The attached Resolution approves a Substantial Amendment to the Omaha-Council Bluffs Consortium
Consolidated Submission for Community Development Programs: 2011 Action Plan(passed by Council on
November 23, 2010, Resolution 1356). The 2011 Action Plan establishes the budget allocations for the
Community Development Block Grant (CDBG), the Emergency Solutions Grant (ESG) and the HOME
Investment Partnerships(HOME)grant
The purpose of the Substantial Amendment is to permit the receipt of$118,245.00 in Emergency Solutions
Grant funds allocated the City by the Department of Housing and Urban Development(HUD). A Substantial
Amendment for the receipt of these dollars is required by the Homeless Emergency and Rapid Transition to
Housing(HEARTH)Act signed into law on May 20, 2009.
This Substantial Amendment also permits the allocation of these funds to subrecipients working on the issue of
homelessness. Through a Request for Proposals process,initiated February 21,2012,the City,in collaboration
with"the Metro Area Continuum of Care,selected Heartland Family Service's Rapid Re-housing Program for
the awarding of 2011 Emergency Solutions Grant dollars to be paid from Fund Number 12139,Organization
Number 129028.

Substantial Amendment to the 2011 Action Plan

2. Summary of Consultation Process
On February 28, 2012, the City met with the Executive Director of the Metro Area
Continuum of Care for the Homeless (MACCH). The purpose of this meeting was to discuss
the consultation process that would be initiated the following day with the CoC. The outline
of that process was presented and discussed,with HEARTH Act requirements clarified.
The following day, February 29th, the City met with approximately thirty-five members of
Metro Area CoC, including its Administrative Staff, members of its Board of Directors and
representatives of key agencies, in particular those agencies likely to apply for ESG funding.
At this meeting, the City discussed in detail four topics: 1) the ESG application process, i.e.,
timeline, funding and eligible activities including funding limits on homeless assistance
activities; 2) the 2011 Substantial Amendment, its timeline, required content and who—City
or CoC—is to take responsibility for the initial draft of amendment sections; 3) the 2012
Substantial Amendment (here the discussion was brief, indicating simply that much of what
is required for the 2012 Amendment will be created for the 2011 Amendment); and 4) the
2013-2017 Consolidated Plan with particular focus on the new sections required by the
HEARTH Act.
Determining how to allocate ESG funds for eligible activities: On February 215t, 2012, the
City initiated a Request For Proposals (RFP) process for 2011 (and 2012) ESG funds.
Proposals were due on March 13'h and were shared, when all proposals had been received (by
4:00 p.m. on the 13th), with the CoC application review committee.
Prior to the 13th, MACCH convened a meeting of the CoC to discuss funding priorities. At
that meeting, the CoC decided to prioritize as follows: 2011 funds -- 1) Rapid Re-Housing; 2)
Prevention; and 3) HMIS; 2012 funds -- 60% for emergency shelter; 40% for non-homeless
assistance activities; with non-homeless assistance activities prioritized as follows: 1) Rapid
Re-Housing; 2)Prevention; and 3)HMIS.
The CoC application review committee reviewed proposals and made a recommendation to
the City concerning which proposals it felt should be funded. In making their
recommendation,the review committee indicated why they selected the proposed activities in
the amount specified and explained how their funding priorities were in line with the national
priorities established by HUD in Opening Doors: Federal Strategic Plan to Prevent and End
Homelessness. (See 5b, below.)
The City agreed to allocate ESG funds in accord with CoC recommendations.
Developing the performance standards for activities funded under ESG: At the meeting
on February 29` , the City requested of the CoC that it draft performance standards for
activities funded under ESG. The City reviewed the CoC's draft, asked for additional
information and/or clarification on some standards; received that additional
information/clarification and agreed to accept the standards as stated in question 6 below.

