This project is located between 39th and 42nd Streets and between "T" and "V" Streets. This work was planned as the Monroe Watershed Sewer Separation project and is included in the 1998-2002 Capital Improvement Program as Project#105. The attached Agreement authorizes Hill-Farrell Associates,Inc.to provide a survey along the length of the project. The preliminary survey is needed before detailed design work can be started. Details of what will be included in the survey are included in the Agreement as Exhibit "B". Hill-Farrell Associates, Inc. has agreed to perform the services detailed in the attached agreement for a fee not to exceed $24,950.00, which is payable from the Sanitary Sewer Improvement Organization 1485, Sewer Revenue Improvement Fund 544. Hill-Farrell Associates,Inc. was selected to perform the required engineering services in accord with the Architect's and Engineer's Review and Selection Ordinance. Hill-Farrell Associates, Inc. has filed the required Annual Contract Compliance Report Form, CC-1. As is City policy,the Human Relations Director will review the consulting firm to determine its compliance with Ordinance 28885. D-vis, Jr. Date Finance Director Human C'elations Director • 0 P:\PW1\7572.SKz L�N. i PROFESSIONAL SERVICES AGREEMENT THIS AGREEMENT is hereby made and entered into this 24th day of April, 1998, by and between the City of Omaha, a municipal corporation located in Douglas County, Nebraska (hereinafter referred to as the "City"), and Hill_Farrell associates, Inc. (hereinafter referred to as the "Provider"), on the terms, conditions and provisions as set forth hereinbelow. I. PROJECT NAME AND DESCRIPTION Preliminary survey for sewer separation project. RNC 5588,42nd & "U" St. II. DUTIES OF PROVIDER A. Provider agrees to perform professional services, as set out and more fully described in the Proposal attached hereto, for the City, relative to the above-referenced project which is illustrated in Exhibit"A" attached hereto. Such services shall be completed within a120 day period after receipt of a purchase order from the City. B. Provider designates Mr. Bill Farrell whose business address and phone number are 1008 Lincoln Rd., Bellevue, NE 68005, (402) 291-6100, as its project manager and contact person for this project. C. Provider agrees to maintain records and accounts, including personnel, financial and property records, sufficient to identify and account for all costs pertaining to the project and certain other records as may be required by the City to assure a proper accounting for all project funds. These records shall be made available to the City for audit purposes and shall be retained for a period of five(5)years after the expiration of this Agreement. D. Provider agrees to prepare a schedule of compensation, detailing hourly rates for all compensated providers, employees, and subcontractors. E. Provider agrees to complete,within 120 calendar days of receipt of a purchase order from the City, the necessary services. The City recognizes that completion within this deadline is contingent upon timely response from utilities and City input. HI. DUTIES OF CITY A. City designates Ronald Neal whose business address and phone number are 6880 Q St., Omaha,NE 68117, (402) 444-4966, as its contact person for this project, who shall provide a notice to proceed and such other written authorizations as are necessary to commence for proceed with the project and various aspects of it. IV. COMPENSATION AND PAYMENT A. The cost of services as specified in the Scope of Service, shall be performed on an hourly basis, but in no event shall exceed$24 ,.9 5 0 . 0 0 B. Reimbursable expenses shall be billed to the City by the Provider. C. INCREASE OF FEES The parties hereto acknowledge that, as of the date of the execution of the Agreement, Section 10-142 of the Omaha Municipal Code provides as follows: Any amendment to contracts or purchases which taken alone increase the original fee as awarded (a) by ten percent, if the original fee is one hundred fifty thousand dollars ($150,000) or more, or(b) by seventy-five thousand dollars ($75,000) or more, shall be approved by the City Council in advance of the acceptance of any purchase in excess of such limits. However, neither contract nor purchase amendments will be split to avoid advance approval of the City Council. The originally approved scope and primary features of a contract or purchase will not be significantly revised as a result of amendments not approved in advance by the city council. The provisions of this section will be quoted in all future city contracts. Nothing in this section is intended to alter the authority of the mayor under section 5.16 of the Charter to approve immediate purchases. V. OWNERSHIP All plans and specifications provided pursuant to the terms of this Agreement, including, but not limited to,reproducible mylars, shall be and become the property of the City. No additional compensation shall be due the Provider from the City for such plans. However, if the plans are re-utilized on projects other than those noted above, by the City, the City agrees to first contact the Provider to contract with the Provider for any required adaptations, contingent upon the negotiation of a fee for this service acceptable to both the City and the Provider. VI. ADDITIONAL SERVICES In the event additional services for the aforementioned project not covered under this Agreement are required, Provider agrees to provide such services at a mutually agreed upon cost. VII. INSURANCE REQUIREMENTS Provider shall carry professional liability insurance in the minimum amount of one half million