RE: Construction Materials Testing Services
Omaha Park Six Parking Structure
Omaha, Nebraska
PSI Proposal Number: 171-017 Revised
Dear Mr. Peterson:
Professional Service Industries Inc. (PSI) is pleased to submit the following proposal
for providing construction materials testing services for the Omaha Park Six project, as per
the project specifications located in the Dodge Room.
PSI has performed a cursory examination of the project plans and specifications. In
order to arrive at an estimated fee, several assumptions have been made. These assumptions
include, but are not limited to, the quantity of materials to be placed and contractor
scheduling. These assumptions could be impacted by the actual frequency of service requests
by your on-site personnel. Based on our assumptions, it is estimated that the cost to provide
inspection and testing services will be in the order of $86,000.00.
Project Understanding
PSI understands that the project consists of a technician(s) to perform inspection of
footings, casting sets of 6 and 8 concrete cylinders, testing concrete for air content, slump,
temperature for every truck on site, compressive strength of cylinders, and inspection of high
strength bolted connections on an on-call basis as requested by city representative.

are enclosed herewith and incorporated into this proposal. PSI's total fees would be
determined by the actual amount of technical time expended for this project and the amount
of laboratory testing performed.
Authorization
PSI will proceed with the work on the basis of verbal authorization. Please sign and
return one (1) copy of this proposal intact.
PSI appreciates the opportunity to offer its services to your project and looks forward
to working with you during the construction phase. If you have questions, or would like to