Developing funding, policies, and procedures for the operation and administration of
the HMIS: The City asked the CoC to draft the policies and procedures that are to govern the
operation and administration of its HMIS. Thus, the same process is being followed here as
with performance standards and the determination of ESG allocations: the City requesting a
draft from those working most intimately with the issue at hand and then seeking
understanding, clarification and potentially modification en route to policies and policies that
can be embraced by both City and CoC.
(As background, funding for the CoC's HMIS is applied for via ESG, CoC NOFA, Nebraska
Information Management System, partner agencies and other sources. HMIS system
administration is provided by the Iowa Institute for Community Alliances, a nonprofit agency
using Service Point, Bowman Systems as vendor.
Further, MACCH is a member with the Nebraska Management Information System (NMIS)
and its policies and procedures for NMIS agencies apply to MACCH agencies. NMIS
supports a comprehensive system of care for clients served through universal data sets that
meet regulatory compliance. Collection and dissemination of aggregate data(devoid of client
identifiable information) is available for analysis and is used to evaluate services and identify
gaps in the homeless service system.)
3. Summary of Citizen Participation Process
The City's citizen participation process: Federal regulations require that the City involve
the public in the review of Substantial Amendments to its Consolidated/Action Plan. This
Substantial Amendment (to the 2011 Action Plan) is being published in the Omaha World
Herald on March 26th and posted that same day on the MACCH List serve, thereby marking
the beginning of a thirty day comment period. The City considers any comments or views of
citizens received in writing during this thirty day period. A summary of these comments or
views, and a summary of any comments or views not accepted and the reasons they were not
accepted will be attached to this Substantial Amendment. In addition, the public hearing on
this Substantial Amendment will occur May 1st, the date that the amendment goes before the
Omaha City Council for approval. Public comments will be accepted at that time, as well.
4. Match
Type of cash and/or non-cash resources used as match: Heartland Family Service will use
agency cash to match their award.
Specific amount used as match: $104,111.00
Proposed use of match: Short and medium term financial assistance; housing relocation and
stabilization services.
5. Proposed Activities and Overall Budget
a.Proposed Activity: Heartland Family Service Rapid Re-housing Program
1) Priority Need from Action Plan: Homeless individuals and families, and persons at risk
of becoming homeless.
2) Description of Activity: This program will serve homeless individuals and families
through the provision of short term rental assistance (170 households), medium term
rental assistance (49 households); rental arrears up to 6 months (24 households); rental

homelessness within a year of discharge.

The City chose to fund the Heartland Family Service Rapid Re-Housing Program for two
reasons: first, HUD has encouraged ESG recipients to fund rapid re-housing activities, i.e.,
"HUD is encouraging communities to focus as much of their funding as possible on rapidly
re-housing persons who are literally homeless in order to reduce the numbers of person who
are living in shelters and on the streets, in order to end homelessness in this country
(Emergency Solutions Grant Notice of Funding Allocations and Requirements)"; and second,
the CoC ranked rapid re-housing its number one non-homeless assistance priority and
identified Heartland's program for rapid re-housing funding.
HUD has asked ESG recipients to explain how their funding priorities support the national
priorities established in Opening Doors: Federal Strategic Plan to Prevent and End
Homelessness. That plan holds that"no one should experience homelessness—no one should
be without a safe, stable place to call home."Among the plan's core values are the following:
"Homelessness is unacceptable"; "There are no `homeless people,' but rather people who
have lost their homes who deserve to be treated with dignity and respect"; "Homelessness is

expensive; it is better to invest in solutions"; and "Homelessness is solvable; we have learned
a lot about what works". According to HUD, rapid re-housing is a very promising solution, or
partial solution, in line with its Federal Strategic Plan. In many respects, according to HUD,
rapid re-housing appears to work. This is the reason HUD has encouraged communities to
focus on rapid re-housing. And it is for this reason that the City considers its prioritization of
rapid re-housing consistent with HUD's national funding priorities.
Finally, given that the City's 2011 ESG allocation has available only $104,111 for homeless-
related services, given the cost of administering and monitoring ESG programs, given that the
CoC rated rapid re-housing its number one non-homeless assistance funding priority, and
given that the CoC recommended not allocating to homeless assistance activities the
relatively small amount of 2011 ESG funds available for that purpose but ,instead to allocate
that amount for non-homeless assistance activities, the City felt that the entire allocation of
these funds to a single program—in this case, a rapid re-housing program—was the best use
of available funds. There is no question concerning the need for rapid re-housing in this
community(other needs exist, certainly, but rapid re-housing, as well).
Obstacles to addressing underserved needs in the community: The principle obstacle is
funding. More funding is needed in order to address fully the issue of homelessness in our
community. The following additional obstacles were identified at a CoC general membership
meeting held February 8, 2012.
Housing--
• Lack of Permanent Supportive Housing (PSH) for people without disabilities (Non-
disabled homeless experience barriers to PSH)
• Lack of group living programs focusing on mental health/substance abuse treatment
(for both families and singles)
• Lack of—and need for redesigned—transitional housing
• Lack of deposit assistance for utilities and rent
• Lack of supportive housing, in general
• Lack of facility-based PSH with low or no rent.
• Lack of housing for families with developmentally disabled children
• Lack of rental assistance;the OHA HOME tbra backlog
• Lack of truly affordable housing for people with poor credit/felony record
• Lack of emergency shelter for families with older children
• Lack of housing for those at or below 50%AMI, rents not affordable