dollars and shall carry workers' compensation insurance in accordance with the statutory requirements of the State of Nebraska. VIII. INDEMNIFICATION (1) To the fullest extent permitted by law, the Provider shall indemnify and hold harmless the City, its agents and employees, their successors and assigns, individually and collectively, from and against all claims, suits, damages, fines, losses and expenses, including but not limited to attorneys' fees, arising out of or resulting from the provision of services under this Agreement, provide that such claim, suit, damage, fine, loss or expense is attributable to bodily injury, sickness, disease or death, or to injury to or destruction of tangible property 2 • including loss of use resulting therefrom, but only to the extent caused in whole or in part by negligent acts or omissions of the Provider, anyone directly or indirectly employed by it, or anyone for whose acts it may be liable, regardless of whether or not such claim, suit, damage, fine, loss or expense is caused in part by a party indemnified hereunder such obligation shall not be construed to negate, abridge, or reduce other rights or obligations of indemnity which would otherwise exist as to a party or person described in this paragraph. Any reuse of the plans or specifications by the City on another project, without the consent of the Provider,will be the sole responsibility of the City,for which this indemnity shall not apply. (2) In claims against any person or entity indemnified under this paragraph by an employee of the Provider, anyone directly or indirectly employed by it, or anyone for whose acts it may be liable, the indemnification obligation under this paragraph shall not be limited by a limitation on amount or type of damages, compensation or benefits payable by or for the Provider under workers' or workmen's compensation acts, disability benefit acts or other employee benefit acts. IX. TERMINATION OF AGREEMENT This Agreement may be terminated by the City upon written notice to the provider of such termination and specifying the effective date at least seven (7) days prior to the effective date of such termination. In the event of termination, the provider shall be entitled to just and equitable payment for services rendered to the date of termination, and all finished or unfinished documents, data surveys, studies, drawings, maps, models, reports or photographs shall become, at the City's option, its property. X. GENERAL CONDITIONS A. Non-discrimination. Provider shall not, in the performance of this Agreement, discriminate or permit discrimination in violation of federal or state laws or local ordinances because of race, color, sec, age, or disability as recognized under 42 USCS 1210] et seq. and Omaha Municipal Code section 13-89,political or religious opinions, affiliations or national origin. B. Captions. Captions used in this Agreement are for convenience and are not used in the construction of this Agreement. C. Applicable Laws. Parties to this Agreement shall conform with all existing and applicable city ordinances, resolutions, state laws, federal laws, and existing and applicable rules and regulations. Nebraska law will govern the terms and the performance under this Agreement. D. Interest of the City. Pursuant to Section 8.05 of the Home Rule Charter, no elected official or any officer or employee of the City shall have a financial interest, direct or indirect, in any City Agreement. Any violation of this section with the knowledge of the person or corporation contracting with the City shall render the Agreement voidable by the Mayor or Council. 3 t E. Interest of the Provider. The Provider covenants that he presently has no interest and shall not acquire any interest, direct or indirect, which would conflict with the performance of services required to be performed under this Agreement; he further covenants that in the performance of this Agreement, no person having any such interest shall be employed. F. Merger. This Agreement shall not be merged into any other oral or written agreement, lease or deed of any type. This is the complete and full agreement of the parties. G. Modification. This Agreement contains the entire Agreement of the parties. No representations were made or relied upon by either party other than those that are expressly sat forth herein. No agent, employee or other representative of either party is empowered to alter any of the terms hereof unless done in writing and signed by an authorized officer of the respective parties. H. Assignment. The Provider may not assign its rights under this Agreement without the express prior written consent of the City. I. Strict Compliance. All provisions of this Agreement and each and every document that shall be attached shall be strictly complied with as written, and no substitution or change shall be made except upon written direction from authorized representative. J. Equal Employment Opportunity Clause. Annexed hereto as Exhibit "C" and made a part hereof by reference are the equal employment provisions of this contract. All reference in Exhibit "C" to "Contractor" shall mean "Provider". Refusal by the Provider to comply with any portion of this program as therein stated and described will subject the offending party to any or all of the following penalties: (1) Withholding of all future payments under the involved contracts to the Provider in violation until it is determined that the Provider is in compliance with the provisions of the contract; (2) Refusal of all future bids for any contracts with the City or any of its departments or divisions until such time