Transmitted herewith is a Resolution approving a change order for OPW 52542, which is for the
Concrete Curb Ramp Construction at Various Locations project. Resolution No. 248 on
February 25, 2014 approved Swain Construction as low bidder for this project. This change
order authorizes additional expenses to construct curb ramps along the West Center Road
corridors recently added to the 2014 resurfacing package.
Swain Construction has agreed to perform all additional work for an increase in fee of
$96,037.55. All other provisions of the original contract shall remain in full force and effect.
The costs incurred will be paid from the 2010 Transportation Bond Fund 13183, Organization
117117.
The Public Works Department requests your consideration and approval of the attached
Resolution and Change Order.

RESOLVED BY THE CITY COUNCIL OF THE CITY OF OMAHA:
WHEREAS, Swain Construction was awarded a contract by City Council Resolution No.
248 on February 25, 2014, to construct OPW 52542, being the Concrete Curb Ramp
Construction at various locations project; and,
WHEREAS, additional resurfacing areas identified for 2014 construction necessitated
increased quantities for the repair and construction of the concrete curb ramps along these areas;
and,
WHEREAS, Swain Construction has agreed to perform this increase in scope for a net
additional cost of $96,037.55, as noted on the attached Change Order No. 2, which by this
reference is made a part hereof, which will be paid from the 2010 Transportation Bond fund
13183, Organization 117117.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF OMAHA:
THAT, as recommended by the Mayor, Change Order No. 2 for Swain Construction to
construct the necessary modifications to OPW 52542, being the Concrete Curb Ramp
Construction at Various Locations project, is hereby approved.
BE IT FURTHER RESOLVED:
THAT, the Finance Department is authorized to pay an additional $96,037.55 for these
changes from the 2010 Transportation Bond fund 13183, Organization 117117.
