RESOLVED BY THE CITY COUNICL OF THE CITY OF OMAHA: •
THAT, in accordance with the provisions of the Nebraska Liquor Control Act, as
amended, after a hearing held within 45 days after receiving notice and a copy of the license
application and upon due consideration of all standards and criteria provided by law,this
Council hereby recommends that the following application for a Package Liquor License
be granted.

der this Cooperative Agreement or perform any work for which it is not
paid or reimbursed by Federal or State funding; and,
WHEREAS, the City Council approved U.S. Environmental Protection Agency
(EPA) Cooperative Agreement V - 98767501, on November 15, 2005 by Resolution No. 1443
for the period from October 1, 2005 to September 30, 2007 subsequently extended to June 30,
2010; and,
WHEREAS, the approved funding in the original agreement was $351,428.00 which was
subsequently increased to $4,900,000.00; and,
WHEREAS, the EPA has requested further expansion of the City paint stabilization
activity; and,
WHEREAS, the attached Amendment No. 4 increases the total approved funding from
$4,900,000.00 to $11,865,000.00 and increases the portion of this total which has been awarded
to $9,716,428.00; and extends the Project Period to September 30, 2012.

(b) and (c)- Recipients of a Continuing Environmental
Program Grant or other annual reporting grant, agree to create and maintain a bidders list.
Recipients of an EPA financial assistance agreement to capitalize a revolving loan fund also
agree to require entities receiving identified loans to create and maintain a bidders list if the
recipient of the loan is subject to, or chooses to follow, competitive bidding requirements. Please
see 40 CFR, Section 33.501 (b)and (c)for specific requirements and exemptions.
8. Recipient agrees to submit the Federal Financial Report(FFR)form SF-425 to EPA no
later than ninety(90)days after the end of the grant budget/project period. The EPA requires
only the information requested on FFR lines 10d through 10o. A blank FFR is available for
completion on the Las Vegas Finance Center's(LVFC)website:

to: U.S. EPA-Las Vegas FC, P.O. Box 98515, Las Vegas, NV 89193-8515.

Programmatic Conditions 1-5 of the original award remain the same, Condition Number 6 is revised as
follows:

environmental data and within 30 days from the date of this award.
