Jim Suttle,Mayor Public Works Director Honorable President and Members of the City Council, Transmitted herewith is a Resolution approving five Change Orders for OPW 52099, being Residential and Major Street Resurfacing at Various Locations in the City of Omaha. Resolution No. 519 on May 3, 2011 approved U. S. Asphalt Co. as low bidder for this project. These change orders authorize additional expenses for additions to the project not anticipated in the original specifications with regard to street repairs due to water main breaks and sewer repair. U. S. Asphalt Co. has agreed to perform the additional work for $350,457.39. The Change Orders contain the details of the additional work. All other provisions of the original contract shall remain in full force and effect. The costs incurred will be paid from the City Street Maintenance Fund 12129, Surface Restoration Organization 116162. U. S. Asphalt Co. has'filed the required Annual Contract Compliance Report, Form CC-1, in the Human Rights and Relations Department. The Public Works Department requests your consideration and approval of the attached Resolution and Change Orders.
