Transmitted herewith is a Resolution approving an extension of contract with Envirosolve, LLC
for OPW 50519 for Household Hazardous Waste Transportation and Disposal. The original
contract was approved by the City Council on September 14, 2004 by Council Resolution No.
1153.
The original contract provided for two annual extensions at the option of the City of Omaha with
the City Council's approval. The first extension was exercised by Resolution 1162, adopted
September 27, 2005.
The cost for the continuation of this contract with Envirosolve, LLC will be at the original bid
prices for OPW 50519 and will be paid from the Household Hazardous Waste Facility
Organization 116925, Chemical Disposal Fund 21129, year 2006 expenditure.
The Public Works Department requests your consideration and approval of the attached
Resolution.

Attached below is the response from Enviro-Solve regarding the contract extension. I had asked them to respond
prior to June 30th. There has been no other correspondence other than this. As I mentioned before, Glen did
indicate that they would be happy to extend the contract during a phone conversation we had in May. The
current contract expires 8/31/06. If there is anything I need to do,just let me know.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, Envirosolve, LLC was awarded contract OPW 50519, being
Household Hazardous Waste Transportation and Disposal by Council Resolution No. 1153,
adopted on September 14, 2004; and,
WHEREAS, OPW 50519 provides for an initial term of one year and includes
provisions for two one-year extensions at the option of the City; and,
WHEREAS, the Public Works Department recommends the second extension of
OPW 50519, through August 31, 2007.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF OMAHA:
THAT, the contract OPW 50519, being Household Hazardous Waste
Transportation and Disposal with Envirosolve, LLC is hereby extended through August 31,
2007.
BE IT FUTHER RESOLVED:
THAT, the Finance Department is authorized to make payments to Envirosolve,
LLC at the bid prices in OPW 50519 from the Household Hazardous Waste Facility
Organization 116925, Household Chemical Disposal Fund 21129, year 2006 expenditure.