as the provider demonstrates that he has established and shall carry out the policies of the program as herein outlined. EXHIBIT "C" During the performance of this contract,the contractor agrees as follows: 1) The contractor shall not discriminate against any employee applicant for employment because of race, religion, color, sex, national origin, or disability as defined by the Americans With Disabilities Act of 1990. and Omaha Municipal Code 13-89. The contractor shall take affirmative action to ensure that applicants are employed and that employees are treated during employment without regard to their race, religion, color, sex or national origin. The contractor shall take all actions necessary to comply with the Americans With Disabilities Act of 1990 and Omaha Municipal Code (Chapter 13) including, but not limited to, reasonable accommodation. As used herein, the word "treated" shall mean and include,without limitation,the following: Recruited, whether advertising or by other means; compensated; selected for training, including apprenticeship; promoted; upgraded; demoted; downgraded; transferred; laid off; and terminated. The contractor agrees to and shall post in conspicuous places,available to employees and applicants for employment, notices to be provided by the contracting officers setting forth the provisions of this nondiscrimination clause. 2) The contractor shall, in all solicitations or advertisements for employees placed by or on behalf of the contractor, state that all qualified applicants will receive consideration for employment without regard to race,religion, color, sex or national origin. 3) The contractor shall send to each labor union or representative of workers with which he has a collective bargaining agreement or other contract or understanding a notice advising the labor union or worker's representative of the contractor's commitments under the equal employment opportunity clause of the city and shall post copies of the notice in conspicuous places available to employees and applicants for employment. 4) The contractor shall furnish to the contract compliance officer all federal forms containing the information and reports required by the federal government for federal contracts under federal rules and regulations, and including the information required by section 10-192 to 10-194, inclusive, and shall permit reasonable access to his records. Records accessible to the contract compliance officer shall be those which are related to paragraphs (1) through (7) of this subsection and only after reasonable notice is given the contractor. The purpose of this provision is to provide for investigation to ascertain compliance with the program provided for herein. 6 • EXHIBIT "C" (continued) 5) The contractor shall take such actions with respect to any subcontractor as the city may direct as a means of enforcing the provisions of paragraphs (1) through (7) herein, including penalties and sanctions for noncompliance; however, in the event the contractor becomes involved in or is threatened with litigation as the result of such directions by the city, the city will enter into such litigation as is necessary to protect the interests of the city and to effectuate these provisions (of this division); and in the case of contracts receiving federal assistance, the contractor or the city may request the United States to enter into such litigation to protect the interests of the United States. 6) The contractor shall file and shall cause his subcontractors, if any to file compliance reports with the contractor in the same form and to the same extent as required by the federal government for federal contracts under federal rules and regulations. Such compliance reports shall be filed with the contract compliance officer. Compliance reports filed at such times as directed shall contain information as to the employment practices,policies, programs and statistics of the contractor and his subcontractors. 7) The contractor shall include the provisions of paragraphs (1) through (7) of this section, "Equal Employment Opportunity Clause," and section 10-193 in every subcontract of purchase order so that such provisions will be binding upon each subcontractor or vendor. 7 HILL-FARRELL ASSOCIATES. XX-X 8/26/ 7 • PUBLIC WORKS DEPARTMENT • DESIGN DIVISION SURVEYING SCOPE 1. ) GENERAL : R N C 5588 1.1 Intent It is the intent of this prepared scope to identify all survey requirements, define the limits of the area to be surveyed and establish an acceptable guideline for the appropriate task performances. The goal shall be to assure that all necessary work is carried out in a reasonable and timely manner, thereby minimizing the potential for costly and untimely follow-up of task repetition or additional field operations. This should provide the Surveyor, and the City, a greater opportunity for the development of a clear understanding of the City's actual survey needs on a more specific Project by Project basis. 1.2 Standard Work The Section 2 and Section 3 survey scope requirements, while setting the tone for general guidance of conformity, shall also serve as the Standard or minimum level of work to be performed. The Surveyor's actual performance regarding Procedures and Methods , once reviewed and found to meet all of the requirements as set forth in Sections 2 , 3, and possibly where modified under Section 4, shall constitute the full measure for surveying scope compliance. 1.3 Modifications Only where specifically stated within Section 4 "SURVEYING SCOPE MODIFICATIONS", shall changes to the Standard Work be recognized. Section 4 modifications shall be as originally provided for within the Surveying Scope as attached to the accepted proposal. 