Attached is an Ordinance authorizing the City of Omaha to accept a grant award in the amount of
$232,330.00 from the Nebraska Emergency Management Agency (NEMA), Sub-grantee Award
Number 2006-GE-T6-0016; Project Title: FY2006 Metropolitan Medical Response System
(MMRS) Grant Program.
NEMA is providing funding for the Omaha Metropolitan Medical Response System (OMMRS).
NEMA funding will ensure the medical community has the necessary equipment, supplies, and
training to respond to an all hazard incident, including those from weapons of mass destruction,
biotenorism and other public health threats.
The Sub-grantee period of performance is August 30, 2006 to June 30, 2008. The grant is for a
two-year period commencing upon final approval. There is no grant match.

AN ORDINANCE approving the acceptance of the Fiscal Year 2006 Metropolitan Medical
Response System (MMRS) in the amount of$232,330.00 from the Nebraska Emergency
Management Agency (NEMA), Sub-grantee Award Number 2006-GE-T6-0016, for the
project period of August 30, 2006 through June 30, 2008, to provided funding for the
Omaha Metropolitan Medical Response System (OMMRS) to ensure the medical
community has the necessary equipment, supplies, and training to respond to all
hazardous incidents, including those from weapons of mass destruction, bioterrorism and
other public health threats; involving appropriations of more than one year in conformity
with Section 5.17 of the Home Rule Charter; providing for payment from a specific
account; and, providing for the effective date hereof.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
Section 1. That, as recommended by the Mayor, approval is hereby given to the Fiscal
Year 2006 Metropolitan Medical Response System (MMRS) Grant Program in the amount of
$232,330.00 from the Nebraska Emergency Management Agency (NEMA), Sub-grantee Award
Number 2006-GE-T6-0016, for the project period of August 30, 2006 through June 30, 2008,
which provides supplemental funding for the Omaha Metropolitan Medical Response System
(OMMRS) to ensure the medical community has the necessary equipment, supplies, and training
to respond to all hazardous incidents, including those from weapons of mass destruction,
bioterrorism and other public health threats; a copy of such grant is attached and by this
reference made a part hereof.
Section 2. The grant period extends for a two-year period commencing upon approval of
this ordinance. In conformity with Section 5.17 of the Home rule Charter, approval of this grant,

1) NEMA herby awards a sub-grant,as described below to the City of Omaha.
2) NEMA shall make payments to the Sub-Grantee as described in Attachment 4. All requests for payment shall be
processed according to the SPARS Grant Management System. Requests that do not include the SPARS summary
sheet and invoice copies,shall be returned to the sub-grantee. Requests for payment and questions concerning
request receipts and payments should be directed to the appropriate party's Financial Contact,as shown in
Attachment 5.
3) Matters concerning the request or negotiation of any changes in the terms,conditions,or amounts cited in this sub-
award agreement should be directed to the appropriate party's Administrative Contact,as shown in Attachment 5.
Any such changes made to this sub-grant agreement require the written approval of each party's Authorized Official,

General Terms and Conditions: ' '±

1. Counties must have completed NIMCAST for FY2006.
2. County must have completed/be current on previous grant
performance/project reporting time lines.
3. LEOP must be current to NEMA FY2006 Planning standards for
review/updating/revision.
4. No communication equipment can be purchased until approved by the
Nebraska Office of the CIO. This will require attendance at the
FY2006 Exercise Planning/Communication Workshop and the
development of a FY2006 Network Communication Integration Plan for
approval.
5. The County PET/Region PET calendar must be current and approved
for FY2006. '
6. The NEMA Grant Management System will be used for submission,
approval, reimbursement and inventory of equipment.
7. No funds can be expended in FY2006 until all past program
performance objectives for MMRS have been submitted and approved
by the State and DHS program officers.
8. The Sub-Grantee will only purchase equipment listed on the FY2006 Authorized
Equipment List found at http://www.rkb.mipt.org.
9. Prior approval by NEMA for any changes of this award will be required. Approval for
changes must be requested of the Grantee Administrator and will be awarded when
written or email verification is sent.
10.Equipment will be purchased by a single entity in the county in accordance with the
approved fiscal policy of the receiving jurisdiction. The entity who will be ordering the
equipment must fill out Attachment 5 and return a copy to the name listed as the NEMA
Grant Administrator on Attachment 5.

