Transmitted herewith is a Resolution approving Change Order No. 1 for OPW 52305, being the
Burt Izard Pumping Station Junction Box Repair Project. Resolution No. 1471, which passed on
November 6, 2012, approved Neuvirth Construction as low bidder for this project. The change
order authorizes modifications to access hatches, ladders and platforms.
Neuvirth Construction has agreed to perform the additional work for a total of$23,680.00. The
Change Order contains the details of the additional work. All other provisions of the original
contract shall remain in full force and effect. The costs incurred will be paid from the FEMA
Fund 12122, Operations Grants- Other Public Service Organization 132038, Year 2012
expenditure.
Neuvirth Construction has filed the required Contract Compliance Report, Foram CC-1, in the
Human Rights and Relations Department.
The Public Works Department requests your consideration and approval of the attached
Resolution and Change Order.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, Neuvirth Construction was awarded a contract by City Council Resolution
No. 1471 on November 6, 2012, to construct OPW 52305, being the Burt Izard Pumping Station
Junction Box Repair Project; and,
WHEREAS, it is necessary to modify access hatches, ladders and platforms; and,
WHEREAS, Neuvirth Construction has agreed to perform the services listed in the
attached Change Order No. 1, which by this reference is made a part hereof, for a total additional
cost of$23,680.00, which will be paid from the FEMA Fund 12122, Operations Grants- Other
Public Service Organization 132038, Year 2012 expenditure.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF OMAHA:
THAT, as recommended by the Mayor, Change Order No. 1 for Neuvirth Construction to
conduct the necessary modifications to OPW 52305, being the Burt Izard Pumping Station
Junction Box Repair Project, is hereby approved.
BE IT FURTHER RESOLVED:
THAT, the Finance Department is authorized to pay an additional $23,680.00 for these
items which will be paid from the FEMA Fund 12122, Operations Grants- Other Public Service
Organization 132038, Year 2012 expenditure.