1.4 Variances Should it become necessary to enact a variance from the Standard Work, once the work has been undertaken, an "AMENDMENT" to the previously accepted scope shall be drafted, and accepted in writing by both the City's Project Manager and the Surveyor. Variances, which result in the addition, deletion, reduction or expansion of the standard tasks, or that significantly change the manner for performing those tasks shall not be enacted by the surveyor without the prior written approval of the City's Project Manager. - -1.5 Acceptance All work shall be completed and provided to the City's Project Manager within the allotted time as specified in the Surveyor's proposal. Upon receipt, the City will review the work for conformance with the scope. The City shall be afforded a reasonable opportunity to accommodate a review of the Surveyor's submittal, (not less than FIVE working days, nor longer than TEN working days). . The City's Project Manager shall not be authorized to release funds to the Surveyor in excess of either 90%of the to date Final billing amount, or 90%of the Contracted amount, which ever being the lesser amount, prior to the City's review and acceptance of all the work performed. 2 97ddss0801 city project no. XX-X 8/26/9 2. ) PROCEDURES : 2.1 Project Awareness Surveying methods (whether Conventional or Electronic) to be employed either in part (by task)or in total (by project) shall be as stated and accepted within the Surveyor's proposal for the work. It shall be recognized that the City does have particular needs with regard to the level of conformity and continuity required when addressing the assembling, engineering,plan preparation and record archiving of the Improvement Projects which are constructed and maintained for the benefit of the Public. Evaluation, design and construction of improvements by the City, and work performed by others on behalf of the City require that specific methods may be applied to electronic data collection and presentation. The methods applied, together with the specified tools(software)shall meet current Public Works requirements. Those current requirements are Autocad-Release 13, coupled with Softdesk 7.5 . These programs are nationally recognized,broad based tools of the civil engineering industry. Capabilities, such as those field operations which incorporate line connectivity offer significant efficiency gains by streamlining the necessary transition between field data collection and plan preparation. All field work, procedures, field book reduction or notation, together with any drafting or other technical data presentation shall be in accordance with acceptable Public Works Department Policies and Procedures. The Surveyor shall familiarize himself/herself fully as to the nature and location of the work, the project limits, the character and accessibility of the site and any specific requirements within the methods to be applied in the performance of the work. The Surveyor may obtain from the Public Works Department, copies of applicable plans and other such information as may be available to complete the survey. NOTE : Prior to beginning the actual field operations, the Surveyor shall meet with the City's:Project. Manager or his/her designated'.representative to review the total work to be performed as outlined under Sections 3 and as may be modified within Section 4 of this scope. 2.2 Communication The Surveyor shall maintain liaison with the City's Project Manager in order to plan, schedule, coordinate and carry out the work. The Surveyor shall be prepared to meet with the City upon request of the City's Project Manager to discuss and/or resolve unforeseen problems encountered during the performance of the work,whether it be related to field surveying or support operations. When required,establishing a schedule of routine contacts and/or meetings with the City's Project Manager on a more regular basis shall be provided for as outlined under the Section 4 modifications. 2.3 Documentation 2.3.1 FB-Field Book Field books will be used on all projects. All field books will be provided to the Surveyor by the City and shall remain the property of the City. The field book(s)shall be picked up and logged in by the Surveyor at the Public Works Survey Office located at 5225 Dayton Street. The Surveyor may pre-arrange the assembling of the field book(s)and bench mark references between the hours of 7:30 a.m. and 9:00 a.m. (phone 444-5147). Conventional surveying methods require that all data, notation,and computations be recorded in these field books. Where electronic data collection is employed, all horizontal and vertical control, ties to control points, sewer and storm drainage information, and computations shall be recorded by the Surveyor in these field books and shall be submitted as further outlined in Section 3 of this survey scope. 