I I I _ _ EXHIBIT"A" Page of C FUNCTIONAL PROGRAM OMAHA ALZHEIMER'S SPECIAL CARE CENTER OMAHA,NEBRASKA TYPES OF RESIDENTS ADMITTED TO THIS FACILITY: The facility will a 66 bed licensed Memory Care Facility and will specialize in caring for residents with Alzheimer's disease and other related dementias. SERVICES OFFERED: The facility will offer dementia care services in a secure environment to residents in all stages of Alzheimer's disease. Services will include: Specially trained staff to meet the unique needs of Alzheimer's residents; assistance with daily living skills, incontinent programs,housekeeping services,assistance with dining needs,transportation bus, therapeutic social and recreational activities; medication management and assistance with mobility and transferring. STAFFING: Daily staffing includes a full time Administrator, an RN Health Services Director, 24 hours of licensed nursing staff,an eight to one care giver(nursing assistant)ratio on days and evenings,two care givers on nights, Dietary Service manager, cooks, dietary aides, 2 —3 housekeeping and laundry personnel, an Program Director and activity assistant, full time front desk/receptionist and a full time Maintenance director. There are three shifts spanning 24 hours. The maximum number of staff per shift will be 15. RESIDENT ROOMS: There are a total of 38 resident rooms of which 10 are private and 28 are double occupancy. Each resident room as access to a private bathroom. The building contains no complete dwelling units. COMMON AREAS: Resident common areas are available for all residents and families, and all secured outdoor areas are accessible to residents and families. LAUNDRY SERVICE: The resident laundry and facility linens will be done by facility staff in a designated Laundry room.
