Transmitted herewith is a Resolution approving five Change Orders for OPW 52099, being
Residential and Major Street Resurfacing at Various Locations in the City of Omaha. Resolution
No. 519 on May 3, 2011 approved U. S. Asphalt Co. as low bidder for this project. These
change orders authorize additional expenses for additions to the project not anticipated in the
original specifications with regard to street repairs due to water main breaks and sewer repair.
U. S. Asphalt Co. has agreed to perform the additional work for $350,457.39. The Change
Orders contain the details of the additional work. All other provisions of the original contract
shall remain in full force and effect. The costs incurred will be paid from the City Street
Maintenance Fund 12129, Surface Restoration Organization 116162.
U. S. Asphalt Co. has'filed the required Annual Contract Compliance Report, Form CC-1, in the
Human Rights and Relations Department.
The Public Works Department requests your consideration and approval of the attached
Resolution and Change Orders.

WHEREAS, U. S. Asphalt Co. was awarded a contract by City Council Resolution No.
519 on May 3, 2011, to construct OPW 52099,being Residential and Major Street Resurfacing at
Various Locations in the City of Omaha; and,
WHEREAS, it is necessary to add additional street work and related services due to water
main break and sewer repair; and,
WHEREAS, U. S. Asphalt Co. has agreed to perform the services listed in the attached
Change Order Nos. 5 through 9, which by this reference is made a part hereof, for a total
additional cost of $350,457.39, which will be paid from the City Street Maintenance Fund
12129, Surface Restoration Organization 116162.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF OMAHA:
THAT, as recommended by the Mayor, Change Order Nos. 5 through 9 for U. S. Asphalt
Co. to construct the necessary modifications to OPW 52099 being Residential and Major Street
Resurfacing at Various Locations in the City of Omaha, are hereby approved.
BE IT FURTHER RESOLVED:
THAT, the Finance Department is authorized to pay an additional $350,457.39 for these
changes from the City Street Maintenance Fund. 12129, Surface Restoration Organization
