RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, a Professional Services Agreement with R.W. Engineering & Surveying, Inc. to
provide professional services for OPW 54209, being the 1818 Jaynes Street Fencing Addition
Project, was entered into on September 24, 2021; and,
WHEREAS, surveying services not included in the original agreement are required for the
completion of this project; and,
WHEREAS, R.W. Engineering & Surveying, Inc. has agreed to perform the additional
services needed as detailed in the attached Amendment No. 1 to the Agreement, which by this
reference is made a part hereof, for an additional fee not to exceed $9,850.00, which will be paid
from the Street Allocation Fund 12131, Pavement Maintenance Organization 116158, year 2022
funding.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
THAT, as recommended by the Mayor, Amendment No. 1 to the Professional Services
Agreement with R.W. Engineering & Surveying, Inc. for surveying services on OPW 54209, being
the 1818 Jaynes Street Fencing Addition Project, is hereby approved.
BE IT FURTHER RESOLVED:
THAT, the Finance Department is authorized to pay the additional fee not to exceed
$9,850.00, from the Street Allocation Fund 12131, Pavement Maintenance Organization 116158,
year 2022 funding.

AMENDMENT#1 TO THE
PROFESSIONAL SERVICES AGREEMENT
CITY OF OMAHA

THIS AMENDMENT, made and entered into this q' day of Yi \ ,
by and between the firm of R.W. Engineering& Surveying, Inc.,with of ices at 7525 N.
101 st Street, Omaha,NE 68122, and hereinafter referred to as the"Consultant", and the
City of Omaha,Nebraska, acting by and through its Mayor, hereinafter,referred to as the
"City".
WHEREAS, the City desires to engage the Consultant to amend the Engineering
Agreement for OPW 54209, 1818 Jaynes Street Fencing Addition which was entered into
on September 24, 2021, hereinafter called existing Agreement.
NOW THEREFORE, it is hereby agreed that the existing Agreement be amended as
follows:
1. The existing agreement shall be amended to include additional engineering
work as outlined in the attached"Exhibit B", Schedule of Services.
2. The fee will consist of a compilation of estimated direct salary costs, direct
non-salary costs, indirect salary costs, indirect non-salary costs and direct
salary payroll additives, as provided in Exhibit"B"attached.
3. This modification will increase the existing agreement by$9,850.00.
This AMENDMENT shall be deemed a part of, and be subject to all terms and conditions
of the existing Agreement. Except as modified above,the existing Agreement shall
remain in full force and effect.
IN WITNESS WHEREOF,the parties hereto have caused presents to be executed
by their proper officials thereunto duly authorized as to the dates below indicated.

RE: Professional Surveying Services Proposal
Additional Services for Cul-de-Sac design near 20th & Ogden Street
Dear Mike,
R.W. Engineering & Surveying, Inc. (herein referred to as "RW" or the "Consultant") is pleased to submit our proposal for
professional land surveying and civil engineering services to Omaha Public Works Department, Facilities Management Division
(herein referred to as the "CLIENT)for the above referenced project.
Project Understanding:
RW will provide additional services for the construction of a Cul-De-Sac along Ogden Street and near 20th Street in Omaha, Nebraska,
Scope of services:
1. Preparation of a Preliminary and Final Plat(Per direction of Eric Englund on 01/31/2022)
a. Prepare Land Surveyors Certificate and file with Douglas County Land Surveyors Office.
b. Prepare Preliminary and Final Plat(includes 1 round of Douglas County Surveyors comments and 1 round of City
Planning/Engineers comments).
c. Preliminary and Final Plat will be provided in AutoCAD Civi3D 2022 and pdf format.
d. Set monuments (property corners)at all major corners of the boundary of the property, unless already marked
or referenced by existing monuments.
e. Agency Review/Meetings. Estimate 1 meeting.
2. Public Outreach
a. Prepare letter and notify all neighbors within 300 feet of the project. Includes 1 round of City
Planning/Engineering comments.
b. Setup meeting and attend Public Outreach meeting on March 23, 2022
c. Prepare for and attend Planning Board meeting on April 6, 2022
d. Agency Review/Meetings. Estimate 2 meetings.
3. Revisions
a. Review City Planning, City Council, and Public comments
b. Revisions to the Construction Documents for the Cul-De-Sac and fence construction based on comments above.
c. Agency Review/Meetings. Estimate 2 meetings.

Assumptions/ Exclusions:
1. This proposal does not include any other service not directly identified in this proposal. RW is available to perform any or
all additional services contingent upon the Owner's written requests.
Schedule:
All applications and exhibits shall be completed and submitted to City of Omaha Planning Department by March 7, 2022, for the April
Planning Board meeting.
Fees:
We propose to bill for services on the basis of hourly rates incurred. Our fees will not exceed the amount listed below.
1. Preparation Preliminary/Final Plat $ 4,200.00
2. Public Outreach $ 3,890.00
3. Revisions $ 1,760.00
TOTAL FEE $9,850.00
Additional services beyond our scope of services outlined above will be billed on the basis of hourly rates incurred.
Supplemental Terms and Conditions:
1. Invoices will be submitted monthly around the 25th of the month. All invoices are net 10 days.
2. No amendments or additions to this Agreement shall be made between the parties except in writing.
3. All reports, drawings, specifications, computer files, field data, notes and other documents prepared by RW are
instruments of professional service and shall remain the property of RW. RW shall retain all common law, statutory and
other reserved rights, including, without limitation,the copyrights thereto.
4. This proposal is valid if accepted within thirty days from the date of this proposal.