• Lack of support services focusing on health and independent living skills for non-
• disabled households in permanent supportive housing

d. Standards for determining the share of rent and utilities costs that each program
participant must pay, if any, while receiving homelessness prevention or rapid re-
housing assistance.
Verification of income occurs prior to approval for financial assistance and again before
approving any additional financial assistance. Documentation of the participant's
expenses, including how the participant is contributing to housing costs will be
determined and on file. This will serve as documentation of need and will determine the
amount of the financial assistance provided. The participant's file also will contain a
plan to sustain housing following financial assistance, including either a plan to increase
income or decrease expenses.
e. Standards for determining how long a particular program participant will be
provided with rental assistance and whether and how the amount of that assistance
will be adjusted over time.
Participants receive approval for the minimum amount of financial assistance necessary
to prevent homelessness and if rapidly re-housed, prevent re-entry into homelessness.
Documentation of a financial need should be kept if the participant's file for each month
of financial assistance received, therefore participants should not be approved for more
assistance than can be justified given their income and expenses at a given time,
generally this should be no more than three months of assistance at a time.
f. Standards for determining the type, amount, and duration of housing stabilization
and/or relocation services to provide a program participant, including the limits, if
any,on the homelessness prevention or rapid re-housing assistance that each program
participant may receive, such as the maximum amount of assistance, maximum
number of months the program participants receives assistance; or the maximum
number of times the program participants may receive assistance.
Participants receive case management services at least once per month for the duration of
assistance. The participant and case manager develop a housing stability plan with
specific goals and referral linkages.
Participant circumstances—along with their performance on the housing stability plan—
determine the type, amount and duration of housing stabilization and/or relocation
assistance that they will receive. However, eighteen months of assistance is the maximum
participants can receive, whether in the form of Prevention or Rapid Re-Housing
services. Referrals occur if more intensive services are needed. Three months of
assistance is typically adequate for successful housing stability. The determination of a
maximum amount of financial assistance that participants can receive and the
determination of the maximum number of times a participant can receive assistance have
not been determined at this time and are awaiting further experience with these programs.

proposal(s)that should be funded to the City.
Prior to the proposal due date, the following process for establishing funding priorities and
evaluating proposals was implemented by the Metro Area Continuum of Care for the
Homeless(MACCH).
A MACCH list serve message was sent as an open invitation to a March 7th meeting at which
CoC representatives would review the ESG allocation process and develop CoC funding
priorities. Twenty-two CoC representatives participated in March 7th discussion. Among the
participants were the following: Jody Jackson and Joanie Spitznagle (Heartland Family
Service), Jessica Jones, Carla Paterson and Michelle Moyes-Dill (Together, Inc.), Jamie
Grayson-Berglund (MACCH Board), Theresa Christiansen (Salvation Army), David
Eberbach, Joyce O'Neil and Rob Leonard (Iowa Institute for Community Alliances —
MACCH HMIS), Jennifer Griffen and Deborah Dancer(Family Housing Advisory Services),
Frankie Holeton (Catholic Charities — the Shelter), Del Bomberger (Stephen Center), Mike
Saklar (Siena/Francis House), Candace Gregory (Open Door Mission), Pat O'Hanlon (One
World),Kris Stapp(Visiting Nurses),Erin Porterfield(MACCH Executive Director)and Erin
Bock(MACCH Assistant Director).
On March 9`h, MACCH convened its ESG Review Team consisting of the following
individuals: Shinobu Watanabe Galloway (UNMC), Mary Lee Fitzsimmons (Community
Advocate), Tina Hochwender (City of Council Bluffs), Marilyn Wegenhaupt (Community
Advocate), Jeannette Winkler (Salvation Army), Jamie Grayson- Berglund (Chamber of
Commerce and MACCH Board), Sara Woods (UNO CPACS), and Linda Twomey (Veterans
Administration). The purpose of the meeting was to discuss the review process. It was
decided that the following would be emphasized when reviewing proposals: 1) performance
outcomes—through they may differ across programs and by program types—must relate to
HEARTH goals; 2) reducing racial disparities is valued, as is 3) linkage to mainstream
resources.
On March 14-15, the Review Team reviewed the applications and identified its funding
recommendations. On March 16th, the MACCH Review Team invited applicants and others
who might be interested to review its funding recommendations and to provide feedback. On
March 16-19, the Review Team, in conjunction with the MACCH Board, finalized its
funding recommendations and notified applicants of its decision. On March 20t, MACCH
provided its funding recommendations to the City.
ESG Priorities: At the March 7`h meeting, it was agreed that 60% of ESG funding—the
maximum amount allowable—should be prioritized for emergency shelter operations and that
the remaining 40% should be prioritized for non-homeless assistance activity in the following
"activity"order: 1) rapid re-housing, 2)prevention, 3)HMIS.
(NOTE: Since the amount available for homeless assistance activity in the second 2011 ESG
allocation was a minimal amount ($4,710), it was recommended that all available dollars be
used to fund non-homeless assistance activity.)
The City, upon reviewing MACCH's priorities and funding recommendations, elected to
accept MACCH's recommendation for the use of 2011 ESG funds. Heartland Family Service
was the only agency recommended for funding in the Rapid Re-Housing category and so, the
City has elected to fund Heartland with available 2011 funds($104.111 —see item 5 above).

