The attached Resolution expresses the Omaha City Council's approval of a previously approved
Big Red satellite keno location due to a change of ownership, to be operated by EHPV Lottery
Services, LLC, at 5051 S. 108th Street. This location is a business currently operated by Michael
Montgomery and Betty Pestal, a limited liability•,corporation, as Montgomery Kudym LLC., doing
business under the name "Monty G's."
At this location the keno business will be conducted as an ancillary activity to the existing bar and
lounge business. Keno bets will be received and paid at the location but the actual number
selection will continue to be performed at the keno games operated at Peony Park. Additional
information about the location and EHPV's plans are included within the application submitted to
the City.
A liquor license is presently issued to this location.

11.Additional or Conflicting Terms.Any different or additional terms and conditions proposed by Customer as a part,of this transaction are
objected to by Dallas Airmotive,Inc.,unless specifically accepted in writing.
12.Applicable Lawflien Rights.The laws of the State of Texas shall govern this transaction,which is entered into in Dallas County,Texas.
Customer agrees that the work performed hereunder involves part of an aircraft and that Dallas Airmotive,Inc.may lien the work performed or the
aircraft pursuant to the laws of the State of Texas.
13.Assignment. Customer may not assign this transaction in whole or in part,to a third party without the advance written approval of Dallas
Airmotive, Inc. Any such assignment or transfer of Customer's right, title and interest in its property or the work performed shall not relieve
Customer of its obligations hereunder.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA: •
WHEREAS,the City of Omaha has entered into a Lottery Operator's Agreement with
EHPV Lottery Services,LLC,("EHPV")to operate a Big Red satellite keno-type lottery within the=City
of Omaha and areas of Douglas County("the Lottery Operator's Agreement"); and,
WHEREAS, the Lottery Operator's Agreement allows EHPV to receive the City
Council's approval of satellite and other keno locations'o;.be operated in accordance with the Lottery
Operator's Agreement and Keno Application Procedures and Criteria adopted by this Council; and,
WHEREAS,on December 13,2011,EHPV applied for permission to operate a satellite
keno location at 5051 S. 108th Street,a business operated by Michael Montgomery and Betty Pestal,a
limited liability corporation,as Montgomery Kudym LLC.,and doing business under the name"Monty
G's."
NOW,THEREFORE,BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF OMAHA:
THAT,EHPV Lottery Services,LLC,is granted approval to operate a Big Red satellite
keno location at the business operated by Michael Montgomery and Betty Pestal, a limited liability
corporation,as Montgomery Kudym LLC.,and doing business under the name"Monty G's"at 5051 S.
108th Street, in Omaha,Nebraska.
BE IT FURTHER RESOLVED:
THAT,approval is subject to contii eel: ompliance with the law,the Lottery Operator's
