Transmitted herewith is a Resolution approving a Contract Change Order with Hawkins
Construction Company for the construction of the hockey locker facilities as part of the Civic
Auditorium Renovation project. The City Council, on May 14, 1996, authorized this project when
it approved the renovation contract and the agreement with the University of Nebraska at Omaha for
the installation of the ice rink for the university hockey program.
The hockey facility was relocated from the interior space of the arena to the western exterior of the
Auditorium east of the parking garage in an effort to accommodate the floor space requirements of
"flat show" users; Leo A. Daly Company was authorized to prepare plans and specifications for the
locker facilities.
Bids were received for the hockey locker facilities on January 15, 1997 with bids being received
from twelve contractors as shown on the attached Bid Tabulation Sheet and Hawkins Construction
Company submitting the lowest bid of$442,000.00 for this project. In addition to the cost of the
hockey locker structure,Hawkins Construction Company will provide fixtures previously included
in the renovation contract but being relocated from the prior design concept to the current location.
Since Hawkins Construction Company is the low bidder,the work is being recommended under a
Contract Change Order to avoid the necessity of a new contract,new performance bond and assorted
documentation. The value of this Change Order, however, must be approved by the City Council
prior to work initiation in accord with Section 10-142, Omaha Municipal Code. This Contract
Change Order will be paid from the Civic Auditorium Ice Facility Fund 337, Organization 2338.

The bidder hereby states that the above total cost to the City of Omaha includes all material, taxes, insurance royalties;
transportation charges, use of all necessary tools and equipment, supervision, inspection and tests of materials, bond and
overhead expense, all equipment furnished in accordance with the requirements of the contract documents considered
severally and collectively.

It is understood that the contract documents shall consist of the advertisement,'the requirements for bidding and instruction

and dated: 20 DECEMBER 1996 , the specifications and the duly executed agreement or contract proper; that, said
complementary and that which is required, shown or specified by any one of these, shall be as binding as if required by all.
The bidder hereby represents that the price contained in this proposal is neither directly nor indirectly the result of any
agreement or any secret understanding with anyone with respect to any provisions contained in this proposal.
In accordance with the requirements for bidding and instructions to bidders, there is deposited herewith a bid bond or a