Sub-Grant Agreement Attachment 2
11.Sub-grant agreements will not be terminated except by circumstances beyond the
control of either NEMA or the sub-grantee, by mutual agreement or material breach of
the agreement by the Sub-grant recipient. Failure of the Sub-Grantee to comply with
any terms and conditions of the grant agreement shall be deemed a material breach of
the grant agreement. Failure of the Sub-Grantee to remedy such breach within thirty
days after written notice from NEMA, the Grantee shall have the right to terminate the
agreement.
12.Title to equipment costing $5,000 or more and acquired by the sub-grantee with funds
provided under this award shall vest in the sub-grantee. After the end of the grant
period, if the equipment is no longer needed by the sub-grantee, and the per unit fair
market value is less then $5,000, the sub-grantee may retain, sell, or dispose of the
equipment with no further obligation to NEMA. If, on the other hand, the per unit fair
market value is $5,000 or more, then the sub-grantee must submit a written request to
NEMA for disposition instructions.
13.The U.S. Audit Act requires that non-Federal entities that expend $500,000 or more in a
year in Federal awards shall have a single audit conducted in accordance with OMB A-
133.200 Subpart B (b) except when they elect to have a program-specific audit
conducted in accordance with paragraph (c) of Section A-133. A copy of these audits
must be sent to NEMA thirty (30) days upon receipt by the sub-grantee. (see OMB
- Circular A-133)

SPECIAL CONDITIONSr°
1. The sub-grantee agrees to complywith the financial and administrative
requirements set forth in the current editionfof the Office of Grant
o
Operations Financial Management Guide.

2. The sub-grantee agrees.to comply with the organizational audit
requirements of OMB Circular A-133, Audits of States, Local
Governments, and Non-Profit Organizations.
3. Sub-grantee understands and agrees that it cannot use any federal funds,
either directly or indirectly, in support of the enactment, repeal,
modification or adoption of any law, regulation or policy, at any level of
government, without the express prior written approval of G&T.
4. The sub-grantee further agrees to comply with the standards put forth in
OMB Circular A-87, Cost Principles for State, Local, and Indian Tribal
Governments.
5. When implementing G&T-funded activities, the sub-grantee must comply
with all federal civil rights laws, to include Title VI of the Civil Rights Act, as
amended. The sub-grantee is required to take reasonable steps to ensure
persons of limited English proficiency have meaningful access to language
assistance services regarding the development of proposals and budgets
and conducting G&T funded activities.
6. Sub-grantee acknowledges that G&T reserves a royalty-free, non-
exclusive, and irrevocable license to reproduce, publish, or otherwise use,
and authorize others to use, for Federal government purposes: (a) the
copyright in any work developed under an award or sub-award; and (2)
any rights of copyright to which a sub-grantee or sub-sub-grantee
purchases ownership with Federal support. The sub-grantee agrees to
consult with NEMA regarding the allocation of any patent rights that arise
from, or are purchased with, this funding.
7. The sub-grantee agrees that all publications created with funding under
this grant shall prominently contain the following statement: "This
document was prepared under a grant from the Office of Grants and
Training, U.S. Department of Homeland Security. Points of view or
opinions expressed in this document are those of the authors and do not
necessarily represent the official position or policies of the Office of Grants
and Training or the U.S. Department of Homeland Security, nor the
Nebraska Emergency Management Agency."

8. The sub-grantee also agrees that, when practicable, any equipment
purchased with grant funding shall be prominently marked as follows:
"Purchased with funds provided by the U.S. Department of Homeland
Security."
9. The sub-grantee agrees to cooperate with any assessments, national
evaluation efforts, or information or data collection requests, including, but
not limited to, the provision of any information required for the assessment
or evaluation of any activities within this project.
10.The sub-grantee agrees that federal funds under this award will be used to
supplement, but not supplant, state or local funds for homeland security
preparedness.
11.The sub-grantee agrees that all allocations and use of funds under this
grant will be in accordance with the Fiscal Year (FY) 2006 Homeland
Security Grant Program (HSGP) Guidance and Application Kit. Allocations
and use of grant funding must support the goals and objectives included in
the State and/or Urban Area Homeland Security Strategies. Allocations
and use of grant funding must also support the Investments identified in
the Investment Justifications which were submitted as part of the FY 2006
HSGP application.
12.FY 2006 HSGP Sub-Grantees may only fund Investments that were
included in the FY 2006 Investment Justification that was submitted to
NEMA.
13.The sub-grantee is prohibited from transferring funds between programs

