Transmitted herewith is a Resolution authorizing the purchase of and payment for thirty-seven
(37) cases of Cyanokit in the amount of$24,050.00. The Cyanokits will be used by the Omaha
Metropolitan Medical Response System (OMMRS), providing an adequate supply of this
antidote to cyanide poisoning.
Meridian Medical Technologies is the sole vendor of the Cyanokits that will allow OMMRS to
treat cyanide poisoning victims. In accordance with the provisions of Section 5.16, Home Rule
Charter, in the event of a sole source vendor, the City Council may, by resolution, authorize the
Purchasing Division to issue a purchase order for the services.
The Cyanokits are budgeted and will be funded by the Nebraska Emergency Management
Agency(NEMA) FY 2010 Metropolitan Medical Response System Grant, Award # 2010-SS-TO-
0013, approved by City Council on August 23, 2011, Ordinance No. 39103, Fund 12151,
Organization 130762.

JUSTIFICATION FOR NON-COMPETITIVE PROCUREMENT
(SOLE SOURCE JUSTIFICATION)
Cyanokits
PARAGRAPH 1: A brief description of the program and what is being contracted.
The Cyanokit's are the antidote for a cyanide poisoning and is
needed in this area due to limited supply. It is important to a victim
for life safety to be treated in a timely manner.
PARAGRAPH 2: Explanation of why a non-competitive contract is necessary, to
include the following:
• Expertise of the contractor.
Meridian is the vendor that Methodist Hospital has a contract
with and that our OMMRS Pharmacy Committee has
purchased items from in previous grant years.
• Management:
The OMMRS Pharmacy committee is responsible for the
distribution and maintaining the inventory list of product
location.
• Responsiveness.
The company has been very responsive with answers to
questions regarding the product and to provide a quote as
requested in a timely manner.
• Knowledge of the program.
The OMMRs Pharmacy Committee is comprised of Licensed
Pharmacist who have the expertise and knowledge of the
product usage and limitations. .
• Experience of personnel.
OMMRS has personnel that is trained in the pharmaceutical
field who are aware of the efficiency of the product and are
available to provide training as needed with the distribution
of the product.The product will be utilized by the trained EMS
personnel and hospital trained personnel.

PARAGRAPH 3: Time Contracts
• When contractual coverage is required and why.
This product is a limited resource in the OMMRS
Jurisdictional area and the purchase oft his product will help
fill a gap in the community in our CBRNE response
capability..
• Impact on program if dates are not met.
This product will be purchased with FY MMRS 2010 grant
funds which the City of Omaha has requested that we spend
the funds as soon as possible. The grant end date is June
30th. 2013.
• How long would it take another contractor to reach the same
level of competence? (Equate to dollars if desired)
Not applicable.
PARAGRAPH 4: Uniqueness
This product is the antidote to be utilized with a cyanide poisoning.
PARAGRAPH 5: Other points that should be covered to make a convincing case.
This product will help in a major event where cyanide exposure
could occur and increase life safety rather than having a .
fatality situation.

This product will not only benefit the personnel that is involved
In trying to save the life of the patient who is contaminated but
will also assist in enhancing the care of personnel that is providing
the service.

Dear Phyllis Dutton:
Nebraska Methodist Hospital (NMH) has purchased drugs for the Omaha Metropolitan
Medical Response System (OMMRS) since 1999 as a courtesy service. In that course
of time we have used three companies to primarily provide drugs for OMMRS:
Amerisource Bergen, LLC out of Kansas City, MO, Dey Pharmaceuticals and Meridian
Medical Technologies out of Columbia, MD. NMH is limited on who they can order
drugs from based on contracts.
In the past NMHS has purchased the product CyanoKit®directly from Dey
Pharmaceuticals. Since 2009, Dey has since sold this product to Pfizer
Pharmaceuticals who now exclusively distribute this product through Meridian Medical
Technologies and Emergency Medical Products, Inc. It is NOT available through
Amerisource Bergen, LLC.
As noted previously, NMH has limited contracts for ordering drugs and Emergency
Medical Products, Inc. is a vendor NOT listed for NMH to use. In addition, the cost of
the CyanoKit®from Emergency Medical Products is $999.35/kit (see page 2) (FYI, cost
was $935.35 in 2011) versus $650.00/kit from Meridian Medical Technologies (see page
3) (FYI, no increase for 2013).
We are requesting to continue to use Meridian Medical Technologies to order
CyanoKit®for OMMRS based on contract availability and costs. Please feel free to
