The attached proposed Resolution amends a Supportive Housing Program Grant Agreement
approved on June 24, 1997, by Resolution No. 1747, between the City of Omaha and the U.S.
Department of Housing and Urban Development for Fiscal Year 1996 Continuum of Care
Supportive Housing Funding in the amount of $397,630. These supportive housing funds are
being used by Community Alliance Rehabilitation Services for outreach teams and case
management for homeless individuals who are mentally ill. This Amendment changes the
Agreement to permit the creation within this project of a new supportive services activity:
Medical and Psychiatric Services, so as to address the shortage and difficulty in securing both

medications and psychiatric assessment/treatment; and for the use of available funds for the
purchase of a second vehicle for Community Alliance's outreach staff(the original vehicle was
purchased in 1993 and is now in poor mechanical condition).

GRANT AGREEMENT AMENDMENT •

This Agreement is made this 3rd day of March, 2000, by and between the United States Department of
Housing and Urban Development(HUD)and the City of Omaha(the Recipient).
RECITALS
1. HUD and Recipient entered into the Grant Agreement dated June 26, 1997, for project number
NE26B960205 located in Omaha, Nebraska(the Grant Agreement).
2. Under the terms of the Grant Agreement, Recipient received a grant from HUD in the amount of
$ 397,630.00, to be used to carry out the project described in the Grant Agreement over a three (3)
year period.
3. Recipient desires to make a change in the project because there is a need to provide medical and
psychiatric services for clients and to provide a second vehicle for outreach.
4. HUD has reviewed the initial application and the proposed change and has determined that, with the
change, the application ranking would have been high enough to have been competitively selected in
the year the application was initially selected. .
5. The need for assistance for homeless persons continues within the jurisdiction within which the
project is located and the need for the project continues.
6. HUD has reviewed the project and the performance of the Recipient and has determined that the
project is worthy of continuation.

7. HUD and the Recipient have agreed to amend the Grant Agreement to add a medical and psychiatric
services activity; to transfer funds from the mobile outreach, referral and case management activities
and to allow for the purchase of a second vehicle for the project.
AGREEMENTS
1. The Grant Agreement is hereby changed by amending the Supportive Services budget as shown in
Exhibit A attached to this agreement.
2. The effective date of these changes is March 3, 2000.

3. All other provisions of the Grant Agreement remain unamended.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA: -,
WHEREAS, on June 24, 1997, by Resolution No. 1747, the`City Council

Resolution No. 1747 as recommended by the Mayor, between the City of Omaha and the U.S.
Department of Housing and Urban Development, changing the Supportive Services budget as
shown in Exhibit A attached to this agreement to thereby permit medical and psychiatric services
as well as the purchase of a second vehicle for outreach is hereby approved. Funds in the
amount of $397,630 shall be paid from the Continuum of Care Supportive Housing Grant
NE26B960205, Fund 190.