3 97ddss0801 city project no. X.X. 8/26/97 • • 2.3.2 EDC-Electronic Data Collector Software: Data collection shall be performed and presented with the specified software( Softdesk 7.5 ). Software other than that specified by the Public Works Department shall not be substituted by the Surveyor, except where previously approved, accepted and acknowledged under Section 4 " SURVEYING SCOPE MODIFICATIONS". Included in unacceptable software are all "Updates", "Upgrades and or any "New Releases" of the current specified software which have not received the appropriate review, evaluation and acknowledgment by the Public Works Department. Software compatibility, as it relates to associated needs within the Department is a priority and shall remain one of the major determining considerations. Descriptor Codes : All data collected in the data collector shall incorporate Standard Design Division Point Descriptors, and shall be submitted to the City on 3 1/2" floppy diskette(s). A current list of descriptors is provided herein under Exhibit 2.3.2 "Current Listing-SDD Point Descriptors". Data represented shall list the Point number, Northing, Easting, Elevation, DESCRIPTION comma delineated. Values of coordinate and elevation shall be recorded and presented having a minimum of (4)four decimal places. Example: 200,1234.5678,2345.6789,1208.1234,WALL TOP BEGIN STONE 300,1234.7345,2345.8456,1204.7654,WALL BOT BEGIN STONE Within the DESCRIPTION portion of the example as shown above, the following shall be presented. The total representation of the DESCRIPTION shall be limited to no more than twenty(20)characters, with the Descriptor Code entered first ( indicated here by WALL ). Where necessary, the descriptor code shall be followed by additional identifying data ( indicated here by TOP BEGIN STONE). Codes and descriptions shall be input using only UPPER CASE characters and presented with single space delineation. Detailed descriptions shall not include commas. NOTE Prior to the work, the City will provide 3 1/2"floppy diskette(s)to the.Surveyor containing drawing layer and descriptor data relative to the current :Standard Design Division Point Descriptors:: Line Connectivity : The following data collection activity shall incorporate Line Connectivity: (BC) @ BACK OF CURB LINES (PAVE) @ PAVEMENT EDGE LINES....and (PAVE CL JT) @ PAVEMENT CENTERLINE JOINT (GUT) @ GUTTER LINES (DRV) @ DRIVEWAY EDGE and (PRKG) @ PARKING LOT EDGE (WALK) @ SIDEWALK EDGE 2.3.3 Submittals All surveys (conventional and electronic) require the submittal of the FIELD BOOK(s). The book(s)shall be delivered to the Project Manager, together with a copy of all pertinent documents that were utilized by the Surveyor in determining or establishing base lines and horizontal control, right of way and property or lot lines, i.e. 1/4 section maps, plats, recorded land or lot surveys and legal descriptions. Surveys where Electronic Data Collection is employed shall require additional submittals as follows: A print representing the Surveyor's compilation or check print drawing. The check print shall be limited in scale to no smaller than 1"=40'. XX-X 8/26/97 • • 3. ) METHODS : 3.1 Preliminary Survey 3.1.1 Survey Limits The actual street,path or area location, including the required limits of the field work beginning and ending points shall be defined as specified under Exhibit 3.1.1 "SURVEY LIMITS ". 3.1.2 Survey Corridor The actual width for inclusion within the survey area along a particular street, path or other such designation shall be as indicated under Exhibit 3.1.2 "SURVEY CORRIDOR". Unless otherwise provided for, a singular presented Corridor width shall represent the total intended width for the performance of a complete survey along that Corridor, and the width specified shall be equally divided on either side of the centerline and/or baseline. NOTE While the Survey Corridor width specified: is in`general the limiting-.area for complete survey ....performance;..And represent the perimeter: cut off for Full Width cross sectioning, there are two(2) specific items of work to be performed;beyond or outside of the Corridor. These items shall be:considered routine ( included in the Standard Work );and are comprised of the recording of building corner;locations and additional driveway elevations which extend to the floor of garages in proximity to the Survey Corridor:. 3.1.3 Daily Entry Daily survey field book(FB) and electronic data collector (EDC)entries shall include the following general information. (a) Bench marks shall be established along the project at regular intervals not to exceed three-hundred (300) feet, unless other points of known elevation have been previously established. On the intersecting streets,bench marks shall also be established beyond the limits of the project. Bench marks shall be established only on points of permanent character. Fire hydrants are adjusted regularly,and shall be utilized only as temporary bench marks. A minimum of three(3) permanent bench marks shall be established per project. Descriptions of bench marks shall be recorded in the field books. (b) USGS datum shall be used. (c) A closed level circuit is required to verify elevation of all bench marks. Elevations shall check within 0.05 feet per square root of the distance in miles. All bench marks shall be recorded in the field book and when employed, recorded in the data collector. (d) Established permanent bench marks are kept on file for reference at the Public Works Survey Office located at 5225 Dayton Street. The Surveyor shall provide to the Survey Office,all newly established permanent bench marks as may be created by the Surveyor in the performance of the work. These new bench marks shall be logged into the file with the Public Works Survey Office prior to the submittal of the work to the City's Project Manager. (e) All vertical control points shall be recorded in the field book, and when employed, recorded in the data collector to be included in the digital point file. 3.3 Horizontal Control (a) Horizontal control shall be established by referencing to existing section or quarter-section corners,and lot pins. All located corners and lot pins shall be shown on a sketch in the field book,with both platted and measured distances. Those lot pins used to establish the centerline and/or baseline