8.Homeless Participation Requirement
The City of Omaha, as recipient, is implementing a plan to consult with homeless or formerly
homeless individuals when considering and making policies and decisions regarding ESG-
funded facilities, services, or other assistance.
That plan involves at least one meeting annually with homeless/formerly homeless
individuals to discuss proposed and possible ESG uses. That meeting also will provide the
opportunity to discuss other issues concerning the network of homeless services available in
our community.
The first such meeting is planned for late May and is being coordinated through shelter
providers and the CoC's Consumer Advisory Task Force.

March 16-19, the Review Team, in conjunction with the MACCH Board, finalized its

ESG Certifications
The Emergency Solutions Grants Program Recipient certifies that:
Major rehabilitation/conversion—If an emergency shelter's rehabilitation costs exceed 75
percent of the value of the building before rehabilitation, the jurisdiction will maintain the
building as a shelter for homeless individuals and families for a minimum of 10 years after the
date the building is first occupied by a homeless individual or family after the completed
rehabilitation. If the cost to convert a building into an emergency shelter exceeds 75 percent of
the value of the building after conversion, the jurisdiction will maintain the building as a shelter
for homeless individuals and families for a minimum of 10 years after the date the building is
first occupied by a homeless individual or family after the completed conversion. In all other
cases where ESG funds are used for renovation,the jurisdiction will maintain the building as a
shelter for homeless individuals and families for a minimum of 3 years after the date the building
is first occupied by a homeless individual or family after the completed renovation.
Essential Services and Operating Costs—In the case of assistance involving shelter operations
or essential services related to street outreach or emergency shelter, the jurisdiction will provide
services or shelter to homeless individuals and families for the period during which the ESG
assistance is provided,without regard to a particular site or structure, so long the jurisdiction
serves the same type of persons (e.g., families with children, unaccompanied youth, disabled
individuals, or victims of domestic violence) or persons in the same geographic area.
Renovation —Any renovation carried out with ESG assistance shall be sufficient to ensure that
the building involved is safe and sanitary.
Supportive Services—The jurisdiction will assist homeless individuals in obtaining permanent

WHEREAS,the Homeless Emergency and Rapid Transition to Housing(HEARTH)
Act was signed into law on May 20, 2009; and,
WHEREAS, this law transforms the Emergency Shelter Grant into the Emergency
Solutions Grant and in the process makes available additional funds to cities and other eligible
recipients for the purpose of addressing homelessness; and,
WHEREAS, the City of Omaha is eligible to receive $118,245.00 in Emergency
Solutions Grant funds; and,
WHEREAS,the HEARTH Act requires that a Substantial Amendment to the Omaha-
Council Bluffs Consortium Consolidated Submission for Community Development Programs:2011
Action Plan be approved in order to receive its Emergency Solutions Grant allocation; and,
WHEREAS,the Substantial Amendment permits not only the receipt by the City of
Emergency Solutions Grant funds but also their allocation to subrecipients working on the issue of
homelessness; and,
WHEREAS,the City,through a Request For Proposals process and in collaboration
with the Continuum of Care,selected Heartland Family Service's Rapid Re-housing Program for the
awarding of 2011 Emergency Solutions Grant dollars; and,
WHEREAS, it is beneficial and desirable to provide rapid re-housing, a service
emphasized by the HEARTH Act, to individuals and families experiencing homelessness; and,
WHEREAS, the Planning Department De and the Mayor recommend approval of this
resolution approving the Substantial Amendment to the 2011 Action Plan.
