Grant Agreement approved on June 23, 1998 by Resolution 173 , between the City of Omaha and the US Department of Housing and Urban Development for Fiscal Year 1997 Continuum of Care Supportive Housing Funding in the amount of $172,880.00 (Fund 190, Agency 200, Organization 8013). Mercy Housing Midwest receives these funds as a subrecipient and uses them for a project called "Harrington Homes Transitional Housing," a project that provides safe and affordable housing, case management, and supportive services for homeless families with children. This amendment permits the transfer of this grant to a new project sponsor, the Salvation Army (Organization 8010), replacing Mercy Housing Midwest (Grant Number NE26B970101, Fund 190, FY 1997, Agency 200). RECITALS 1. HUD and Recipient entered into the Grant Agreement dated May 7, 1998, for project number NE26B970101 located in Omaha,Nebraska(the Grant Agreement). 2. Under the terms of the Grant Agreement, Recipient received a grant from HUD in the amount of $ 172,880.00, to be used to carry out the project described in the Grant Agreement over a three (3) year period. 3. Recipient desires to change the sponsor of the project from Mercy Housing Midwest to the Salvation Army. 4. HUD has reviewed the initial application and the proposed change and has determined that, with the change, the application ranking would have been high enough to have been competitively selected in the year the application was initially selected. 5. The need for assistance for homeless persons continues within the jurisdiction within which the project is located and the need for the project continues. 6. HUD has reviewed the project and the performance of the Recipient and has determined that the project is worthy of continuation. 7. HUD and the Recipient have agreed to amend the Grant Agreement to change the sponsor of the project from Mercy Housing Midwest to the Salvation Army. AGREEMENTS 1. The Grant Agreement is hereby changed by replacing Mercy Housing Midwest with the Salvation Army as the project sponsor. • 2. The effective date of these changes is June 27, 2000. 3. Funds in the amount of $172,880.00 shall be paid from the Continuum of Care Supportive Housing Grant NE26B970101 (Fund 190, FY 1997, Agency 200).