shall be noted as such. At no time shall it be assumed that the pavement is centered in the right-of-way. (b) If required, stationing shall be from Southerly to Northerly and from Westerly to Easterly. When applicable, every attempt should be made to use the stationing of a prior survey. Stationing will be provided for all conventional surveys, but shall only be appropriate under electronic data collection when specified under Section 4 "Survey Scope Modifications". (c) Ties to control points shall be established from topographic features of permanent character and recorded in the field book when established. Distances between control points and ties shall be measured horizontally. (d) Record the points of beginning and ending of the survey. On curved right-of-way, record a11- points of intersection (P.I. ) , points of curvature (P.C. ), points of tangency (P.T. ) , and other pertinent points as may be necessary to enable others to re-establish the alignment. (e) All traverses shall be closed within a minimum accuracy of 1:5000. (f) All horizontal control points shall be recorded in the field book, and when employed, recorded in the data collector to be included in the digital point file. 7 97ddss0801 city project no. X.X. XX-X 8/26/97 • 3.4 Topographic Data Requirements 3.4.1 General All fixed physical items and cross sections shall be recorded within the required area of survey,as defined by Exhibit 3.1.2 the "SURVEY CORRIDOR". Singular topography items such as trees,bushes, poles, posts and manholes, etc. shall be referenced to their center. Items of irregular shape, configuration or alignment shall be presented with a sufficient number of appropriate point references to accurately identify the feature. These points shall identify the beginning or end of a feature, changes in direction, such as a corner (COR) or turning point (TP) , point of curvature(PC), point of reverse curvature(PRC), or point of tangency (PT). The minimum number of references within any segment of uniform curve shall be three(3)points. Additional point references shall be recorded along other non-uniform or irregular features as may be required in order to clearly define the feature. NOTE In addition to those :requirements for documenting sewer and drainage facilities in the field book, those cases where an item cannot ade uatel be referenced and described, shall also be-presented • in the field book;'as a sketch Where appropriate, more than one view of the item shall.be°provided 3.4.2 Streets : USE LINE CONNECTIVITY (a) Improved Roadway: Record points along the baseline (conventional survey), centerline joint of existing pavement, at the flowline of gutters, back of curblines (or edge of pavement), at the ends of all paved returns, sidewalks, and at the points of intersection (P.I.'s)of gutter lines of all intersecting streets and driveways. Those gutter P.I.'s at street intersections may be collected outside of the normal connectivity routine. Record additional points and note the location of those dissimilar, yet abutting surface materials. (b) Unimproved Roadway: In addition to that stated under(a), Record points along the edges of the traveled roadway. Locate and record roadway shoulders, drainageway toes and embankments. All surfacing materials shall be appropriately noted. Line connectivity is not required along shoulders or along the tops or toes of banks. 3.4.3 Driveways : USE LINE CONNECTIVITY (a) Improved Drive: Record points on flares, approaches (aprons)and at the ends of returns. Record all points of intersection (P.I.'s) with the edges of the driveway extended to the edge (or curb) and to the gutter line of the roadway (excluding the flares, approaches or aprons). Record all existing joint lines of those concrete driveway approaches or aprons located within the public right of way. At all residential concrete drives, record all joint locations between the edge (or curb) of the roadway and the garage floor. (d) Unimproved Drive: In addition to the (P.I.'s)as stated under (a), Record points along the edges of the traveled drive at maximum ten (10)foot intervals, between the edge of the roadway and continuing to the garage floor. - 3.4.4 Sidewalks and Ramps: USE LINE CONNECTIVITY (a) Sidewalks: Record points along all edges of sidewalks, at corners, changes of direction aid where required to define curves and at junctions with other features such as driveways, ramps, and steps. (b) Ramps: Record all perimeter points defining.the complete ramp location and include those points which_ represent the actual ramped surface. 4111 8 97ddss0801 city project no. X.X. XX-X 8/26/97 • 3.4.5 Walls and Steps: Line Connectivity NOT REQUIRED (a) Walls : Record points that define the beginning and the end of the wall and additional points along the wall to represent it's alignment or location. Multiple level or terraced walls shall be presented as multiple walls, each level of wall shall indicate a separate wall from those of other levels. Points of reference on walls shall be recorded on the exposed face of the wall, at the bottom(BOT)and at the TOP. At the first point, or the beginning of the wall, include a notation of type or material composition. Where the thickness at the top of the wall is readily discernible, it shall be recorded by appropriate notation. Certain concrete and similar type wall construction, where the top of the wall has been formed with a sloping or an inclined surface, shall require a second data point be recorded at the top of the wall identifying this feature, i.e. "WALL TOP END 8" CONC"at the'exposed face,and additionally "WALL TOP END 8"BACK" at the back of the wall. (b) Steps : Record all perimeter points which define the actual limits of the steps and any entermediate landings. Where appropriate, include wall and/or handrail points. Each flight or series of steps shall include a notation to indicate the number of risers in that flight. i.e., 11R would indicate a continuous series or flight with 11 RISERS. 