Categorical Exclusion for Nebraska Sub-Grantees
The U.S. Department of Homeland Security (DHS), Office of Grants and Training (G&T)
must consider the environmental impacts of a grant action under the National
Environmental Policy Act (NEPA). NEPA requires that any federally funded grant
activity be reviewed for potential environmental impact within the place of performance
of the project. As stated in the assurances agreed to by your organization at the time of
award, recipients of the Fiscal Year 2006 Homeland Security Grant Program, Urban
Areas Security Initiative, Law Enforcement Terrorism Prevention Program, Citizens
Corps Program, or Metropolitan Medical Response System grants are subject to
compliance with NEPA.
DHS has identified several categories of actions that do not individually or cumulatively
have a significant impact on the human environment and therefore do not require an

Environmental Assessment (EA) or Environmental Impact Statement (EIS). These
categorical exclusions allow grantees to avoid unnecessary analysis, process, and
paperwork and concentrate their resources on those proposed actions having real
potential for environmental concerns. For an action to be categorically excluded,
grantees must satisfy three conditions:
1. The entire action must clearly fit within one or more of the categories of
excludable actions listed in DHS Management Directive 5100.1, Environmental
Planning. Program.
2. The scope of the action has not been segmented to be a small piece of a
larger action in order to avoid the appearance of significance.
3. No extraordinary circumstances with potentially significant impacts relating to
the proposed action exist.
Activities that involve greater potential for environmental effect require a Record of
Environmental Consideration. Projects in the following categories require grantees to
complete a NEPA Compliance Checklist addressing the environmental issues for each
project funded by G&T grants:.

2. Acquisition, installation, maintenance, operation, evaluation, removal, or
disposal of target hardening security equipment, devices, or controls to enhance
the physical security of existing critical assets.
Grantee shall not undertake any construction project without the approval of DHS, as
required by the grant guidance.
Use of HSGP funds for construction is prohibited; however, for those projects that
specifically address enhanced security at critical infrastructure facilities, such as
improved perimeter security, minor construction or renovation necessary for guard
facilities, fencing, and related efforts, project construction and renovation not exceeding
$1,000,000 is allowable, as deemed necessary by the Secretary of Homeland Security.
Such construction and renovation shall be strictly limited and allowable when it is a
necessary component of a security system. Written approval must be provided by G&T
prior to the use of any HSGP funds for construction or renovation.
Activities conducted using G&T grant funding that require specific documentation of
NEPA compliance must be coordinated between the grantee, NEMA and the G&T
Preparedness Officer.

1. A copy of each invoice:..received by the Sub-grantee will be sent to the NEMA Grant
Administrator as proof of actual price.
2. The Sub-Grantee will send a copy of the invoice and SPARS payment summary sheet
to the NEMA Grant Administrator. All equipment on the invoices will be cross-
referenced in the SPARS system
3. The NEMA Grant Administrator will compare the invoice to the approved equipment list
and process the payment with the NEMA fiscal officer.
4. The payment will be sent to the Sub-grantee in the form of electronic transfer. If your
jurisdiction does not have an account with the State of Nebraska, fill out the ACH
Enrollment form (Attachment 5) and return to the Grantee Administrator with the signed
award document and the copy of Attachment 3. If a paper check is desired for amounts
under $75,000, please contact Jackie Maly, Federal Aid Administrator for NEMA, to
make arrangements.
5. We anticipate that the transfer of money from the State will be processed and received
before the invoice becomes due, so the Sub-grantee will not be required to front any
money.
6. Jurisdiction is required to complete and submit the Biannual Strategy Implementation
Report (BSIR) each July 15 and January 15 until the end of the grant. The report will
include a narrative summary on the progress of each project. A final BSIR is due 90
days after the end of the grant award period. The BSIR is accessed through the on line
Grant Reporting Tool at www.reporting.odp.dhs.gov.

