City of Omaha Public Works Director Hal Daub,Mayor Honorable President and Members of the City Council, • Transmitted herewith is a Resolution approving a Price Agreement to run through April 30, 2002, for the purchase of ready mixed concrete, to be delivered in various quantities to job sites throughout the city as needed to repair streets, cuts, pavement panel replacements, and other street repairs requested primarily by the Street Maintenance Division and for such concrete used by other divisions and departments as necessary. The Purchasing Agent will be authorized to issue a purchase order to Ready Mixed Concrete Co. for this material. This Price Agreement was bid on April 11, 2001. Ready Mixed Concrete Co. was the sole supplier of the concrete mix FL65P(L6 1/2) air entrained concrete, used on the city street system. The Public Works Department recommends that the price proposal from Ready Mixed Concrete Co. as a sole source of such material be accepted. The cost of the concrete purchased under this Price Agreement shall be paid from the Street Maintenance Organization 1417, Street and Highway Allocation Fund 103, Agency 140, year 2001 funding; other depailnients or divisions utilizing this Price Agreement will pay for such usage from the respective cost centers. • Ready Mixed Concrete Co. has filed the required annual Contract Compliance Report Form, CC- 1, in the Human Relations Department; as is City policy, the Human Relations Director will review the firm to determine its compliance with Ordinance 28885.