3.4.6 Sewers and Storm Drainage : Line Connectivity NOT REQUIRED All manholes shall be referenced to the center of their manhole cover. Invert elevations of all pipes shall be recorded at all inlets and at all manholes. Sketches shall be drawn in the field hook to show and note all pipes, manholes,junctions, inlets and outlets. All pipes shall be presented in diameter inches, and where appropriate (HE) horizontal elliptical , (VE) vertical elliptical, and (LH) low-head arch shall be noted. Rectangular or box configurations shall be presented in decimal feet and shall list both a rise and a span. Size, material, elevations, and direction of flow shall be presented. Inlets in series or on main lines of sewers shall be so noted. All other Storm Drainage facilities, including but not limited to culverts, complete with headwalls, wingwalls, or aprons, drains, flumes and spillways shall be sketched and noted in the field book. Area inlets, catch basins and similar structures that do not substantially meet a current City of Omaha Standard Plate configuration and are not readily identifiable as a City Standard shall not be labeled as such. Those type of items shall be recorded with an appropriate notation and sketched within the field book. 3.4.7 Utilities All manholes, boxes and valves shall be referenced to the center of their cover and the utility identified. Substantial above ground facilities shall be recorded and referenced by no less than their two nearest corners. Where concrete bases or pads exist, these shall be referenced separately and where appropriate, these facilities shall include notation by dimensions, i.e. Length(L), depth or Width(W), and Height(H). 3.4.8 Landscape (a) Trees : Trees shall be measured two(2)feet above ground,presented in diameter inches, and identified as to type or species. Unless otherwise provided for under Section 4 "Survey Scope Modifications", all trees shall be singularly noted and shall not be grouped, lumped or in any other manner presented. (b) Shrubs : Bushes or shrubs shall be recorded separately,except where found to have been placed or allowed to grow in such a manner so as to form a hedge. Other features of plantings, such as flower and vegetable gardens may be presented by perimeter location and notation. 3.4.9 Structures (Buildings) : Line Connectivity NOT REQUIRED All buildings corners (along the abutting side) within the Survey Corridor shall be located and recorded. These structures shall be coded as to type,i.e.,building(BLDG), house(HSE), garage(GAR), shed(SHED),etc. Additionally, those corners of buildings and houses in proximity to and yet beyond the actual Corridor shall also be ( recorded. Where a house number,or address of a building is readily available, it shall be included in the notation. 9 97ddss0801 city project no. X.X. XX-X 8/26/97 • 3.5 Cross Section Data Requirements 3.5.1 General Exhibit 3.5.0 " CROSS SECTIONING GUIDE" has been provided to help illustrate the types and location of recorded elevation being requested and anticipated to fulfill the cross section requirements. Full Width cross sections shall be recorded at maximum fifty(50)foot intervals along the survey corridor and at sag locations in the roadways. These sections shall extend out on either side of the centerline and/or baseline for a minimum distance as specified under Exhibit 3.1.2 " SURVEY CORRIDOR". Intersecting streets and alleys shall be cross sectioned perpendicular to the centerline of that side street or alley for a minimum distance, as measured from the main Corridor centerline, of one-hundred fifty (150) feet. Where Conventional methods are employed the cross section baseline shall be referenced and witnessed to the centerline of the right of way, if not coincident, or to a known previously witnessed data line. This line shall be noted and recorded in the field book. It is important that particular attention be given to the gathering of cross section (data elevation) of all types when Electronic Data Collection is employed. The manner of data gathering shall meet a significantly higher degree of completeness in order to reasonably represent surface definition. The City is relying on this level of completeness as the most important tool for use in TIN editing and DTM development. Electronic Data Collection for presentation in the form of Full Width cross sections is intended to replicate, as closely as is reasonably possible, a series of continuous singular sections. The Surveyor shall make adjustment in the performance of his/her normal data collection routines to assure that every attempt to accommodate the development of Full Width cross sectioning is being carried out. 3.5.2 Curb and Gutter (a) Curb : Data elevations shall be recorded at the Back of the curb. This point reference will help to provide continuity in the presentation of roadway and lane widths, and a more uniform representation of curb and gutter relationships. These Back of curb points shall also serve to define curb height, as these elevations are to be taken on top at the outer edge of the roadway. It will no longer be necessary to log points as Lip Curb, Ramp Curb,Drive-over Curb, etc. as this reference to Back of curb shall represent all such entries. Data points need only be properly located