I.BACKGROUND
IA: Provide a summary description of this Project and Its purpose.(Not to exceed 200 words)
This project is requested to purchase cots and supplies for use at shelters or Alternate Care Sites in the Omaha Metropolitan area in the event
mass care is necessary. When Hurricane Katrina evacuees presented in the Omaha area we discovered that we have a limited source for
obtaining the cots necessary to set up an emergency shelter.Having cots available will allow for timely and effective set up of Mass Care
shelters,as would be the situation during a Pandemic Influenza Epidemic.Medical surge is defined as the capability to provide triage and
then to provide medical care.This includes providing definitive care to individuals at the appropriate clinical level of care and at the
appropriate time to minimize medical complications.Being able to triage and care for those involved quickly with experienced personnel and
having proper equipment available will prevent additional problems from arising.Having the necessary equipment will provide our workers the
tools necessary to perform their duty and to set up stations to triage and care for the patients.
I.B: Explain how this Project will support the State Initiative of the 2008 Enhancement Plan.(Not to exceed 300 words)
This project supports the Tri-County Urban Area Homeland Security Strategy.and the National Priorities by preparing the Omaha Metropolitan
Medical Response System(OMMRS)to be able to respond to any disaster through an all-hazards approach when the medical community
would need to implement the decontamination process.Being able to have trained personnel practice using the equipment during planned
exercises will be beneficial in responding to the Caring for displaced persons or caring for those who are ill.Prioritizing the areas where
special assistance would be needed in advance will provide a coordinated and focused response.OMMRS jurisdictional area covers five
counties in Nebraska and two counties in Iowa.This project will create the need to work in a regional collaboration and have a good
interoperable communications plan in place so that efforts to coordinate the set up of Red Cross Shelters or Alternate Care Facility sites will I
be handled efficiently.

This project supports the state strategy goals and objectives by enhancing our capabilities to decontaminate victims in inclement weather.
The state goals and objectives that we support are as follows: Goal A. Emergency Operation Plans will be used as the basis for planning,
training and exercise activities. Goal G. Enhance capabilities to detect,identify,verify,respond to and recover from a health related terrorist
incident. (Strengthen medical surge and mass prophylaxis capabilities) Objective G 5. Identify equipment needs to recover from a public
health and medical event of national significance. Lessons learned from hurricane Katrina has shown a vulnerability for hospitals to evacuate.
This project will enhance our ability to evacuate the non-ambulatory patient without injury to the patient or staff.

The combined population of this area is approximately 798,977. The geographical area is located in the east-central portion of Nebraska and
includes the west-central portion of Iowa.The area is serviced by two interstates and a major airport.It contains a mix of urban and rural
residents;numerous Fortune 500 companies,and the U.S.Strategic Air Command is headquartered at Offutt Air Force base in Bellevue
Nebraska.OMMRS has worked with the Iowa counties for some time and realize that if something happens in Council Bluffs it will affect
Omaha and if something happens in Omaha it will affect Council Bluffs for that reason and due to the hospitals in that area being affiliated
with the health care System's in Omaha we have from the beginning of OMMRS worked and planned together.The three newest counties
added to OMMRS were Washington,Dodge and Saunders.We have been working closely with them to help with resources.The main goal is

Project#:2 RETURN EQUIPMENT PET NEXT SUMMARY
I.BACKGROUND
LA: Provide a summary description of this Project and its purpose.(Not to exceed 200 words)
This project will provide Omaha Metropolitan area hospitals with tents,heaters and related supplies to facilitate decontamination during
inclement weather conditions as was recommended in the"Operation Secure Metro"Exercise-After Action Report.
To further enhance our decontamination capabilities,a Medical Reserve Corps(MRC)decontamination team will be trained.This will ensure a
cadre of professionals is available to assist the hospitals and the American Red Cross Heartland Chapter in decontaminating individuals prior to
admittance into the hospitals and the ARC shelters.Being prepared to decontaminate individuals in every situation is critical to preventing
unnecessary contamination of persons and sites.Medical surge is defined as the capability to provide triage and then to provide medical care.
This includes providing definitive care to individuals at the appropriate clinical level of care and at the appropriate time in order to minimize
medical complications.Having the capacity to triage and decontaminate patients quickly using the proper equipment and trained,experienced
personnel will lessen the likelihood of medical complications.Providing our professionals the training and tools necessary will allow them to safer

This project supports the Tri-County Urban Area Homeland Security Strategy and the National Priorities by preparing the Omaha Metropolitan
Medical Response System(OMMRS)to be able to respond to any disaster through an all-hazards approach when the medical community would
need to implement the decontamination process.Being able to have trained personnel practice using the equipment during planned exercises
will be beneficial in responding to the decontamination of persons involved in a chemical or radiological incident. Prioritizing the areas where
special assistance would be needed in advance,will provide a coordinated and focused response.OMMRS jurisdictional area covers five
counties in Nebraska and two counties in Iowa.This investment will create the opportunity to work in a regional collaboration and create a good
interoperable communications plan for coordination of the decontamination process for all hospitals and American Red Cross Shelters in the
OMMRS area.

