
basin effluent; and,
WHEREAS, Commonwealth Electric Company submitted a bid of$23,773.00,being
the lowest and best bid received within the bid specifications for Dam Site 18 wastewater operation,
inspection,maintenance and repair.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF OMAHA:
THAT,the bid of$23,773.00 from Commonwealth Electric Company for Dam Site
18 wastewater operation, inspection, maintenance and repair including pump, pump station and
testing of holding basin effluent,a copy of the bid documents is attached and by this reference made
a part hereof, being the lowest and best bid received within the bid specifications,be accepted and
the purchase order be issued.
BE IT FURTHER RESOLVED:
THAT, the Purchasing Agent be authorized to issue a purchase order to
Commonwealth Electric Company for Dam Site 18 wastewater operation,inspection, maintenance
and repair.
BE IT ALSO FURTHER RESOLVED:
THAT, the Finance Department is authorized to pay the cost of Dam Site 18
wastewater operation,inspection, maintenance and repair from the Flood Control and Water Quality
Organization 1451,Fund 001.
