The attached proposed Resolution approves a Supplemental Agreement between the City of Omaha
and the State of Nebraska Department of Economic Development. The original Agreement was
approved by the City Council on August 1, 1995, by Resolution No. 1885, in the amount of
$29,783.00, to provide shelter and services to homeless persons.
This agreement extends the termination date of the Agreement from June 1, 1995, to September 1,
1996. .All activities were completed and all funds expended before the revised termination date.
This Supplemental Agreement provides for reimbursement of funds to the City of Omaha for
expenses incurred by service providers after the original termination date. All other terms and
conditions of the original agreement remain unchanged.

95-ESG-022
WHEREAS,on August 1, 1995,by Resolution No. 1885,the City Council of the City
of Omaha entered in to an Agreement with the State of Nebraska Department of Economic
Development;-in the amount of$29,783.00,to provide shelter and services to homeless persons; and,
WHEREAS,this Supplemental Agreement revises the previous Agreement between
the City of Omaha and the State of Nebraska by extending the termination date of the Agreement
from June 1, 1995, to September 1, 1996; and,
WHEREAS, all activities were completed and all funds expended before the revised
termination date; and,
WHEREAS,this Supplemental Agreement provides for reimbursement of funds to
the City of Omaha for expenses incurred by service providers after the original termination date;and,
WHEREAS, all other terms and conditions of the original agreement remain
unchanged.
It is hereby agreed to,by the parties to 95-ESG-022 that the following section of the
original agreement is amended to read as noted. This document is effective upon the date of
signature by both parties.

1.04 Time of Performance.
The period covered under this Contract will be from June 1, 1995,until September
1, 1996. The termination date of this Contract will be September 1, 1996. All of the
required activities and services, except for administration and audit, will be
completed by or before this date.