This project supports the state strategy goals and objectives by enhancing our capabilities to decontaminate victims in inclement weather.The
state goals and objectives that we support are as follows: Goal A. Emergency Operation Plans will be used as the basis for planning,training
and exercise activities. Goal G. Enhance capabilities to detect,identify,verify,respond to and recover from a health related terrorist incident.
(Strengthen medical surge and mass prophylaxis capabilities) Objective G 5. Identify equipment needs to recover from a public health and
medical event of national significance. Lessons learned from hurricane Katrina has shown a vulnerability for hospitals to evacuate. This project
will enhance our ability to evacuate the non-ambulatory patient without injury to the patient or staff. Objective G 6. (Decontamination).

OMMRS has been involved with the Urban Area Security Initiative(UASI)working group and the Public Health Section for that funding source.
We have also been involved with the HRSA Hospital Preparedness Grant funding.MMRS funds have been leveraged with these grants since
they have been available to us.We will continue to work to make sure that the funding is
coordinated to meet the State and Local UASI Strategy.

Project#: 1 EQUIPMENT PET NEXT SUMMARY
I.BACKGROUND
I.A: Provide a summary description of this Project and its purpose.(Not to exceed 200 words)
The purpose of this project is to provide the Omaha Metropolitan area hospitals with the EvacuSleds necessary to evacuate the non-ambulatory
patients from their facilities.
From lessons learned with Hurricane Katrina and the problems encountered with evacuation of patient's,hospitals have realized their risks and
vulnerabilities.Based upon assessment of the metro area it has been determined that hospitals in this area need training and equipment to
safely and quickly be able to move patients in the event that their facility needs to be evacuated.Being prepared to evacuate the non-
ambulatory patients is critical in saving life.Medical surge is defined as the capability to provide triage and then to provide medical care.This
includes providing definitive care to individuals at the appropriate clinical level of care and at the appropriate time to minimize medical
complications.Being able to triage and evacuate patients quickly with experienced personnel will prevent additional medical injuries to the non-
ambulatory patient.Having the necessary equipment will provide the worker the tools necessary to safely perform the duty without injury to self.
I.B: Explain how this Project will support the State initiative of the 2006 Enhancement Plan.(Not to exceed 300 words)
This project supports the Tri-County Urban Area Homeland Security Strategy and the National Priorities by preparing the Omaha Metropolitan
Medical Response System(OMMRS)to be able to respond to any disaster through an all-hazards approach when the medical community would
need to evacuate patients.Being able to have trained personnel,and practice using the equipment with planned exercises,will be beneficial in
responding to the implementation of the evacuation plan.Having prioritized the areas that special assistance would be needed in advance will
provide a coordinated and focused response.OMMRS jurisdictional area covers five counties in Nebraska and two counties in Iowa.This
investment will create the need to work in a regional collaboration and have good interoperable communications plan for coordination of the
evacuation process.

This project supports the state strategy goals and objectives by enhancing our capabilities and reducing our vulnerabilities for prevention of
injury that could result in the event of an evacuation scenario. The state goals and objectives that we support are as follows: Goal A.
Emergency Operation Plans will be used as the basis for planning,training and exercise activities. Goal G. Enhance capabilities to detect,
identify,verify,respond to and recover from a health related terrorist incident. (Strengthen medical surge and mass prophylaxis capabilities.)
Objective G 5. Identify equipment needs to recover from a public health and medical event of national significance. Lessons learned from
hurricane Katrina has shown a vulnerability for hospitals to evacuate. This project will enhance our ability to evacuate the non-ambulatory
patient without injury to the patient or staff.

The Omaha Metropolitan Medical Response System OMMRS)covers seven counties in Nebraska and Iowa.

Saunders. Population: 20,344; (5)Dodge Population: 36,066
Iowa Counties:(1) Pottawattamie Population:89,236; (2)Harrison Population:15,821
Per the US Census Bureau:The combined population of this area is approximately 798,977.The geographical area is located in the east-
central portion of Nebraska and includes the west-central portion of Iowa.The area is serviced by two interstates and a major airport.It contains
a mix of urban and rural residents;numerous Fortune 500 companies,and the U.S.Strategic Air Command is headquartered at Offutt Air Force
base in Bellevue Nebraska.OMMRS has worked with the Iowa counties for some time and realize that if something happens in Council Bluffs it
will affect Omaha and if something happens in Omaha it will affect Council Bluffs.For that reason,and due to the hospitals in that area being
affiliated with the health care System's in Omaha,we have from the beginning of OMMRS worked and planned together.The three newest count

OMMRS will establish an ongoing plan to exercise with the equipment.No plan is ever finished without making sure that new personnel have
been properly trained on the use of the equipment.As funding is made available we hope to have a supply on each unit to ensure that all non-
ambulatory patients can effectively be evacuated in a timely manner.