in order to identify the ramping or transitioning of curb heights, i.e. those points that are normally associated with the definition of driveway approaches and wheel chair ramps. By recording both a back of curb and a gutter at the flares and the P.I.s, the identification of these features will be enhanced. NOTE All Back of Curb(BC)data which.are actually collected on the;top.,of an existing curb shall be accompanied by;a corresponding Gutter(GUT)data:point. Those which identify pavements edge will not Transverse joints serve as reasonable points of reference when attempting to replicate Full Width cross sections. Where available, these joints can provide the position and location for collecting a Full Width section. The existence and the spacing of such reference joints could allow collection of data by selecting for example every third or fourth joint as a stationing reference and then repeating this positioning along the survey. The interval location for cross sectioning need only meet the maximum fifty (50) foot distance requirement. Lacking available or reasonable transverse jointing, the Surveyor should implement some method of field marking • which would allow the collection of the Back of curb, Gutters, Centerline Joint, and pavement Crown elevations in some acceptable manner that permits Full Width sectioning. (b) Gutter: Data elevations shall be recorded at the flowline(lowest elevation at curb face)along the roadway's curb and gutter area. These points shall represent an elevation on the roadway surface and shall include those areas through intersections. The collection of data elevation points shall conform to Full Width sectioning as provided for under Section 3.5.1 and also as indicated in (a) above. • 10 • 97ddss0801 city project no. X.X. `GX-X 8/26/97 3.5.3 Driveways Data elevations shall be provided as follows. Record points along the edges of the driveway,between the roadway edge (or curb)and extending to the garage floor. Driveways without garages shall include data to the end of the driveway. P.I. data points shall be provided as stated under Section 3.4.3 Along both edges of improved drives, record points at each corresponding joint line and at all breaks in grade. Multiple-lane residential driveways shall require additional point elevations along subsequent longitudinal joints. Along both edges of unimproved driveways, record points at maximum ten (10) foot intervals and also at all breaks in grade. 3.5.4 Walls and Steps (a) Walls : Data elevations shall be recorded along the exposed face of the wall. Record both a bottom (BOT) and a TOP elevation. Bottom and top point elevations shall (reasonably) represent that same vertical face of the wall at one location. Certain concrete and similar type wall construction, where the wall has been formed with an inclined or sloping top surface, shall require that a second data point be recorded identifying this feature, i.e. "WALL TOP END 8" CONC" at the face, and "WALL TOP END 8" BACK" recorded at the back of the wall. (b) Steps : Data elevations shall be recorded in the following manner. On steps where it is readily apparent that the run is in good condition(true to line and grade), data point elevations need only be taken at the exposed faces of the bottom and top risers. Record two(2)points on each side of each step, as follows : at the bottom(BOT OF TOP) of the top riser, and at the top (TOP OF TOP) of the top riser, also, at the bottom(BOT OF BOT) of the bottom riser, and at the top(TOP OF BOT)of the bottom riser. This will present a total of eight (8)data points for any one continuous run or flight. Additional points shall be recorded to adequately define all landings: 3.5.5 Open Drainageways (a) Large Channels and Ditches : Record cross sections perpendicular(as nearly as possible)to the centerline of the channel or to a baseline which follows and approximates the channel alignment. Cross sections shall be developed in Full Width form. Record points along the channel bottom (both toes)or, along the edges of water on both sides and note its depth. Also include the tops and flowlines of any significantly defined trickle channel, and at all significant breaks in grade along and within the banks. These Full Width cross sections shall be recorded along the length of any creek, channel or ditch that is located within the Survey Corridor and shall be provided at maximum intervals of fifty (50) feet along its length. Record additional points to properly define all other noticeable features, i.e.branch drainageways or ditches, isolated berms,bars,plunge pools,abrupt drops and other significant breaks in grade. Record points identifying the location or limits of in place slope protection measures. Include all fixed physical items and breaks in grade outside (beyond) the top of the bank for a minimum distance of fifty (50) feet. (b) Swales and Small Roadside Ditches : Data elevations shall correspond with and shall meet the data point requirements for Section 3.4.2(b) "Unimproved Roadways", and also shall conform to Full Width cross sectioning as indicated under Section 3.5.1 and as further provided for under (a) above. • 11 97ddss0801 city project no. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA: THAT, the Engineering Services Agreement with Hill-Farrell Associates, Inc., to provide preliminary surveying for project RNC 5588, located between 39th and 42nd Streets and between "T"and "V" Streets,is hereby approved. BE IT FURTHER RESOLVED: THAT, the Finance Department is authorized to pay $24,950.00 from the Sanitary Sewer Improvement Organization 1485, Sewer Revenue Improvement Fund 544.
