Professional Services at Knolls Golf Course, located at 1630 Sahler Street, Omaha, Nebraska, for the Parks, Recreation, and Public Property Department; and, WHEREAS, JEO Consulting Group, Inc. has agreed to perform the additional work for $6,500.00; the original quote was $18,000.00. This Change Order is required because of additional design services are needed to complete a golf cart parking area. All other provisions of the original contract shall remain in full force and effect; and, WHEREAS, JEO Consulting Group, Inc. has complied with the City's Contract Compliance Ordinance No. 28885 in relation to Civil Rights - Human Relations; and, WHEREAS, the Mayor has recommended acceptance of Change Order No. 1, NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA: THAT, as recommended by the Mayor, Change Order No. 1 to the proposal that awarded JEO Consulting Group, Inc. Professional Services at Knolls Golf Course, located at 1630 Sahler Street, Omaha, Nebraska. The Change Order will add cost because of additional design services are needed to complete a golf cart parking area, is hereby approved. A site plan showing this area is included as Exhibit 'C'. The following tasks are based on information provided by John William, LA/Park Planner for City of Omaha Parks, Recreation and Public Property Department(Client): 1. TOPOGRAPHIC SURVEY 1.1 Engineer will schedule and obtain a topographic survey containing the following: 1.1.1 Survey the locations of all visible physical features (concrete, asphalt,gravel, rock, driveways,sidewalks, trees, utility poles,valves, manholes,signs, drainage structures, curb stops,water meter pits,terrain profiles, etc.). 1.1.2 Collect available utility location information and incorporate on preliminary plans (gas, telephone, electric, water,sanitary sewer, storm sewer, communications, etc.). 1.1.3 Create an electronic drawing using AutoCAD illustrating elevations,site features, and existing utilities resulting from the surveys performed. 1.1.4 See attached exhibit C for survey area. 2. SITE DESIGN AND CONSTRUCTION PLANS 2.1 Develop the final construction plans for the proposed improvements in accordance with the Owner's guidance and the local jurisdictions requirements. Construction plans and documents to include the following: 2.1.1 Cover sheet and general location maps 2.1.2 Existing Conditions/ Removal Plans 2.1.3 Site Layout Plans 2.1.4 Site Grading Plans 2.1.5 Site Erosion Control Plans 2.1.6 Site Construction Details 2.1.7 Technical Specifications—City to provide front end sections 2.2 Attend up to two meetings with the Owner to review the construction documents. (In Person) 2.3 Conduct an internal QA/QC review of the 90%construction documents and specifications. 2.4 Revise the plans and specifications as necessary following the final review(s) and sign and seal by a professional landscape architect or civil engineer registered in the State of Nebraska. 3. OWNER RESPONSIBILITIES 3.1 Provide timely review of documents or requests for information. 3.2 Provide access to property for survey and investigation services. Exhibit A: Page 1 of 2 4. FEE 4.1 JEO proposes to provide the services defined above for the lump fees defined below: Task Fee Topographical Survey $ 2,000.00 Site Design and Construction Plans $ 4,500.00 Total (Not to Exceed) $ 6,500.00 4.2 Progress Payments: 4.2.1 JEO will bill for services completed near the end of each month.All invoices are due payable upon receipt and are considered delinquent after 30 days. 4.2.2 Invoices not paid within 30 days may be charged interest at the annual rate of 12% (1.0%/month). 4.2.3 Payments will be applied first to the interest then principal. 4.2.4 Work by JEO will cease if invoices have not been paid in full within 60 days and will not begin again until full payment with interest has been received. 5. CONTRACT TIME 5.1 JEO will work as expeditiously as possible, pending authorization from Owner to complete the tasks in this project. 6. EXCLUSIONS 6.1 Geotechnical services 6.2 Architectural services 6.3 SWPPP Permitting 6.4 SWPPP Inspection services 6.5 Bidding and Negotiating 6.6 Construction Administration 6.7 Value engineering design services 6.8 Retaining wall design 6.9 Review fees associated with building permits, etc. 6.10 Construction staking 6.11 Any other item not outlined in the scope of services 7. REIMBURSABLE EXPENSES 7.1 Typical reimbursable expenses are included in the lump-sum and include: Mileage for trips required to complete the work defined above, long-distance phone calls, meals,other travel expenses, software, copies/prints, and faxes. 7.2 Other reimbursable expenses shall be billed at 110%of their cost. (None are anticipated on this project.) 8. ADDITIONAL TERMS 8.1 The General Conditions are specified in Exhibit B.
