Transmitted herewith is a Resolution approving Change Order No. 5 for Project SP90-03A,
which is the construction of the Gibson Road Viaduct from 13th Street to Missouri Avenue. City
Council Resolution No. 871 on August 1, 2006 approved Hawkins Construction Company, Inc.
as low bidder for this project. This change order authorizes additional expenses for the
construction of the storm sewer outfall to the Missouri River in accordance with updated
specifications from the U.S. Army Corps of Engineers.
Hawkins Construction Company, Inc. has agreed to perform the services for an additional
$209,302.85. An exact list of changes is included in the attached change order. All other
provisions of the original contract shall remain in full force arid effect. The costs incurred will
be paid from the 2006 Transportation Bond Fund 13182, 2006 Issue Transportation Bond
Organization 117111.
The Public Works Department requests your consideration and approval of the attached

WHEREAS, Hawkins Construction Company, Inc. was awarded a contract by
Resolution No. 871 on August 1, 2006 to construct Project S.P. 90-03A the Gibson Road

$7,366,887.77; and,
WHEREAS, it is necessary to change the materials and configuration for the
storm sewer outfall to the Missouri River in accordance with updated specifications from the
U.S. Army Corps of Engineers; and,
WHEREAS, Hawkins Construction Company, Inc. has agreed to perform the
services listed in the attached Change Order No. 5, which by this reference is made a part hereof,
for an additional fee of $209,302.85, which will be paid from the 2006 Transportation Bond
Fund 13182, 2006 Issue Transportation Bond Organization 117111.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF OMAHA:
THAT, as recommended by the Mayor, Change Order No. 5 to Hawkins
Construction Company, Inc. to change materials and configuration for the storm sewer outfall to
the Missouri River in accordance with updated specifications from the U.S. Army Corps of
