Attached is a Resolution authorizing the City of Omaha to purchase 104,000 3M Respirator
Masks from Life Assist, Inc. in the amount of 72 cents each. Funds for this purchase are from a
Department of Homeland Security grant, FY 2005 Urban Area Security Initiative (UASI), Grant
Award Number 2005-GE-T5-0020 for use by the Omaha Fire Department.
The following bids were received, opened and tabulated on March 28, 2007.

WHEREAS, bids were received and opened on March 28, 2007, for the purchase
of 104,000 3M N95 Respirator Masks for the Omaha Fire Department's use; and,
WHEREAS, the bid of Life Assist, Inc., in the total amount of $74,880.00 is the
lowest and best bid received and meets specifications, attached hereto and by this reference made
a part hereof; and,
WHEREAS, the contractor has on file a current Annual Contract Compliance
Report Form (CC-1), and the Human Rights and Relations Director will monitor the contract to
ensure compliance with the contract compliance ordinance.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF OMAHA
THAT, the bid of Life Assist, Inc., in the total amount of$74,880.00 is accepted
as the lowest and best bid, and the Purchasing Agent is directed to issue a Purchase Order in
accordance with specifications for the purchase of 104,000 3M N95 Respirator Masks for the use
